Bu araştırmanın amacı, sağlık sistemleri etkinliğini Dinamik ve Dinamik-Ağ VZA yöntemleriyle ölçmektir. Araştırmanın diğer amaçları ise, etkin olmayan ülkelere potansiyel iyileştirme önerileri sunmak, dönemler arası etkinlik değişimlerini saptamak ve etkinliğe etki eden faktörleri tespit etmektir. Araştırmada sağlık sistemlerinin teknik etkinlik skorları Dinamik ve Dinamik-Ağ VZA yöntemleri kullanılarak elde edilmiştir. Dönemler arası etkinlik değişimlerinin tespiti için Malmquist Toplam Faktör Verimliliği (MTFV) kullanılmıştır. Ayrıca etkinliğe etki eden açıklayıcı ve çevresel faktörlerin belirlenmesi için Tobit regresyon analizi kullanılmıştır. Dinamik VZA modelinin araştırma evreni, Dünya Sağlık Örgütü'ne üye 194 ülke oluşturmaktadır. Araştırmada örneklem seçilmemiş, evrenin tamamına ulaşılması hedeflenmiştir. Ancak seçilen değişkenlere ait bazı verilere ulaşılamadığı için 16 ülke kapsam dışına çıkartılmıştır. Geriye kalan 178 ülke analize dâhil edilmiştir. Dinamik-Ağ VZA modelinin araştırma evrenini ise IMF'ye göre gelişmiş ülkeler oluşturmaktadır. Girdi yönelimli sabit getiri ve girdi yönelimli değişken getiri modeline göre Dinamik VZA genel etkinlik skoru ortalaması sırasıyla üst gelirli ülkelerde 0,8865 ve 0,9160; üst-orta gelirli ülkelerde 0,8342 ve 0,8990 ve alt-orta gelirli ülkelerde 0,8503 ve 0,9492 olarak hesaplanmıştır. Malmquist endeksi etkinlik değişimi bulgularına göre, üst gelirli ülkelerde %14,4, üst-orta gelirli ülkelerde %1,9 ve alt-orta gelirli ülkelerde %8,1 artış yaşanmıştır. Tobit regresyon analizi bulgularına göre, sağlık hizmetlerine erişim endeksi ve insani gelişmiş endeksinin sağlık sistemi etkinlik performansını olumlu yönde etkiledeği; sosyo-demografik endeks, alkol tüketimi, tütün kullanımı ve bebek ölüm hızının sağlık sistemi etkinlik performansı üzerinde olumsuz bir etkisi olduğu tespit edilmiştir. Dinamik-Ağ VZA bulgularına göre, IMF'ye göre gelişmiş ülkelerin genel sağlık sistemi etkinlik skoru ortalaması 0,9493, halk sağlığı alt bölümü etkinlik skoru ortalaması 0,9405 ve tıbbi bakım alt bölümü etkinlik skoru ortalaması 0,9581 olarak hesaplanmıştır. Malmquist endeksi etkinlik değişimi sonuçlarına göre, IMF'ye göre gelişmiş 31 ülkeden sadece 6 ülke etkinliğini arttırabilmiştir. Tobit regresyon analizi bulgusuna göre ise, halk sağlığı alt bölümü etkinlik skorları tıbbi bakım alt bölümü etkinlik skorlarına göre genel sağlık sistemi etkinliğini %2,6 daha fazla etkilemektedir. Sağlık sistemleri etkinliğinin arttırılması için sağlık hizmetlerine erişim ve insani gelişmişlik endeksinin arttırılması, sağlığı etkileyen risk faktörlerinin azaltılması oldukça önemlidir. Ülkelerin, genel sağlık sistemi performansını arttırmak için halk sağlığı hizmetlerine öncelik vermesi önerilmektedir. The study aims to measure the efficiency of the healthcare systems with Dynamic and Dynamic-Network DEA methods. Other aims are to offer potential improvement suggestions to inefficient countries, determine the inter-period efficiency changes and identify the factors influencing the efficiency. In the study, technical efficiency scores of the healthcare systems were obtained using Dynamic and Dynamic-Network DEA methods. Malmquist Total Factor Productivity (MTFP) was used to determine the inter-period efficiency changes. Tobit regression analysis was also used to determine the explanatory and environmental factors influencing the efficiency. The target population of the Dynamic DEA model consists of 194 member countries of the World Health Organization. The sample was not selected in the study, and it was aimed to reach the whole target population. However, 16 countries were excluded since some data on the selected variables could not be accessed. According to the IMF, the research population of the Dynamic-Network DEA model consists of developed countries. According to the input-oriented constant return to scale and input-oriented variable return to scale model, the Dynamic DEA overall efficiency score average is calculated as 0.8865 and 0.9160 in upper-income countries, 0.8342 and 0.8990 in upper-middle-income countries, and 0.8503 and 0.9492 in lower-middle-income countries, respectively. According to the Malmquist index efficiency change results, there was an increase of 14.4% in upper-income countries, 1.9% in upper-middle-income countries, and 8.1% in lower-middle-income countries. According to Tobit regression results, the healthcare access index and human development index have positive effects on the healthcare system's efficiency performance; while the socio-demographic index, alcohol consumption, tobacco use, and infant mortality rate had a negative effect on the healthcare system's efficiency performance. According to the Dynamic-Network DEA results, according to the IMF, the general health system's average efficiency score for developed countries was calculated as 0.9493, the public health subdivision's average efficiency score as 0.9405, and the medical care subdivision's average efficiency score as 0.9581. In terms of the results of the Malmquist index efficiency change, only 6 countries out of 31 developed countries were able to increase their efficiency according to the IMF. According to the Tobit regression analysis result, the efficiency scores of the public health subdivision affect the efficiency of the general health system 2.6% more than the efficiency scores of the medical care subdivision. In order to increase the efficiency of the country's healthcare systems, it is required to increase healthcare access and the human development index and to reduce the risk factors affecting health. The countries should give priority to public healthcare services to improve the performance of the general healthcare system.
