Background: To create pneumoperitoneum at the beginning of laparoscopic surgery,several gases have been trialled. The most commonly used of these gases iscarbon dioxide. Although pneumoperitoneum created with carbon dioxide providesan appropriate area for surgeons to perform laparoscopic operations, it canalso cause several unwanted changes to the peritoneal structure. Some of theseside-effects are inevitable but some exhibit changes depending on the method ofapplication. Methods: This review was prepared taking into consideration the previousexperimental and clinical studies in literature related to the effects on theperitoneal structure of pneumoperitoneum created with carbon dioxide. Results: The creation of pneumoperitoneumaffects the peritoneal environment and thereby the organism. This effect iscaused by increased intra-abdominal pressure, the local peritoneal destructiveeffect, carbon dioxide absorption and chemical metabolic changes. Theinsufflation rate and pressure generally make haemodynamic changes. The hypoxiceffect of carbon dioxide, the toxic effect, the dryness and the coldness maycause structural impairment of the peritoneal environment.Conclusions: Laparoscopic surgery is becoming more widespread.Therefore, it is important to be aware of the effects created bypneumoperitoneum, especially in obese patients or those with concomitantdiseases. With this awareness, it will be possible to reduce the effects of theundesired changes wrought by pneumoperitoneum. |

Giriş : Laparoskopikameliyatların uygulanmaya başlamasıyla beraber pnömoperiton oluşturmak içinbirçok gaz denenmiştir. Bunlar arasındahalen en yaygın kullanımda olankarbondioksit gazıdır. karbondioksit ileoluşturulan pnömoperiton laparoskopikameliyatlarda cerrahlara uygun alan oluşturmasına rağmen peritoneal yapı üzerinde birtakım istenmeyen değişikliklere de sebep olmaktadır. Bu yanetkilerden bazıları kaçınılmaz olmasına rağmen bazı yan etkilerde uygulamaşekline bağlı değişiklik gösterebilmektedir .Metod: Karbondioksit ile oluşturulan pnömoperitonunperitoneal yapı üzerine etkilerine ilişkin literatürdeki yapılan deneysel ve klinik çalışmalardikkate alınarak bu derleme hazırlandı. Sonuçlar : Pnomöperiton oluşumu peritonealçevreyi ve dolayısıyla organizmayı etkiler. Bu etkilenim intraabdominal basınçartışı, karbondioksitin lokal peritoneal destrüktif etkileri, karbondioksitemilimi ve kimyasal metabolik değişimlergibi sebeplerle olmaktadır. İnsuflasyonhızı ve basıncı genel olarak hemodinamik değişiklikler yapmaktadır.Karbondioksitin hipoksiketkisi, toksik etkisi, kuruluğu,soğukluğuperitoneal çevrenin yapısal bozulmasına sebep olmaktadır. Tartışma : laparoskopik cerrahi hergeçen gündaha da yaygınlaşmaktadır. Bu ndenle özellikle ek hastalığı olan ya da obezhastalarda pnömoperitonun yaptığı etkiler hakkında farkındalığa sahip olunmasıönemlidir. Bu farkındalık hastaların pnömoperitonun yaptığı istenmeyendeğişikliklerden etkilenimini azaltacağı kanısındayız. |
