SüleymanDemirel Üniversitesi TIP FAKÜLTESİ DERGİSİ: 1998 Aralık; 5(4) Kronik Dakriyosistit Olgularında Cerrahi Sonuçlarımız Mustafa Durmuş, Atilla Veli Kuşçu, Ercan Mensiz, Yavuz Bardak, Erdal Aytuluner, Yusuf Özertürk, Kenan Sönmez ÖZETKronik dakriosistit olgularında uyguladığımız çeşitli dakriyosistorinostomi (DSR) yöntemlerinin sonuçlarını irde¬lemek amacıyla retrospektif çalışma planlandı. Süleyman Demirel Üniversitesi Tıp Fakültesi Göz Kliniğinde, 1995-1998 yılları arasında öpere edilen 71 olguya, durumuna göre farklı yöntemler uygulanarak DSR yapıldı. Preoperatif dakriyosistografide kesesi dolan, kese büyüklüğü normal, kanalikül problemi olmayan ve önceden operasyon geçir¬memi^ 47 olgu l .gruba, dakriyosistografide kesesi dolan ancak küçük bulunan 15 olgu 2. gruba, ortak kanalikülde tı¬kanıklık bulunan 2 olgu 3. gruba, önceden operasyon geçirmiş ve başarısız olmuş 7 olgu 4. gruba dahil edildi, l. Gru¬ba Dupuy-Dutemps tekniğine göre DSR, 2 gruba Dupuy-Dutemps DSR+çift tüp entübasyonu, 3 gruba kanalikülodak-riyosistorinostomi (KDSR)+çift tüp entübasyonu, 4. gruba endonazal DSR yapıldı, l. grupta ortalama izleme süresi 13.23±5.99 ay, basan oranı %89.3, 2. grupta ortalama izleme süresi 12.67 ±727 ay, başarı oranı %86.6, 3. grupta ortalama izleme süresi 6.0±424 ay, başarı oram %50, 4. grupta ortalama izleme süresi 13.0±113 ay, basan oranı %57.1 olarak bulundu. Komplikasyon olarak 2 olguda peroperatuvar kanama, l olguda alt kanalikülde yırtık, 2 ol¬guda postoperatif'enfeksiyon, l olguda altpunktumda erozyon ve granulom, 2 olguda burun içindeki alt ostiımda gra-nuloma, 4 olguda keşi yerinde plika teşekkülü, l olguda keşi yerinde fistül gelişti.Daha önce ameliyat olmamış olgularda basan oranlan tatminkar olmakla birlikte, kanalikül problemi olanlar ve nüks dakriyosistit olgularında haşan oranlan düşük bulundu.Anahtar Kelimeler: Dakriyosistorinostomi, Dupuy-Dutemps tekniği, çift tüp uygulaması endonazal dakriyosisto¬rinostomi. ABSTRACT OUR SURGICAL RESULTS İN CHRONIC DACRYOCYSTITIS CASESThe retrospective study was planned ta evaluate our surgical results in dacriocystorhinostomy cases. it consists of 71 consecutive chronic dacriocystitis cases underwent various dacriocystorhinostomy procedures in our depan-ment benveen the years of 1995 and 1998. The cases were grouped in respect to presenting clinical situations, The 47 cases in which lacrimal sac isfilled in dacryocystography with normal size and without canaliculus problem and not operated previously constituted thefirst group. Fifteen cases with small sacs in dacryocystography constituted the se-cond, 2 cases with common canaliculus obstrıtction constituted the third and 7 cases that undenvent unsuccessful sur¬gical interventions previously constituted thefounh group. Thefirst group underwent dacryocystorhinostomy (DCR)with Dupuy-Dutemps technique, second group undenvent DCR with Dupuy-Dutemps technique + double tube imp-lantation, third group undenvent canaliculodacryocystorhinostomy + double tube implantation.fourth group under-went endonasal DCR procedure.Our success rate isfound as 893% after a meanfollow up period of 1323±5.99 months infirst group, 86.6% and 12.671-7.27 months in second group, 50.0% and6.0±424 months in third group and57.1% and 13.0±11.3 months in fourth group respectively. Observed complications are peroperative inferior canalicular tear in öne case, postopera-tive infection in 2 cases, formation ofgranuloma at intranasal inferior ostium of 2 cases, erosion and granuloma of inferior punctum in öne case, skin plication in incision site of 4 cases and woundfistula in öne case. Our results are found satisfactory in previously unoperated cases but low in cases with canaliculus problems and recurrent chronic dacryocystitis.
