Nefi Kara ÖZETMeningiomlar genellikle tüm intranial tümörlerin %14.3-19'unu oluşturur. Bazı olgularda ölüme kadar klinik bulgu vermeden, sinsi seyredebilir.Olgumuz 62 yaşında, erkek. Bize gelmeden 2 gün önce bening prostat hipertrofisi tanısıyla üroloji kliniğinde öpere edilmiştir. Postoperatif 2. gün şuurunda bozulma olan hastaya, derin koma halinin gelişmesi üzerine yapılan BBT'de, beynin sağ hemisferinde 9x8x5 cm boyutlarında ve yoğun kontrast tutan, çevresi ödemli dev meningiom olduğu görüldü.Değişik nedenlerden dolayı genel anestezi alan bu tip hastalarda yeterli beyin oksijenlenmesinin sağlanama-masına bağlı olarak ya da aşırı sıvı yüklenmesi sonucu tümör çevresinde var olan ödemde artma olur. Bu da sonuç olarak herniasyona kadar giden serebral bulguları ortaya çıkarabilir. Anahtar Kelimeler: Dev meningiom, komputerize tomografi, postoperatif. ABSTRACT INTRACRANIAL GIANT MENINGIOMA: A CASE REPORTMeningiomas accountfor 14.3-19% ofprimary intracranial neoplasms. in some cases it may be asymptomatic until death. Our case is a 62 year old man. He was operated on f ör beningn prostatic hypertrophyin in urology depart-ment two days ago. On the postoperative second day his consciousness state deteriorated and deep coma status devel-oped. Computed Tomography (CT) was performed and in CT a giant meningioma in the right hemisphere of the brain was seen and it was seen and it was 9x8x5 cm in dimension. There was dense retaining ofcontrast in the meningioma and edema around it. in patients who take general anesthesia far different reasons, there is increase in the edema around the tümör because ofinsufficient brain oxygenation ör excessive fluid loading. Finally, itproduces some cere-bral signs which may result in herniation.Key Words: Giant meningioma, computed tomography, postoperative. |
