İskemik inme, insidansı yaşla birlikte artan, genetik ve çevresel faktörlerin etkilediği kompleks multifaktöryel bir bozukluktur. İnmenin yol açtığı yaşamsal ve sosyoekonomik kayıp göz önüne alındığında, inme risk faktörlerinin detaylı olarak belirlenerek, prevalansının ve insidansının azalmasına yönelik çalışmalar temel oluşturmaktadır. Bütün inmelerin % 4-10 kadarı gençlerde görülür. Gençlerde inme etyolojisinin gerçek sıklığını belirlemek oldukça güçtür. Çeşitli çalışmalarda inme etyolojileri arasında büyük farklılıklar görülmektedir. Genç iskemik inmeli hastalarda etiyolojik faktörler ileri yaş popülasyonuna oranla daha heterojen olup vakaların %15-40'ında neden bulunamamaktadır. Yakın geçmişte, protrombotik gen mutasyonlarının gençlerdeki iskemik inme için bir risk faktörü olabileceği ifade edilmiştir. Fakat, bu mutasyonların gerek iskemik gerekse hemorajik inmedeki rolü hala tartışmalı bir konudur. Vaka kontrollü olarak gerçekleştirilen çoğu çalışmada protrombotik gen mutasyonuyla inme arasında müspet bir bağlantının olduğu öne sürülmesine rağmen, bu durum geniş çaplı vaka kontrol çalışmalarında teyit edilmemiştir. Örnek sayısının az olması, etnik grupların ve kullanılan yöntemlerin çeşitliliği, alınan sonuçlardaki farklılıklara katkı sağlamış olabilir. Daha önce gerçekleştirilen çalışmaların çoğu bu protrombotik mutasyonlardan FV Leiden, MTHFR C677T ve Protrombin G20210A üzerinde yoğunlaşmıştır. Bildiğimiz kadarıyla genç iskemik inmeli hastalarda protrombotik gen polimorfizimlerinin çoklu mutasyonlarının sıklığının araştırıldığı çalışma sayısı son derece azdır. Bu çalışmanın amacı, genç iskemik inmeli hastalarda multipl protrombotik gen mutasyonlarının (Faktör V Leiden (G1691A) ve Faktör V H1299R, MTHFR C677T ve MTHFR A1298C, Protrombin G20210A, B-Fibrinojen, PAI-1 4G/4G, PAI-1 4G/5G ve PAI-1 5G/5G, HPA-1 a/a, HPA-1 a/b ve HPA-1 b/b,) kontrol grubuyla karşılaştırmalı olarak sıklığını araştırmaktı. Çalışmaya 45 yaş altında genç iskemik inmeli 53 hasta ve 40 sağlıklı kontrol alındı. Çalışma sonucunda yalnızca MTHFR C677T heterozigot ve homozigot gen mutasyon sıklığını kontrol grubuyla karşılaştırıldığında genç iskemik inmeli hastalarda istatistiksel olarak anlamlı artış bulduk (P<0.05). İlginç olarak bazı protrombotik mutasyonları kontrol grubunda daha sık tespit ettik. Bu sonuçlar tekrar göstermiştir ki genç iskemik inme etiyopatogenezinde protrombotik gen mutasyonlarının etkisi gizemini korumakta ve daha detaylı çalışmalara ihtiyaç duyulmaktadır. Ischemic stroke is a complex and multifactorial disorder which is seen more frequently by age and affected from genetical and environmental factors. When one take into consideration the life and socio-economical disabilities caused by stroke, studies which target to decrease the prevalance and incidence of stroke via determining the stroke risc factors constitue an important base. Of the whole stroke cases, 4-10 percent is seen among the young people. It is very hard to determine the real frequency of the stroke ethiology in youngs. In various studies, large differencies were observed in reasons of the stroke ethiology. Ethiological factors are more heterogeneous in young patients with ischemic stroke, when compared with the older age populations and the real cause can not be found in 15-40 percent of all cases. In recent past, it has been reported that prothombic gene mutations could be a risc factor for the ischemic stroke in young people. However, the role of these mutations in both ischemic and hemorrhagic stroke types is a controversial issue. Although it has been proposed in many case-controlled studies that there was a positive correlation between the prothrombic gene mutation and stroke, this has not been confirmed by the large scaled case control studies yet. Insufficient sample size, diversity of the ethnic groups and methods might contribute to the difference in the reported results. Most of the studies conducted before have been focused on FV Leiden, MTHFR C677T and G20210A mutations. To the best of our knowledge, there is a very little number of studies in which the frequency of the multiple mutations of prothrombic gene polymorphisms is investigated in young patients with ischemic stroke. The aim of this study was to investigate the frequency of the multiple prothrombic gene mutations such as Factor V Leiden (G1691A) and Factor V H1299R, MTHFR C677T and MTHFR A1298C, Prothrombin G20210A, B-Fibrinogen, PAI-1 4G/4G, PAI-1 4G/5G and PAI-1 5G/5G, HPA-1 a/a, HPA-1 a/b and HPA-1 b/b in young patients with ischemic stroke, with respect to the control group. 53 young patients under 45 with ischemic stroke and 40 healthy control individuals were included in this study. At the end of the study, we have found a statistically significant increase only in the frequencies of heterozygote and homozygote MTHFR C677T gene mutations in young patients with ischemic stroke, when compared with the control group (P < 0,05). Interestingly, we have found some prothrombic mutations more frequently in the control group. These findings have indicated again that the effect of prothrombic gene mutations still remains its enigma in the ethiopathogenesis of ischemic stroke and further more detailed studies are required.
