Bu çalışma Isparta'da, sosyoekonomik düzeyi birbirinden farklı olduğu düşünülen 7-17 yaş arası okul çocuklarında, kırsal, kentsel ayırımı yapılarak hipertansiyon prevalansının ve hipertansiyon ile ilgili risk faktörlerinin saptanması amacıyla yapılmıştır. Çalışma, kentsel olarak Isparta İl Merkezi'nde, bir ilköğretim okulunda ve bir lisede , kırsal olarak da Isparta merkeze bağlı Atabey ilçesinde bir ilköğretim okulu ile bir lisede, 7-17 yaş grubu toplam 702 [%31,2'si erkek(219 kişi), %68,8'i kız(483 kişi)] öğrenci ile yürütülmüş olan kesitsel tipte bir araştırmadır. Öğrencilerin tanımlayıcı özellikleri, velilerince veya bizzat kendileri tarafından doldurulan bir anket formundan, antropometrik ölçümleri, kan basıncı ölçümleri ve kan değerleri ölçümleri ise üç araştırmacı tarafından elde edilmiştir. Öğrencilerin kan basıncı ölçümleri 10-15 dakikalık bir dinlenme sonrasında ve oturur pozisyonyonda yapılmıştır. Ayrıca sağ koldan ve yaşa uygun manşon (boy ve genişliği öğrencinin koluna uygun olan) kullanılmıştır. Yapılan ölçümlerden faydalanılarak yaş gruplarına göre kız ve erkeklerde ayrı ayrı sistolik ve/veya diyastolik kan basıncı 95.persentil ve üzeri olan çocuklar hipertansif olgular olarak değerlendirilmiştir. Ölçümlerden elde edilen verilerden SPSS 9.05 paket program kullanılarak istatistiksel analizler yapılmıştır. Buna göre çalışmaya alınan toplam 483 kız öğrenciden 282'nin (%3,9), çalışmaya alınan toplam 219 erkek öğrenciden 11'inin (%1,5) hipertansif olduğu görüldü. Genel Hipertansiyon sıklığı %5,5 (39 öğrenci) olarak bulunmuştur. Hipertansif öğrenciler daha ayrıntılı incelendiğinde bunların 31 tanesi (%4,41) sadece Sistolik HT, 4 tanesi (%0,56) sadece Diyastolik HT ve yine 4 tanesinin de (%0,56) hem Sistolik hemde Diyastolik HT olduğu görülmüştür. Çalışmamızda kan basıncı değerlerinin yaş, boy, kilo artışı ile beraber arttığı görülmüştür. Araştırmada obesite prevalansı erkeklerde %5, kızlarda %11,5 ve genelde %16,5 olarak tesbit edilmiştir. Vücut kitle indeksi %85 ve üzeri olanlar obez kabul edilmiştir. Ülkemizde çocuklarda kan basıncı persentil eğrilerinin belirlenmesi amacıyla daha geniş çaplı araştırmalara ihtiyaç vardır. Çocuklarda HT sıklığı sanılandan daha fazla olabilir, bu nedenle üç yaşından sonra kan basıncı ölçümleri fizik muayenenin bir parçası olmalıdır.
This study is prepared in Isparta city, to determine the prevalance of hypertension and the related risc factors with hypertension, by making town and rural seperation in school children between 7-17 ages, who have different socioeconomic status. Study is applied in Isparta city center as a town, in one primary and one high school; and applied in Atabey district in one primary and one high school as a rural area. This study is a cross sectional invastigation, that is made by total 702 [%31,2 boy (219 people), %68,8 girl(483 people)] students between 7-17 ages. Characteristic features of the students, their anthropometric measures, their blood pressures and their blood levels are held by three (3) investigators; from the questionare which is filled by themselves or their parents. Blood pressure of the students measured in sitting position after they rest 10-15 minutes. And it is measured from the right arm with a cuff suitable with the age. (lenght and wide suitable fort he student) According to our measurements, for the age groups and the sexuality; students that have systolic and diastolic blood pressure equal or higher than 95.percentil are called hypertensive children. Statistical analysis are made by the datas from the measurements by using SPSS 9.05 programme. According to findings; 28 of 483 girl students (%3,9) and 11 of 219 boy students (%1,5) are found hypertensive. General hypertension prevalance is found %5,5 (39 students). When we look at the hypertensive students detailed; 31 of them (%4,41) were only systolic HT, 4 of them(%0,56) were only diastolic HT and again 4 of them (%0,56) were both systolic and diastolic HT. İn the study, it is shown that blood pressure levels are increased with age, lenght and weight. We found the obesity prevalance %5 (35 people) for boys and %11,5 (80 people) for girls and %16,5 (115 people) for the general population. Body mass index equal and above %85 are called obesity. In our country, we need more detailed studies to detect the blood pressure percentile curves for children. Prevalance of HT for children can be more higher than it is thought. Because of this, after age 3, blood pressure measurements must be done for every patient as a part of the normal physical examination.
