El OA'inde hastaların el eklemlerinin tutulumuna bağlı olarak kavrama gücü ve ince el becerilerinde azalma oluşabilir. El OA'nin postmenopozal kadınlardaki el kuvveti, el fonksiyonları, ince el becerileri ve günlük yaşam aktiviteleri üzerindeki etkisi araştırmak amacıyla çalışmaya el OA tanısı konulmuş 77 hasta ve 81 sağlıklı kontrol alındı. Hastaların demografik özellikleri kaydedildi. Ağrı, Likert ağrı skalasına göre değerlendirildi. Nodül ve hassas eklem sayısı kaydedildi. Olguların AP el grafileri çekildi. Radyolojik değerlendirme Kellegren ve Lawrence skalasına göre yapıldı. Her iki elin kavrama ve tutma güçleri Jamar dinamometresi ve pinchmetre ile ölçüldü. İnce el becerisi Grooved Pegboard Testi ile değerlendirildi. El fonksiyonlarındaki yetersizlik DEİ ve JTEBT ile değerlendirildi. Yaşam kalitesi HAQ skoru ile değerlendirildi. Grup 1 ve grup 2 arasında demografik özellikler açısından anlamlı fark bulunmadı (p>0.05). Grup 1'de nodül ve hassas eklem sayısı daha fazlaydı (p<0.001). HAQ ve DEİ skoru, ağrı şiddeti ve radyolojik evre grup 1'de daha yüksekti (p<0.05). Hasta grubunun el kavrama ve tutma gücü azalmış olarak bulundu (p<0.05). Hastalığın radyolojik evresinin el kavrama gücünü etkilediği belirlendi. Grooved Pegboard Testi ve JTEBT kontrollere göre uzamış bulundu (p<0.001). DEİ'ni lateral tutma gücü, JTEBT'ni kavrama ve tutma güçleri, HAQ skorunu, hassas eklem sayısı ve el kavrama gücü, Grooved pegboard testini yaşın etkilediği bulundu (p<0.05). Çalışmamızda el OA'li postmenopozal hastaların kavrama ve tutma güçlerinin azaldığı, el fonksiyonları ve yaşam kalitelerinin bozulduğu saptanmıştır. Bu nedenle, hastalar yakından takip edilmeli, eklemleri koruyucu ve günlük yaşam aktivitelerini kolaylaştırıcı yardımcı cihazlar konusunda bilgilendirilmelidir. Reduced grip strength and dexterity of hand may occur due to involvement of hand joints in patients with hand OA. Seventy-seven patients with hand OA and 81 healthy controls were participated in this study for searching the effects of hand osteoarthritis on hand strength, hand functions, dexterity and daily life activities of post-menopausal women. Demographic data of patients was recorded. Pain was assessed with Likert pain scale. Node and tender joint counts was recorded. AP radiograph of hands were performed for all cases. Radiologic assessment was performed according to Kellgren and Lawrence classification. Grip and pinch strength of both hands was tested with Jamar dynamometer and manual pinchmeter. Manual dexterity was tested with Grooved Pegboard Test. Hand functional disability was tested with DHI and JTHFI. Quality of life was measured by using HAQ. There were no significant differences between demographic features of group 1 and 2 (p>0.05). Node and tender joint counts was higher in group 1 than group 2 (p<0.001). HAQ and DHI scores, severity of pain and radiographic grade were higher in group 1 than group 2 (p<0.05). The grip and pinch strengths were lower in the patient group (p<0.05). The hand grip strength was found to be affected by the radiographic grade of the disease. Grooved Pegboard Test and JTHFT were prolonged compared to controls (p<0.001). DHI was found to be affected by lateral pinch strength, JTHFT by grip and pinch strength, HAQ score by tender joint count and hand grip strength, Grooved pegboard test by age (p<0.05). In the present study, grip and pinch strength was reduced, hand function and quality of life was negatively affected in post-menopausal patients with hand OA. For this reason, patients must be followed closely and they must be informed about assistive devices which protect the joints and facilitate daily life activities.
