Bu çalışmanın amacı, Isparta bölgesinde obez erişkin kohortunda, oreksin-A hormonunun obezite ile ilişkisini değerlendirmektir. Çalışmaya, Süleyman Demirel Üniversitesi Tıp Fakültesi Uyku Bozuklukları Merkezi'ne ve Isparta Gülkent Devlet Hastanesi Göğüs Hastalıkları Uyku Laboratuvarı'na başvuran 20-59 yaş arasındaki 261 birey dâhil edildi. Vücut kitle indekslerine (VKİ) göre, bireyler beş kategoriye (normal kilolu, fazla kilolu, orta obez, ağır obez ve morbid obez) ayrıldı. Bireylerden alınan plazma örnekleri, ticari olarak temin edilebilen analitik sistemler kullanılarak her bir plazma parametresi analiz edildi. Plazma oreksin-A seviyelerinin VKİ ve C-reaktif protein (CRP) ile negatif korelasyon gösterdiği belirlendi. Orta, ağır ve morbid obez gruplarda, normal ve fazla kilolu gruplara göre oreksin-A düzeyleri anlamlı olarak daha düşük bulundu. VKİ, düşük dansiteli lipoprotein kolesterol (LDL-C) ve CRP ile pozitif korelasyon gösterdi. Orta obez grubun diğer VKİ gruplarına göre karşılaştırıldığında anlamlı olarak yüksek plazma VLDL-C (çok düşük yoğunluklu lipoprotein kolesterol), trigliserid ve total kolesterol seviyelerine sahip olduğu belirlendi. Elde ettiğimiz sonuçlar, oreksin-A'nın periferal olarak etki gösterdiğini doğruladı. Obez bireylerde daha düşük plazma oreksin-A düzeylerinin bulunması, insan enerji metabolizmasının düzenlenmesinde rol oynadığını ve obezitede oreksin-A'nın aktivitesinin bozulduğunu göstermektedir. Obezitenin artmış CRP seviyeleri ile pozitif ilişkili olması, obezite tedavisinin veya sağlıklı kilo kaybının obeziteye bağlı oluşan sistemik inflamasyonun azaltılmasında önemli olduğunu göstermektedir. The aim of this study is to assess the role of orexin-A hormone and its relationship with obesity in cohort of adults with obesity in Isparta region. A total of 261 subject, aged 18-60 years, who applied to Sleep Disorder Center of Medicine Faculty of Süleyman Demirel University were taken into the study. According to their body mass index (BMI), subjects were grouped into five categories (normal weight, overweight, moderately obese, severely obese and morbidly obese). Plasma samples from subjects were assayed for each plasma parameter using commercially available analytic systems. Plasma orexin-A levels correlated negatively with BMI and C-reactive protein (CRP). In moderately obese, severely obese and morbidly obese individuals, orexin-A levels were significantly lower when compared to normal weight and overweight groups. BMI correlated positively with low-density lipoprotein cholesterol (LDL-C) and CRP. In moderately obese subjects have significantly highest plasma very low density lipoprotein cholesterol (VLDL-C), triglyceride and total cholesterol levels compared to other BMI groups. Our results confirmed that orexin-A acts also in a peripheral manner. The fact that lower levels of plasma orexin-A are present in obese individuals implicates that it is involved in the regulation of human energy metabolism, and the activity of orexin-A is disturbed in obesity. Obesity is associated with elevated levels of CRP. The positive associations of obesity with elevated CRP levels suggest the importance of reducing obesity or healty weight loss to prevent elevations in obesity-related systemic inflammation.
