Doksorubisin geniş spektrumlu antineoplastik etkisi sayesinde gerek erişkin, gerekse çocukluk çağı kanserlerinin tedavisinde yaygın olarak kullanılmaktadır. Doksorubisinin klinik kullanımını kısıtlayan en önemli yan etkisi kardiyotoksisitedir. Kardiyotoksisiteyi önlemek için bir çok ajan denenmiş, ancak etkin ve güvenilir bir ajan bulunamamıştır. Bu çalışmada doksorubisinin kalp dokusunda oluşturduğu toksisiteye karşı antioksidan bir ajan olan N-asetilsistein'in (NAC) koruyucu etkileri araştırıldı. Ratlar "kontrol" (n:10), "doksorubisin" (n:10) ve "doksorubisin ? NAC" (n:10) grupları olmak üzere üç gruba ayrıldı. Kontrol grubuna 0. ve 24. saatlerde gavajla serum fizyolojik (SF) ile 48. saatte intraperitoneal SF; doksorubisin grubuna 0. ve 24. saatlerde gavajla SF ile 48. saatte intraperitoneal doksorubisin (20 mg/kg); "doksorubisin ? NAC" grubuna 0. ve 24. saatlerde gavajla NAC (140 mg/kg) ile 48. saatte intraperitoneal doksorubisin (20 mg/kg) uygulandı. Deney yedinci günde sonlandırılarak kalp dokusunda biyokimyasal ve histopatolojik analizler yapıldı. Kontrol grubuna göre; doksorubisin grubunda MDA yüksek (p<0.05), SOD, CAT ve GSH-Px değerleri anlamlı olarak düşük bulundu (p<0.05), "doksorubisin ? NAC" grubuna ait enzim değerlerinde ise fark yoktu (p>0.05). Doksorubisin uygulanan ratların kalp dokularında, kontrol grubunda görülmeyen ödem, enflamasyon, vakuolizasyon, hemoraji, nekroz ve dağılma şeklinde histolojik değişiklikler saptandı (p<0.05). Doksorubisin grubundaki histopatolojik bozulmaların "doksorubisin ? NAC" grubunda görülmediği belirlendi (p<0.05). Doksorubisin grubunda sol ventrikül duvarı miyofibril kalınlıklarının, kontrol grubuna göre arttığı (p<0.05), "doksorubisin ? NAC" grubuyla kontrol grubu arasında ise fark bulunmadığı görüldü (p>0.05). Bu bulgular doksorubisinin lipid peroksidasyonu ve serbest oksijen radikallerinin sentezini artırıp, antioksidan enzim etkinliğini azaltarak kardiyotoksisiteye yol açtığı, NAC'nin histopatolojik düzeyde kardiyotoksisiteyi önlediğini göstermektedir. Doxorubicin is an antineoplastic agent which is widely used in the treatment of both childhood and adulthood cancer. Cardiotoxicity is the most important side effect of doxorubicin which has been restricted its clinical usage. Although many agents are employed to prevent cardiotoxicity, an efficient and a safe agent has not been found. In this study, we investigated the protective effect of N-acetylcysteine (NAC), an antioxidant agent, against doxorubicin-induced cardiotoxicity on the heart tissue. Rats were divided into three groups as control (n:10), doxorubicin (n:10) and doxorubicin ? NAC (n:10). The control group was administered physiological saline (PS) by gavage at 0. and 24. hours and intraperitoneal PS was given at 48. hour; the doxorubicin group was administered PS by gavage at 0. and 24. hours and intraperitoneal doxorubicin (20 mg/kg) was administered at 48.hour; the doxorubicin? NAC group was administered NAC by gavage (140 mg/ kg) at 0.and 24. hours and intraperitoneal doxorubicin (20 mg/ kg) was administered at 48. hour. The experiment lasted seven days and then biochemical and histopathologic analysis were made on heart tissue. Compared to the control group; M DA was high (p<0.05) and SOD, CAT and GSH-Px values were significantly low in the doxorubicin group (p<0.05) and there was no difference in the enzyme values of the doxorubicin?NAC group (p>0.05). Unlike the control group; there were histological changes (p<0.05) such as edema, inflammation, vacuolization, hemorrhage, necrosis and disorganization in the heart tissues of rats administered doxorubicin. The histopatologic degenerations in the doxorubicin group did not occur in the doxorubicin+NAC group (p<0.005). An increase was observed in the myofibril thickness of the left ventricle wall of the doxorubicin group compared to the control group (p>0.05); and no difference was observed between the doxorubicin?NAC group and the control group (p>0.05). These results show that increasing the synthesis of free oxygen radicals and lipid peroxydation and decreasing the antioxidant enzyme efficacy, doxorubicin causes cardiotoxicity and NAC prevents cardiotoxicity by histopathological evaluation.
