HBV dünya genelinde mortalite ve morbidite nedeni olan önemli bir halk sağlığı sorunudur. HBsAg ve anti-HBs negatifliğinde anti-HBc pozitifliği izole anti-HBc olarak adlandırılmakta olup tüm serolojik belirteçlerin negatif sadece anti-HBc'nin pozitif olması ise salt anti-HBc olarak adlandırılmaktadır. Moleküler tekniklerin kullanıma girmesiyle Hepatit B infeksiyonu için kullanılan serolojik testlerin tanıda ve takipde yetersiz olduğu görülmüştür. Bu çalışmada öncelikle salt anti-HBc pozitif donörlerde ve atipik serolojik profildeki serumlarda HBV DNA araştırılması amaçlandı. Yaptığımız çalışmada hastanemiz kan merkezine başvuran 2500 HBsAg negatif donörden 401'inde anti-HBc pozitif olarak tespit edilmiş olup bunlardan da izole anti-HBc pozitif 45 donör (36'sı salt anti-HBc pozitif), anti-HBs/anti-HBe pozitif 116 donör ve anti-HBs/HBeAg pozitif olan 8 donör olmak üzere toplam 169 donör HBV DNA araştırılmak üzere çalışmaya alınmıştır. Çalışmaya alınan anti-HBs pozitif 124 donörün 32 (% 25,8)'sinde, izole anti-HBc pozitif 45 donörün 3 (% 6,6)'ünde, bunlarında 36'sı salt anti-HBc pozitif olup 2 (% 5,5)'sinde, 161 HBeAg negatif donörün 32 (% 19,8)'sinde, 116 HBeAg negatif/anti-HBe pozitif donörün 29 (% 25)'unda, toplam 125 anti-HBe pozitif donörün de 30 (% 24)'unda HBV DNA pozitif olarak bulundu. Sonuç olarak hepatit B infeksiyonun tanısında, viral yükün ve prognozun belirlenmesinde mutant suşlarla infeksiyon ve tüm serolojik markerları negatif olanlarda da HBV DNA'nın bulunması göz önünde bulundurulduğunda serolojik testlerin yetersiz olduğu görülmüştür. Dolayısıyla kan donörlerinde sadece HBsAg negatifliğine bakılarak kan transfüzyonun yapılmasının yetersiz kaldığı bununla beraber anti-HBc'nin de bakılmasının da maliyeti arttırması ve yalancı pozitifliğe bağlı olarak kan donör kaybına neden olabileceğinden özgüllük ve duyarlılık açısından daha gelişmiş anti-HBc total testlerine ya da maliyeti daha düşük ve uygulanabilirliği daha fazla HBV DNA testlerine ihtiyaç duyulduğu görülmektedir. HBV (Hepatitis B Virus) is one of the most important public health issues which causes mortality and morbidity all over the world. The absence of HBsAg and anti-HBs, together with the presence of anti-HBc is defined as isolated anti-HBc. On the other hand, it is called absolute anti-HBc, if anti-HBc is positive in the absence of all the serologic markers. When the molecular techniques were intoduced, it was realized that serologic analysis were incapable of diagnosing and estimating the prognosis of Hepatitis B infection. In the present study, the first aim was to investigate HBV DNA in the donors with absolute anti-HBc and in the sera with atypical serology. In this study, 2500 donors who applied to our blood transfusion centre were HbsAg negative and anti-HBc was detected in 401 of these 2500 donors. 45 donors were isolated anti-HBc positive (36 out of these 45 were absolute anti-HBc positive), 116 donors were anti-HBs/anti-HBe positive and 8 donors were anti-HBs/HBeAg positive. Totally, 169 out of these 401 donors were taken into the study to investigate HBV DNA. HBV DNA was positive in 32 out of 124 anti-HBs positive donors (25,8%), 3 out of 45 isolated anti-HBc positive donors (6,6%), (2 out of 36 absolute anti-HBc positive donors (5,5%)), 32 out of 161 HBeAg negative donors (19,8%), 29 out of 116 HBeAg negative/anti-HBe positive donors (25%) and 30 out of 125 (24%) anti-HBe positive donors. As a result, HBV DNA is positive also in those who have been infected by mutant strains and whose serologic markers are all negative, indicating that serologic tests are inefficient for diagnosing Hepatitis B infection, quantitating viremia and estimating the prognosis. Consequently, blood transfusion is potentially unsafe in the absence of HbsAg alone in the donors. Moreover, examining anti-HBc together with HBsAg increases the cost and may become a reason for losing a donor due to the false positive result. In terms of specificity and sensitivity, more sophisticated anti-HBc IgG tests or more applicable and cost effective HBV DNA tests are required. Keywords : Isolated anti-HBc, Hepatitis B Virus, PCR.
