Bu çalışmada bölümümüzdeki frozen section yönteminin uygulamasındaki problemleri belirlemek ve kalite kontrolü yapmayı amaçladık. Süleyman Demirel Üniversitesi Tıp Fakültesi Patoloji Anabilim Dalı'nda 2000-2006 yılları arasında yapılan frozen section vakaları retrospektif olarak tarandı. Frozen sectiona gönderilen sistemler, endikasyonlar, frozen section sonucu ile parafin tanıları arasında uyumsuzluk varsa sebepleri araştırıldı. Bu tarihler arasında patoloji laboratuvarımızda toplam 51165 olgu incelenmiştir. Olguların 869'unda frozen section çalışılmıştır. Çalışmamızda frozen section yönteminde doğruluk oranı, spesifite ve sensitivite sırasıyla % 98, % 100, % 93,4 olarak saptanmıştır. Tanıyı erteleme oranı % 5,98, yanlış negatif tanı oranı % 1,95, yanlış pozitif tanı oranı % 0'dır. Yanlış negatif 17 sonucun % 47,05'i makroskopik örnekleme, % 35,30'u patoloğun yorum hatası, % 17,65'i mikroskopik örnekleme hatasından kaynaklanmıştır. En sık frozen section incelemesi istenen bölgeler sırasıyla, kadın genital sistemi, tiroid ve gastrointestinal trakttır. Kadın genital sisteminden serinin % 30,6'sını oluşturan 266 materyal değerlendirilmiş olup doğruluk oranı % 98,5, spesifite % 100, sensitivite % 94 tür. Tiroitten serinin % 10,4'ünü oluşturan 91 materyal değerlendirilmiş olup, doğruluk oranı % 93,4, spesifite % 100, sensitivite % 68'dir. Gastrointestinal trakttan serinin % 10,1'ini oluşturan 88 materyal değerlendirilmiş olup, doğruluk oranı % 98,8, spesifite % 100, sensitivite % 95 dir. Sonuçlarımız frozen section' ın kaçınılmaz bir hata payı olsa da güvenilir bir yöntem olduğunu göstermiştir. Frozen section, sonucu cerrahi prosedürün seyrini değiştirecekse uygulanması gereken intraoperatif konsultasyondur. Patolog ve cerrahın iyi iletişimi, yöntemin endikasyon ve sınırlılıklarının bilinmesi frozen sectionın doğruluk oranını arttıracaktır. Our aim was to define the problems at the application of frozen section and to make quality control at our department. Frozen section cases were retrospectively evaluated at Süleyman Demirel University, Faculty of Medicine, Department of Pathology between 2000-2006. Systems those frozen sections were sent, endications, the cause of discrepancy between frozen section and paraffin sections diagnosis were investigated. Fifty one thousand one hundred and sixty five cases were evaluated at our department during this period. Frozen section was performed at eight hundred and sixtynine of them. Our accuracy rate, spesivity and sensitivity were, 98%, 100% and 93,4%, respectively. The percentage of deferred diagnosis was 5,98%, false negativety was 1,95%, false positivity was 0%. False negative 17 results were due to macroscopic sampling error (47,05%), faulty interpretation of the pathologist (35,30%) and microscopic sampling error (17,65%). Female genital tract, thyroid and gastrointestinal tract were the most frequent frozen section requested sites, respectively. Two hundred sixty six specimens that consisted of 30,6% of our series were from female genitale tract. The accuracy rate, spesivity and sensitivity were, 98,5%, 100% and 94%, respectively. Ninty one specimens that consisted of 10,4% of our series were from thyroid. The accuracy rate, spesivity and sensitivity were, 93,4%, 100% and 68%, respectively. Eighty eight specimens that consisted of 10,1% of our series were from gastrointestinal tract. The accuracy rate, spesivity and sensitivity were, 98,8%, 100% and 95%, respectively. Our results demonstrated that frozen section is a reliable technique in spite of its inevitable mistakes. Frozen section is an intraoperative consultation that should be performed if it is results will change the surgical procedure. Good communication between pathologist and surgeon, knowledge of endication and limitations of the procedure will increase the accuracy rate of frozen section.
