dc.description Primary gastric tumors are encountered more frequentlythan metastatic tumors. The gastric metastasis of clearcell renal cell carcinoma (CCRC) is very rare. Weherein report a case of gastric metastasis of CCRC.A sixty-year-old man patient underwent a radicalnephrectomy and received adjuvant chemotherapy.Pulmonary metastasis occurred at the second-yearfollow-up, and interferon alpha-2a was used forrelapse. The Computed Tomography examination ofthe patient presented with dyspepsia two years afterpulmonary metastases revealed 11 mm thickness inthe (along) gastric wall. Endoscopic examination wassuggestive of a neuroendocrine tumor.Histopathologic evaluation of endoscopic biopsiesshowed tumoral cell groups in a solid pattern composedof clear, vacuolated cytoplasm. These tumor cellswere positive for RCC, PAX8 CD-10, Vimentin, andimmunohistochemistry findings were similar to theinitial tumor at diagnosis. Finally, the patient wasdiagnosed with CCRC's gastric metastasis and treatedwith a multi-targeted receptor tyrosine kinase inhibitor.
dc.description Primer mide tümörlerine, metastatik tümörlerindendaha sık rastlanır. Berrak hücreli renal hücreli karsinomun(BHRHK) mideye metastazı çok nadirdir. Bu yazıdaBHRHK’un mideye metastazı olgusunu sunduk.Altmış yaşında erkek hasta radikal nefrektomi geçirdive adjuvan kemoterapi aldı. Takibinde ikinci yıldapulmoner metastaz izlendi ve relaps için interferon alfa-2a kullanıldı. Akciğer metastazlarından iki yıl sonradispepsi ile başvuran hastanın bilgisayarlı tomografikincelemesinde mide duvarında (boyunca) 11 mmkalınlık saptandı. Endoskopik inceleme nöroendokrintümörü düşündürdü.Histopatolojik incelemede berrak vakuolize sitoplazmalısolid paternde tümöral hücre grupları gözlendi.Tümör hücreleri RCC, PAX8, CD10, Vimentin ile pozitifolup immünhistokimyasal sonuçlar ilk tümör ilebenzerdi. Sonuç olarak olguya BHRHK’un mide metastazıtanısı verildi ve çok hedefli bir reseptör tirozinkinaz inhibitörü ile tedavi edildi.
