Giriş: Kan dolaşımı infeksiyonu (KDİ), mortalitesi yüksek çeşitli klinik tablolara neden olabildiği için acil tanı konulup antimikrobiyal tedaviye başlanması sağkalım açısından önemlidir. Pozitif sinyal veren kan kültürü şişelerinden hızlı antimikrobiyal duyarlılık test (HADT) yöntemi ile ilgili standart öneriler “The European Committee on Antimicrobial Susceptibility Testing (EUCAST)” tarafından 2019 yılında yayımlanmıştır. Bu çalışmada pozitif sinyal vermiş, Gram boyamasında bakteri tespit edilmiş olan kan kültürü şişelerinden EUCAST HADT yöntemi kullanılarak yapılan ADT sonuçlarının standart disk difüzyon (SDD) yöntemi ve BD Phoenix 100 otomatize sistemi ile elde edilen sonuçlarla karşılaştırılması amaçlanmıştır. Materyal ve Metod: BacT/ALERT 3D kan kültürü inkübatöründe pozitif sinyal vermiş 96 kan kültürü şişesinden Gram boyama sonrası HADT işlemi uygulanmış, ayrıca kan örneklerinin katı besiyerlerine inokülasyonları sonrası üreyen izolatlar EUCAST SDD ve BD Phoenix otomatize sistemi ile test edilmiştir. Hızlı antimikrobiyal duyarlılık test sonuçları her iki yöntem ile karşılaştırılarak; kategorik uyum (CA), çok büyük hata (VME), büyük hata (ME), küçük hata (mE) oranları belirlenmiştir. Bulgular: Hızlı antimikrobiyal duyarlılık test sonuçları SDD sonuçları ile %97.5, otomatize sistem sonuçları ile %93.6 oranında uyumlu bulunmuştur. Hızlı antimikrobiyal duyarlılık testi dördüncü, altıncı ve sekizinci saatte otomatize sistem sonuçları ile sırasıyla %1.85, %2.4 ve %2.31 VME oranları gösterirken SDD sonuçları ile sırasıyla %1.32, %1.07 ve %1.06 VME oranları göstermiştir. Karbapenemaz tarama sonuçları değerlendirildiğinde HADT yöntemi 34 izolatın hepsinde otomatize sistem ile uyumlu sonuç vermiştir. Genişlemiş spektrumlu β-laktamaz (GSBL) üretimi açısından 34 izolatın 33 (%97)’ünde SDD ve otomatize sistem ile uyumlu sonuç vermiştir. Pseudomonas aeruginosa ve Enterococcus faecalis izolatlarında HADT tüm saatlerde bütün antimikrobiyal ajanlar için SDD ile %100 uyumlu bulunmuştur. Sonuç: EUCAST HADT yönteminin önemli KDİ etkenleri için 4-8 saat içinde güvenilir ADT sonuçları verebileceği ve mikrobiyoloji laboratuvarlarında kullanılmasının KDİ’lerin tedavilerinin hızlıca yönlendirilmesinde katkı sağlayabileceği gösterilmiştir.
Introduction: Due to the potential for high mortality associated with bloodstream infection (BSI), prompt diagnosis and initiation of antimicrobial therapy are crucial for survival. The European Committee on Antimicrobial Susceptibility Testing (EUCAST) published stan- dard recommendations for the rapid antimicrobial susceptibility test (RAST) method from blood culture bottles showing a positive signal in 2019. This study aimed to compare the AST results obtained using the EUCAST RAST method from blood culture bottles exhibiting a positive signal and bacteria identified through Gram staining with the results obtained by the standard disk diffusion (SDD) method and the BD Phoenix 100 automated system. Materials and Methods: Rapid antimicrobial susceptibility test was applied per EUCAST recommendations. The isolates obtained after inoculation of the blood samples into solid media were also tested with the EUCAST SDD method and the BD Phoenix system. By com- paring the RAST results obtained with both methods, the study determined categorical agreement (CA), very major error (VME), major error (ME), and minor error (mE) rates. Results: The results of the RAST method were found compatible with 97.5% of SDD results, and 93.6% of the automated system results. Rapid antimicrobial susceptibility test demonstrated VME rates of 1.85%, 2.4%, and 2.31% with automated system results at four, six, and eight hours, respectively. In comparison, it showed VME rates of 1.32%, 1.07%, and 1.06% with standard disk diffusion (SDD) results at the corresponding time points. The carbapenemase screening results of RAST were compatible with the automated system for all isolates. In terms of ESBL production, 97% of isolates gave results compatible with SDD and the automated system. In Pseudomonas aeruginosa and Enterococcus faecalis isolates, RAST was 100% consistent with SDD for all antimicrobial agents at all reading times. Conclusion: The EUCAST RAST method can provide reliable AST results within 4-8 hours for important BSI pathogens, and its use in microbiology laboratories can play a crucial role in swiftly guiding treatment decisions.
