Fulminan karaciğer yetmezliği, akut hepatitin ciddi bir koplikasyonudur. Masif karaciğer hücre nekrozu ve ansefalopati ile karakterizedir ve yüksek mortalite oranı ile birliktedir. Etkinliği kanıtlanmış tek tedavi yöntemi acil karaciğer transplantasyonudur. Bugüne kadar pek çok ilaç denenmiş, fakat bütün bu medikal tedavilerin etkinliği sınırlıdır ve yeni tedavi yöntemlerine ihtiyaç vardır. Bu çalışmamızda tam doz TAA'ya bağlı AKY'de kefirin etkinliğini değerlendirmeyi amaçladık. Deneye 48 sıçan alınarak, üç adet altılı ve üç adet onlu toplam 6 grup oluşturuldu. Ayrıca, 30 sıçanda sürvi tayini yapıldı. Hepatotoksik ajan olarak i.p. olarak 350mg/kg/sıçan dozunda TAA kullanıldı. Sakrifye edilen sıçanlarda karaciğer fonksiyon testleri, doku düzeyinde GSH, katalaz, GPX, SOD, MPO, MDA, TNF-a, IL-10 gibi biyokimyasal parametreleri değerlendirdik. Ayrıca histopatolojik değerlendirmede hepatoselüler nekroz derecesine bakıldı. Çalışma sonucunda, karciğer histopatolojisi değerlendirildiğinde TAA verilen gruplarda akut hepatit geliştiği gözlendi. Ancak TAA alan gruplar arasında histopatolojik olarak anlamlı bir farklılık saptanmadı. Bununla birlikte kefir verilen gruplarda transaminaz düzeyleri, PT ve INR değerleri daha yüksek saptandı. Ayrıca antioksidan özelliği olan glutatyon düzeyleri kefir verilen gruplarda düşük saptandı. Sonuç olarak AKY'de kefirin etkinliğinin aksine zararlı olabileceğini saptadık. Fulminant hepatic failure (FHF) is a serious complication of acute hepatitis. It is characterized by massive cell necrosis and encephalopathy, and is associated with high mortality. Liver transplantation is the single treatment option. Many drugs have been tried for the treatment, however none of them have been found successful. Therefore, new treatment modalities are needed. In this study, we aimed to investigate protective effect of kefir on the full dose Thioacetamide (TAA) induced the FHF in rats. Fourty eight rats were divided into six groups, 3 containing 6 rats and other 3 groups were containing 10 rats in each group. Addionally, survey evalution was done on 30 rats. As a hepatotoxic agent, TAA in 350 mg/kg/rat dose i.p. was used. We analyzed liver function tests and biochemical parameters in liver tissue such as GSH, catalase, GPX, SOD, MPO, MDA, TNF-a and IL-10. In addition, hepatocellular necrosis was investigated with histopathologic examination. As a result of histopathologic examination, acute hepatitis was developed in all groups which were treated with TAA. However, there was no significant difference between the groups. In addition, in the groups which Kefir were used, transaminases, PT and INR values were higher comparing to those in TAA was used. Glutation, which has antioxidant effect, was lower in kefir groups comparing to those in other groups. In conclusion, we have found that kefir might be harmful in contrast to our previous hypothesis that kefir can have favorable effects on gastrointestinal system.
