Sağlık sistemleri hem birey ve toplum sağlığı hem de devletlerin gelişmişlik düzeyi açısından önem arz etmektedir. Günümüzde pek çok gösterge üzerinden değerlendirilebilen sağlık sistemleri, başta örgütsel ve finansal yapısı olmak üzere çok sayıda parametre üzerinden mukayese edilebilmektedir. Cumhuriyetin ilan edilmesiyle birlikte Türk sağlık sistemi, yeniden şekillendirilmiş ve günümüze kadar günün koşullarına da bağlı olarak değişen ve dönüşen bir özelliğe sahip olmuştur. Bu noktadan yola çıkılarak kurgulanan bu çalışma, Türkiye’nin yüzüncü yılında Türk sağlık sisteminin yüz yıllık serüveninin betimlenmesini amaçlamıştır. Nitel araştırma yönteminin benimsendiği çalışmada, “Türk sağlık sistemi” fenomeninin derinlemesine incelenmesi amacıyla nitel araştırma desenlerinden fenomenoloji deseni kullanılmıştır. Bununla birlikte çalışma kapsamında doküman incelemesi ve görüşme tekniğinden faydalanılmıştır. Bu doğrultuda yönetim, örgütlenme, hizmet sunumu ve finansman boyutlarında Türk sağlık sisteminin geçmişten bugüne gelişimi ile yüz yıllık süreçteki kırılma noktaları araştırılmıştır. Çalışmada geçmişteki sağlık sitemine dair deneyimlerin aktarılabilmesi amacıyla katılımcılar belirlenirken “50 yaş ve üstü olma” kriteri benimsenmiş, bu amaçla da amaçlı örnekleme yöntemlerinden maksimum çeşitlilik örneklemesi kullanılmıştır. Çalışma sonucunda yönetim boyutu 9 alt tema, örgütlenme boyutu 4 alt tema, hizmet sunumu boyutu 8 alt tema, finansman boyutu ise 12 alt tema altında analiz edilerek Türk sağlık sisteminin 100 yıllık süreci detaylarıyla birlikte incelenmiş ve ortaya konulmuştur. |

Health systems are important in terms of both individual and public health and the level of development of states. Today, health systems, which can be evaluated on many indicators, can be compared on many parameters, especially in terms of organizational and financial structure. With the proclamation of the Republic, the Turkish health system has been reshaped and has had a changing and transforming feature depending on the conditions of the day. Based on this point, the study aims to describe the century-old adventure of the Turkish health system in Turkey's centennial year. In the study, in which the qualitative research method is adopted, the phenomenology design, one of the qualitative research designs, is used in order to examine the "Turkish health system" phenomenon in depth. However, within the scope of the study, document analysis and interview technique are used. In this direction, the development of the Turkish health system from the past to the present in terms of management, organization, service delivery and financing, and the breaking points in the century are investigated. In order to convey the experiences of the past health system in the study, the criterion of "being 50 years and older" is adopted while determining the participants, and for this purpose, maximum diversity sampling, one of the purposive sampling methods, is used. As a result of the study, the management dimension is analyzed under 9 sub-themes, the organizational dimension is under 4 sub-themes, the service delivery dimension is analyzed under 8 sub-themes, and the financing dimension is analyzed under 12 sub-themes, and the 100-year process of the Turkish health system is examined and revealed in detail. |
