Gastrik kanser dünyadaki en sık ve ölümcül malignitelerden biridir. Türkiye'de erkek ve kadınlarda ikinci en sık malignitedir. Gastrik kanserin erken saptanması önemlidir ve çoğu batı ülkesinde 5 yıllık sağ kalım düşüktür. Bu çalışmada Süleyman Demirel Üniversitesi Tıp Fakültesi Patoloji Anabilim Dalı'nda 2000-2006 yılları arasında mide adenokarsinomu tanısı almış 50 olguya ait gastrektomi materyalleri üzerinde çalıştık. Tümörleri Lauren sınıflama sistemine göre intestinal, diffüz ve mikst tip olarak, differansiyasyonlarına göre ise iyi, orta ve az diferansiye olarak sınıflandırdık. Olgularda cinsiyet, yaş, operasyon tipi, invazyon derinliği, metastatik lenf nodu sayısı, metastaz, TNM Evresi, H.pylori, lenfositik infiltrasyon arasındaki ilişkiyi araştırdık. İstatistiksel olarak anlamlı ilişki tespit edilmedi. Klinikopatolojik değişkenleri COX-2 immünreaktivite skoru ile karşılaştırdık. H.pylori varlığı dışında istatistiksel olarak anlamlı bir sonuç yoktur. Olgularımızın çoğunda COX-2'nin kuvvetli eksprese olduğunu saptadık. COX-2'nin ekspresyon skorunun kuvvetli olmasının kötü prognozla ilişkili olduğunu saptadık. Artmış COX-2 ekspresyonun kötü prognoz için bağımsız prognostik faktörler olan yaşla (p=0,039), COX-2 immunoreaktivite skoruyla (p=0,020), epitelyal immunoreaktivite skoruyla (p=0,013), Lauren sınıflama subtipiyle (p=0,037), invazyon derinliğiyle (T skor) (p=0,031), lenf nodu metastazıyla (N skor) (p=0,014) korele olduğunu bulduk. Sonuç olarak COX-2'nin, mide adenokarsinomunda kuvvetli eksprese olduğunu ve yüksek ekspresyonun kötü prognozla ilişkili olduğunu saptadık. Ayrıca H.Pylori varlığı ile COX-2 boyanma skoru arasında korelasyon olduğunu tespit ettik. COX-2 ekspresyonunun mide adenokarsinomu gelişiminde rol alan faktörlerden biri olabileceğini düşündük. Gastric cancer is one of the most frequent and lethal malignancies in the world. It is the second most common malignancy in Turkish males and females. Early detection of stomach cancer is difficult, and in most western countries the 5-year survival is very rare. We have investigated fifty gastrectomy materials diagnosed with "stomach carcinoma" by the Pathology Department of Medical Scholl at Süleyman Demirel University during the period from 2000-2006. Tumors were classified according to the Lauren's classification as intestinal, diffuse and mixt type and also according to their differentiation grade as well, intermediate and poorly differtiated. We investigated the relation between sex, age, type of operation, depth of incasion, number of metastatic lymph node, metastasis, TNM stage, H.pylori, lymphasitic infiltration. In the cases statistical significance was not determined. The clinicopathological variables have been compared with the COX-2 immunoreactivity score. Except the existence of H.pylori, no statistical significance could be found. In most of the cases we have determined that COX-2 has a strong expression. We have found that elevated expression skor of COX-2 is associated with poor prognosis. Upregulated COX-2 expression was also correlated with age (p=0,039), COX-2 immunoreactivity scor (p=0,020), epithelial immunoreactivity scor (p=0,013), Lauren classification subtypes (p=0,037), depth of invasion (T scor) (p=0,031), lymph node metastasis (N scor) (p=0,014), which were an independent prognostic factor for poorer survival. Consequently, we determined that there is a strong COX-2 expression in the gastric adenocarcinoma and strong expression is associated with poor prognosis. Besides, we have found that there is correlation between the existance of H.pylori and the COX-2 immunoreactivity score.
