SüleymanDemirel Üniversitesi TIP FAKÜLTESİ DERGİSİ: 1995 Haziran; 2(2) Septoplasti Sonrası Ekstrafor ve Merocel Tamponların KarşılaştırılmasıOrhan Gedikli Harun Doğru Murat YarıktaşÖzetSeptum deviasyonu ameliyatlarından sonra yeni oluşturulan septumun tesbiti, kanamanın kontrolü ve septal hematomun önlenmesi için buruna tampon uygulaması yaygın olarak yapılmaktadır. Tamponlar, bu olumlu etkileri yanında, lokal ya da sistemik bazı yan etkilere neden olabilirler. İdeal olan, yan etkileri en'az ve optimal fayda sağlayan tampon tipini kullanmaktır. Bu çalışma İsparta Devlet Hastanesi l.K.B. B. Kliniği ve Süleyman Demirci Üniversitesi Tıp Fakültesi K. B. B. Kliniği'nde septum deviasyonu tanısı konarak Cottle yöntemi ile septoplasti yapılan 43 hastada, postoperatif dönemde kullanılan gaz (Bepanthene^'li ekstrafor) ve Merocefâ„¢ tamponların karşılaştırılması amacı ile gerçekleştirildi. Hastaların hepsi lokal anestezi altında ameliyat edildi. Hastalar ameliyat sonrası huzursuzluk, tamponun neden olduğu komplikasyonlar, nazal solunumun varlığı ve ameliyattan l ay sonra yapılan muayene sonuçları yönünden karşılaştırıldı. Merocel tamponların, uygun olgularda yerleştirilme ve çıkarılmasının hem hasta hem de hekim açısından kolay olduğu, ödem, ağrı, tampon sonrası kanama ve kurutlanmanın anlamlı derecede az olduğu, hastaların % 20'sinde tek veya iki taraflı nazal solunum imkanı sağladığı tesbit edildi. Sonuç olarak, bu avantajlarından dolayı Merocel tamponun gaz tampondan daha kullanışlı olduğu ve kullanımının giderek yaygınlaşacağı kanısına varıldı. Abstract Comparison of Merocelâ„¢ and Gauze Packs After SeptoplastyNasal packing is commonly used to stabilize the newly formed septum, to control bleeding, and t o prevent septal haematoma after septum deviation operations. The packs may cause some local ör systemic adverse effects in addition to these beneflts. it is ideal to use the pack type that provides optimal profıts, with least adverse effects. This study was performed in order to compare Merocel â„¢ and gauze pack (Bepantheneâ„¢ impregnated) types used in postoperative period, on 43 patients who were diagnosed as septum deviation and undervvent septoplasty with the Cottle technigue at the l. E. N. T. Clinic of İsparta State Hospilal and E. N. T. Department of Medical School of Süleyman Demirci University. Ali patients were operated under local anesthesia. At the postoperative period the patients were compared with respect to discomfort, complications caused by the packing, presence of nasal respiration with the packing and the short term results evaluated l month after operation. it was proved that the insertion and the removal of Merocel packs in proper cases were easier for both surgeon and patient and edema, pain, bleeding after removal, and crust formation were significantly less, and unilateral ör bilateral nasal respiration wasprovided in 20 percent of patients. in conclusion, because of these advantages it was considered that Merocel pack was more useful than gauze pack and will be generalized in time.
