Son zamanlara kadar ileri derecede kalça çıkığı olan hastalar cerrahi olarak tedavi edilemez olarak kabul edilirken artık günümüzde gelişen teknolojisine birliktelik göstererek tedavi edilebilmektedir. Bununla birlikte gelişimsel kalça displazili hastalara total kalça protezi uygulamak bilgi, dikkat ve tecrübe gerektiren bir konu olup, en iyi cerrahi teknik imkanla dahi normal bir insanın kalça fonksiyonlarına ulaşılamaya bilinir. Bu tez çalışmamda Süleyman Demirel Üniversitesi Tıp Fakültesi Ortopedi ve Travmatoloji Anabilim Dalı Kliniğimizde Ocak 1999- Kasım 2003 tarihleri arasında total kalça protezi uygulanmış, gelişimsel kalça displazisi zemininde gelişen koksartrozlu Hartofilakidis Tip A, Tip B ve Tip C 'li hastaların değerlendirmesini yapmaya çalıştım. Çalışmamızdaki en önemli ameliyat endikasyonu, kişinin günlük yaşamını bozan ağrı ve fonksiyon kusuru oldu. Bacak kısalığı, topallama şikayetleri ile başvuran hastalara ameliyat endikasyonu koyulmadı. Hastalar subjektif değerlendirme için Vizüel Analog Skalası (VAS) ve fonksiyonel sonuçlar için ise Harris'in Kalça Fonksiyonlarını Değerlendirme Skalasına göre pre ve postoperatif olarak değerlendirildi. Ortalama takip süresi 29 ay olan 34 hastanın ameliyat öncesi Vizüel Analog Skalası (VAS) ortalama 84.7, Harris kalça skorunu 37.2 olup, en son kontrolde Vizüel Analog Skalası ortalaması 36.4, Harris kalça skoru 70.2 olduğu görüldü. Olgularımızın Harris kalça skoruna göre 4'ü mükemmel, 8'sı çok iyi, 16'si iyi ve 6'sı orta olarak değerlendirildi, kötü sonuca rastlanmadı. Hartofilakidis tiplendirilmesine göre daha ağır deformitesi olan hastalarda ağrı yönünden daha az düzelme saptanmıştır. Buna karşın, bu hastalarda fonksiyonel düzelme (Postoperatif Harris skoru) açısından diğer tiplerle kıyaslandığında anlamlı bir fark bulunmadı. Until recently, patients with advanced hip luxation were considered incurable surgically but currently they can be treated by the help of developing technology. Beside this, implantation of total hip prosthesis to patients with developmental hip displacement requires knowledge, attention and experience, even with the best surgical technique it may not be possible to achive full hip function In this thesis, I tried to assess patients with Hartofilakidis Tipe A, Tipe B and Tipe C coxarthrosis which developed on the base of developmental hip displacement and total hip prosthesis was implanted at Orthopedics and Traumatology Clinics of Süleyman Demirel University Faculty of Medicine between January 1999-November 2003. Functional disturbance and pain disturbing daily activities constitute the most important surgery indications in the study. The complaint of shortened leg and claudicating were not considered to be operation indication. Patients were evaluated pre and postoperatively for subjective assessment and functional results according to Visual Analog Scale (VAS) and Harris Hip Function Evaluation Scale respectively. Thirty four patients were mean followed up for 29 months. Mean VAS score was 84.7, Harris hip score was 37.2 before the surgery ; at the final check up mean VAS value was 36.4 and Harris hip score was 70.2. According to the Harris hip score, 4 of the cases were evaluated as perfect, 8 were very good, 16 were good and 6 were moderate respectively; any bad results were not encountered. There was less improvement concerning pain among patients having advanced deformity according to Hartofilakidis classification. However, functional improvement according to postoperative Harris Score compared to other types, there wasn't a significant difference.
