Sepsis, patogenez ve tedavide kaydedilen çok önemli gelişmelere karşın bugün önemini koruyan bir infeksiyon ve infeksiyona reaksiyon tablosudur. Tedavi protokollerine eklenen yeni faktörlere rağmen özellikle tabloya organ yetmezliklerinin de eklendiği septik şok durumunda mortalite en ileri merkezlerde bile çok yüksek seviyelerdedir. Son yıllarda standart sepsis tedavisine APC ve Immünglobulin eklenerek yapılan çalışmalarda mortalitenin azaldığı bildirilmektedir. Bu retrospektif taramamızda standart sepsis tedavisi, standart sepsis tedavisine eklenen APC ve immünglobulinin ağır sepsis olgularında mortalite ve kan gazı değerleri üzerine etkisinin incelenmesi amaçlandı. Ağır sepsis tanısı almış 108 olgu üç gruba ayrılarak incelendi. Grup K standart sepsis tedavisi uygulanmış 66 olgudan oluşmaktaydı. Grup APC standart sepsis tedavisine APC tedavisi eklenmiş 22 olgu, Grup Ig ise standart sepsis tedavisine immünglobulin kompleksi eklenmiş 20 olgudan oluşmaktaydı. Her üç grubun tedaviden 48 saat önceki, tedavi günü ve tedaviden 96 saat sonraki kan gazı değerleri saptandı.Grup K'da mortalite oranı % 84.85, Grup APC'de % 68.2 ve Grup Ig'de % 70 olarak bulundu (p>0.05). Aradaki fark istatistiksel açıdan anlamsızdı. Grup APC ve Grup Ig'de FiO2 değerlerinde tedaviden 96 saat sonra anlamlı düşme saptandı. Grup K olgularda ise FiO2 değerlerinde artış saptandı. Grup APC ve Grup Ig'de PaO2, PaCO2, pH değerlerinin değişimi istatistiksel açıdan anlamsız bulundu. Grup K'da ise tedaviden 96 saat sonra pH'da düşme ve PaCO2'deki yükselme anlamlı idi (p=0.001,p=0.001). Bu bulgularımız; APC ve Ig grubunda kliniğimizde uygulanan tedavi protokolünün, Grup K'ya göre pulmoner fonksiyonlar üzerine olumlu etkilerinin bulunduğunu gösterdi. Today; sepsis is still a critical infection and reaction to the infection besides very important advances in its'pathogenesis and treatment. Moreover to the new factors added to treatment protocols; mortality is very high even in high technology centers, especially for septic shock situations together with organ failures. Through the last years; in the standard sepsis studies together with APC and immunoglobulin, mortality is noted to be declined. İn this retrospective study; we aimed to find out both the effects of standard sepsis treatment and standard sepsis treatment together with APC and immunoglobulins, on mortality and blood gas levels, for serious sepsis cases. 108 cases that are identified as serious sepsis, are examined in 3 groups. 66 cases who are treated by standard sepsis treatment are called as group K. Group APC was developed from 22 cases who are treated with APC teatment added to standard sepsis treatment. Finally, group Ig was formed from 20 cases who are treated with immunoglobulin added Standard sepsis treatment. Blood gas levels 48 hours before treatment, during treatment day and 96 hours after treatment are detected for each groups. Mortality rates are found as; 84.85 % for group K, 68.2 % for group APC and 70 % for group Ig (p>0.05). There was a meaningless statical difference. For groups APC and Ig, FiO2 levels significantly decreased 96 hours after treatment. For group K, FiO2 levels increased. Changes in PaO2, PaCO2, pH levels had no statical meaning for groups APC and Ig. Furthermore; for group K, there was a considerable decrease in pH and increase in PaCO2 after 96 hours from treatment (p=0.001, p=0.001). These findings showed positive effects of the treatment protocols on pulmonary functions that are applied in our clinic for groups APC and Ig respecting to group K.
