Abstract:The patients with chronic obstructive pulmonary diseases (COPD) try to improve their impaired pulmonary functions by their own maneuvers and the most popular maneuvers are prone position and pursed-lip breathing. However the prognosis get worse by apnea even in different body positions, when COPD and the obstructive sleep apnea syndrome (OSAS) are seen together. Also the different body positions used in pulmonary rehablition and surgery can cause serious changes in pulmonary functions of COPD patients. Hence, the suitable positions must be chosen for these patients, while they are given the right positions for the treatment. Our patient is eighty two years old and prefers to sleep on prone position. However, respiratory failure gets worse by apnea during his sleep in different body positions. We want to present our patient followed by the diagnosis of COPD,OSAS and metabolic syndrome as it is interesting and educative. Key words: COPD,OSAS, body positions, pulmonary functions. |

ÖzetKronik obstrüktif akciğer hastaları (KOAH) bozulmuş olan solunum fonksiyonlarını kendi kendilerine doğal olarak keşfettikleri kolaylaştırıcı manevralarla düzeltmeye çalışmaktadır. En sık uyguladıkları yöntem öne eğilerek (pron) solunum yapma ve büzük dudak (balık ağzı solunumu) solunumudur. Fakat KOAH ve obstrüktif uyku apne sendromunun (OUAS) birlikte görüldüğü hastalarda farklı vücut pozisyonlarında bile uykuda apne gelişmesi, hastaların prognozunu kötüleştirmektedir. Bununla birlikte pulmoner rehabilitasyon ve cerrahi işlem uygulanacak olan KOAH'lı hastalarda farklı pozisyonlar hastaların solunum fonksiyonlarında ciddi değişikliklere neden olmaktadır. Bu nedenle KOAH hastalarına pozisyon verdirilirken, hastaların durumlarına göre uygun pozisyonların seçilmesi gerekmektedir. Seksen iki yaşındaki olgumuz uyanıkken pron pozisyonda yatmaktaydı. Buna karşın, uykuda farklı vücut pozisyonlarında apneler gelişmesi nedeniyle solunum yetmezliği derinleşmekteydi. KOAH, OUAS ve metabolik sendrom tanıları takip edilen olgumuzu eğitici ve ilginç olması nedeniyle sunmak istedik.
