dc.description ObjectiveThe aim of this study is to compare the radiologicaland clinical results of patients who underwent anteriorcervical discectomy and fusion, and to determine therelationship between fusion formation and bone mineraldensity.Materials and MethodsA total of 23 patients, who were admitted to the studywith a diagnosis of cervical degenerative disc diseaseand cervical spondylosis at a Faculty of MedicineOrthopedics and Traumatology clinic, were includedin the study. The cases consisted of patients who underwentanterior discectomy and fusion, and did notrespond to medical treatment for at least 3 months.Cervical bilateral radiographs of patients were takenbefore and after the operation and bone mineral density,VAS score values, Neck Disability Index, Ishiharaindex and SF-36 evaluation results were compared atthe end of the 1st, 3rd, 6th and 12th months.ResultsIn the study, the joining rate was found to be 95.6%.In the non-smoking group at the end of the 3rd month,the joining success was reached to 45%, 65% atthe 6th month, and 100% at the 12th month. In thesmoker group, the joining success at the 3rd monthwas 35%, 55% at the 6th month and 85% at the 12thmonth. A statistically significant relationship was observedbetween the changes in the Ishihara indexvalues and the standard of living of patients and functionalresults. The functional results were comparedwith the cervical index, and a significant improvementwas observed in the functional results with a significantcorrection of the cervical index.ConclusionNowadays, the most suitable intervention is consideredto be the anterior approach in midline cervicaldisc hernias. This intervention, which can be performedwith or without fusion, is a situation to be decidedaccording to the patient's stability problem. It can beconcluded that autograft, allograft, cage or plate-screwapplication results require comparison of resultsin long-term follow-up. Moreover, in radiological examinations,if there is a flattening or kyphotic deformityin the inclination of the cervical spine in the sagittalplane, it is difficult to correct the sagittal deformity inthe spondylotic spine, which has been fused in placeswith posterior intervention, and it increases theexisting kyphotic deformity, making posterior cervicaldecompression impossible.
dc.description AmaçBu çalışmanın amacı, anterior servikal diskektomi vefüzyon uyguladığımız hastaların radyolojik ve kliniksonuçlarını karşılaştırmak ve füzyon oluşumu ile kemikmineral yoğunluğu arasındaki ilişkiyi saptamaktır.Gereç ve YöntemÇalışmaya, Celal Bayar Üniversitesi Tıp FakültesiHastanesi Ortopedi ve Travmatoloji kliniğinde servikaldejeneratif disk hastalığı ve servikal spondiloz tanılarıile başvuran toplam 23 hasta dahil edildi. Olgular enaz 3 ay süreyle tıbbi tedaviye yanıt vermeyen anteriordiskektomi ve füzyon uygulanan hastalardan oluştu.Hastaların operasyon öncesi ve sonrası servikal bilateralgrafileri alınarak, 1-3-6. ve 12. ay sonunda kemikmineral yoğunluğu, VAS skor değerleri, Boyun Engellilikİndeksi, Ishihara indeksi ve SF-36 değerlendirmesonuçları belirlenerek karşılaştırıldı.BulgularÇalışmada kaynama oranı %95,6 bulundu 3. aydasigara içmeyen grupta kaynama oranı %45, 6.ayda%65, 12. ayda %100’e ulaşıldı. Sigara içen grupta3.ayda kaynama oranı %35, 6. ayda %55, 12. aydaise % 85 bulundu. Ishihara indeksi değerlerindekideğişim ile hastaların yaşam standardı ve fonksiyonelsonuçlar arasında istatistiksel olarak anlamlı ilişkigözlendi. Hastalarda fonksiyonel sonuçlar ile servikalindeks kıyaslanarak, servikal indeksin anlamlı bir şekildedüzeltilmesi ile fonksiyonel sonuçlarda anlamlıbir iyileşme izlendi.SonuçGünümüzde orta hat yerleşimli servikal disk hernilerindeen uygun girişim anterior yaklaşımdır. Füzyonluya da füzyonsuz yapılabilecek bu girişim hastanınstabilite sorununa göre karar verilecek bir durumdur.Otogreft, allogreft, kafes ya da plak-vida uygulamasonuçları, uzun dönem takiplerde sonuçların karşılaştırmasınıgerekli kılmaktadır. Radyolojik incelemelerdeservikal omurganın sagital plandaki eğiminde düzleşmeveya kifotik deformite varsa posterior girişimile yer yer füzyon olmuş spondilotik omurgada sagitaldeformiteyi düzeltmenin güç olması, mevcut olan kifotikdeformiteyi artırması nedeniyle posterior servikaldekompresyon uygulanamaz hale gelmektedir.
