Gestasyonel diabetes mellitus gebelik sırasında ki önemli ve yaygın komplikasyonlardan biridir. İnsülin direncinin GDM.nin patofizyolojisinde önemli rol oynadığı izlenmektedir. Çalışmamızda 1.trimesterde insülin direnç indekslerinin ve adiponektin düzeylerinin GDM ile ilişkisini açıklamayı ve bu değerlerin GDM tanı ve taramasındaki yeterliliklerini saptamayı amaçladık. Çalışmamızda 1 yıl boyunca polkliniğimize gelen, Tip 1 ve Tip 2 DM öyküsü olmayan gebeler tarandı. Rutin takiplerinde 24.-28. haftalar arasında 50 g yükleme testi sonrasında eşik değer 135mg/dl ve üzerindekilere 100 g OGTT uygulandı. GDM insidansı hesaplandı. Daha sonra kitlerin yetersizliğinden dolayı 25 GDM.li ve 25 sağlıklı gebeden oluşan çalışma grubu seçildi. Sağlıklı ve hasta grubun açlık plasma şekerleri, HOMA-IR indeksleri ve adiponektin düzeyleri hesaplandı. Uygulanan testler sonucu GDM tanısı koyulan gebelerle kontrol grubunun verileri karşılaştırıldı. Carpenter ve Coustan kriterlerine göre 300 hastadan 31.ine (%10) GDM tanısı konuldu. Çalışma grubundaki GDM.li hastalarda; normal gebelere göre 1.trimester adiponektin düzeyleri anlamlı olarak düşük(p<0,001) , HOMA-IR indeksleri ve açlık plasma şekerleri anlamlı olarak yüksekti (p<0,001-p<0,001). Adiponektin için eşikdeğer 7269ng/ml alındığı zaman duyarlılık %84, özgüllük %92; HOMA-IR skoru için eşik değer 2,40 alındığı zaman duyarlılık %92, özgüllük %84; APS için eşik değer 89mg/dl alındığı zaman duyarlılık %64, özgüllük %76; 50 g yükleme testi eşik değeri için 140mg/dl alındığı zaman duyarlılık %88, özgüllük %64 olarak saptandı.Serum adiponektin düzeylerindeki azalmanın GDM.de insülin direnci gelişiminde rol aldığı söylenebilir. İnsülin sensitivite indeksleri ve adiponektin GDM tarama ve tanısında faydalı olabilirler. Ama bu belirteçlerin güvenilirliğinin artması için daha geniş prospektif çalışmalara ihtiyaç vardır. Gestational Diabetes Mellitus (GDM) is a common and important complication during pregnancy. Insulin resistance seems to play important roles in pathophysiology of GDM. In our study we aimed to evaluate the relation between first trimester values of serum adiponectin, HOMA-IR indicies and GDM. Especially to investigate whether gestational diabetes mellitus can be screened and diagnosed by the use of these values. Three-hundered pregnant women who didnt have history of DM were scanned during the one year. Between 24.-28.th weeks of gestations, all the women had 50g GCT. Those patients who did have abnormal response accordingly (postload glucose concentrations of over 135mg/dl) underwent a standart 100 g OGTT. The prevalence of GDM was calculated. Then, 25 women with GDM and 25 healthy pregnant women were selected fort he study. Fasting plasma glucose, fasting adiponectin and HOMA indicies were performed. The results of GDM and control groups were compared. According to CC criteria 31 of the 300 women were diagnosed as GDM. First trimester serum adiponectin concentrations were significantly lower (p<0,001), and HOMA-IR values and fating plasma glucose were significantly higher (p<0,001) in GDM women, compared with the group of healthy women. A receiver-operating characteristic curve analysis was performed to determine an optimal cut-off value for HOMA-IR, FPG and adiponectin in GDM. A cut-off value of 7269ng/ml for adiponectin had %84 sensitivity, %92 specificity. Acut-off value of 2.40 for HOMA IR had %92 sensitivity, %84 specificity. A cut-off value of 89mg/dl for fasting plasma glucose had %64 sensitivity, %76 specifity. A cut-off value of 140mg/dl for GCT had %88 sensitivity, %64 specifity. Decrease in serum adiponectin levels are thought to play a role in GDM. patients insulin resistance. The insulin sensitivity indicies and adiponectin may be helpful in the screening and diagnosis of GDM. But we need more extensive prospective study to say this hypothesis.
