GİRİŞ ve AMAÇ: Bakteriyel translokasyon canlı bakterinin, barsak lümeninden barsak dışı sahalara, esas olarak mezenterik lenf nodlarına geçmesidir. Sirozlu hastalarda; mezenter geçirgenliğinin artması ve aşırı bakteri çoğalması sonucu olarak bakteriyel translokasyon riski artmıştır. İntestinal dekontaminasyon ve sistemik sitokin inhibisyonu ile bakteriyel translokasyon önlenebilir. Pentoksifilin sistemik sitokin inhibisyonu yapan bir ilaçtır. Hemorajik şok ve intestinal obstruksiyonda bakteriyel translokasyonu azalttığı gösterilmiştir. Bu çalışmada deneysel siroz modelinde bakteriyel translokasyon üzerine pentoksifilinin etkisinin araştırılması amaçlanmıştır.MATERYAL ve METOD: Wistar-albino tipi ortalama 200 gram ağırlığında 80 erkek erişkin ratın kırkına deneysel siroz oluşturmak için koledok ligasyonu, kırkına laparotomi yapıldı. Yirmi sekiz gün beklendikten sonra siroz oluştuğunu göstermek için ligasyon yapılan gruptan rastgele üç rat sakrifiye edildi, karaciğerleri makroskopik ve histopatolojik olarak sirotikti. Yirmi sekiz gün sonunda ratlar toplam 8 gruba ayrıldı. Koledok bağlanan ve laparotomi yapılan ratlardan birer grup on gün süre ile ilaçsız takip edildi. Birer gruba on gün süre ile peroral 1gr/kg/gün laktuloz, birer gruba on gün süre ile peroral 10 mg/kg/gün pentoksifilin, birer gruba da peroral 1gr/kg/gün laktuloz ile birlikte 10 mg/kg/gün pentoksifilin verildi. Deney sonunda tüm ratlar sakrifiye edildi. Karaciğer (KC), mezenterik lenf nodları (MLN) ve periton çıkartıldı. Kan örnekleri alındı. Doku örnekleri bakteriyel translokasyonu değerlendirmek için eosin metilen blue (EMB) agar ve kanlı agar besi yerlerine ve kan, kan kültürü şişelerine ekildi. Alınan kan örneklerinden TNF-a ELISA yöntemi ile ölçüldü. KC örneklerinin histopatolojik incelemesi yapıldı. BULGULAR: Koledok bağlanan grupta, KC de fibrozis, mononükleer hücre infiltrasyonu ve nekroz sham grubuna göre anlamlı artmıştı (p<0.05) Koledok bağlanıp pentoksifilin verilen grupta bakteri üremesi koledok bağlanıp ilaç verilmeyen gruba göre KC ve kanda anlamlı olarak daha azdı (p=0.015). Mezenterik lenf nodları ve peritonda ise bu azalma istatistiksel olarak tam anlamlı değilse de anlamlıya yakındı (p= 0.061). Laktuloz verilen grupta hiçbir bölgede bakteriyel üremede anlamlı azalma yoktu (p>0.05). Pentoksifilin ile birlikte laktuloz verilen grupta ilaç verilmeyen gruba göre MLN'nda bakteri üremesinde anlamlı azalma vardı (p=0.048). Gruplar arasında üreyen bakterinin türü, koloni sayısı, gram boyanma özellikleri açısından fark yoktu (p>0.05). Grupların serum TNF-a düzeyleri arasında istatistiksel olarak anlamlı farklılık saptanmadı (p>0.05).SONUÇ: Deneysel siroz oluşturulan ratlarda pentoksifilin bakteriyel translokasyonu azaltmaktadır. Pentoksifilin ile BT'un MLN'ndan daha fazla KC ve kanda azalması ve serum TNF-a düzeylerinin değişmemesi etkisini TNF-a dan bağımsız mezenter geçirgenliği değiştirmeden lokal ve sistemik immun defans mekanizmalarını etkileyerek gösterdiğini düşündürmektedir. Deneysel sirozda pentoksifilin ile BT'un azalmasının gösterilmesi klinik çalışmaları cesaretlendirebilir.
INTRODUCTION: Bacterial translocation consists of the passage of viable bacteria from the intestinal tract to extraintestinal sites, mainly mesenteric lymph nodes. In cirrhotic patients the risk of bacterial translocation is increased because of increased mesenteric permeability and intestinal bacterial overgrowth. Bacterial translocation could be prevented by intestinal decontamination and inhibition of systemic cytokines. Pentoxifylline is a drug which inhibits systemic cytokines. It is shown that bacterial translocation has been decreased with pentoxifylline in hemorrhagic shock and intestinal obstruction. The aim of this study is to investigate the effect of pentoxifylline on bacterial translocation in experimental cirrhosis model. MATERIAL AND METHOD: Cirrhosis induced by common bile duct ligation in forty of eigthy Wistar-albino rats weighing average of 200 grams. As control tgroup (sham) laparotomy has been performed to other 40 rats. After 28 waiting days that cirrhosis composed 3 rats randomly sacrified in ligation group. Macroscopic and microscopic development of cirrhotic process were observed. At the end of 28 days animals were divided into eight groups. One of the ligation and sham group has been followed up without any drug treatment. To the other three groups peroral 1gr/kg/day lactulose, peroral 10 mg/kg/day pentoxifylline and peroral 1gr/kg/day lactulose and 10 mg/kg/day pentoxifylline was given separately. The rats were sacrified at the final of experiment. Liver, mesenteric lymph nodes and peritoneum was released. Blood samples were taken. Tissue samples cultured in eosin metilen blue agar and blood agar. Blood cultures were obtained. Serum TNF-a levels were measured by ELISA. Histopathologic examination of liver samples were performed. RESULTS: In ligation group, fibrosis, mononucleer cell infiltration and necrosis were significantly higher than sham group in liver (p<0.05). In the common bile duct ligated and pentoxifylline given group bacterial translocation was significantly lower than the common bile duct ligated and no drug given group at liver and blood (p=0.015). At mesenteric lymph nodes and peritoneum; BT was nearly significantly lower in the ligated and pentoxifylline given group (p=0.061). In the lactulose given group, BT was not low in any area (p>0.05). In the pentoxifylline and lactulose given group BT was significantly lower in the mesenteric lymph nodes than the group that was given no drug (p=0.048). There was no significant difference between groups at bacteria family, colony numbers and gram staining type (p>0.05). TNF-a levels of the groups were not significally different also (p>0.05). CONCLUSION: In the rats with experimental cirrhosis; pentoxifylline reduced the bacterial translocation. It is thought that pentoxyfilline decreases BT independent of TNF-a effect, not by changing mesenteric permeability. Pentoxyfilline decreases BT by acting on local and systemic immune defences as the decrease of BT with pentoxifylline observed was found more in liver and blood than in mesenteric lymph nodes. By showing the reduction of BT with pentoxifylline in experimental cirrhosis model; advanced clinical trails would be encouraged more.
