Pilosebase ünitenin kronik inflamatuar bir hastalıgı olan akne vulgaris, seboreik bölgelerde komedon, papül, püstül, nodül, kist ve skarlarla karakterizedir. Çogunlukla adölesan dönemde görülen multifaktöriyel bir hastalıktır. Bazı kozmetik ürünler akneyi tetikleyebilecegi gibi akne tedavisinde kullanılan topikal ve kozmetik ajanlar allerjik kontakt dermatite sebep olabilmektedir. Bu durum hastaların tedaviye uyumunu bozabildigi gibi yasam kalitelerini de olumsuz yönde etkilemektedir. Bu nedenle hastaların kozmetik ürün seçimi büyük önem tasımaktadır. Literatürde, akne vulgaris hastalarında sadece topikal ilaçlar ile allerjik kontakt dermatitin arastırıldıgı sınırlı sayıda çalısma mevcuttur. Bu çalısmada, akne vulgarisli hastalarda kozmetiklerle olusan allerjik kontakt dermatit sıklıgının arastırılması amaçlanmıstır. Akne vulgaris tanısı alan ve kontakt dermatit klinik bulguları olmayan 50 hasta ile basvuru sırasında kontakt dermatit klinik bulguları olmayan 50 kontrol olgusu çalısmaya alındı. Akne siddetini degerlendirmek için Global Akne Derecelendirme Sistemi kullanıldı. Avrupa standart seri (27 allerjen) ve kozmetik seri (45 allerjen) deri yama testleri, IQChamber test materyali kullanılarak hasta ve kontrol grubundaki olguların lezyonsuz sırt derisine uygulandı. Test maddeleri içeren üniteler 48 saat sonra çıkartılıp 30 dakika sonra ve 72. saatte Uluslararası Kontakt Dermatit Arastırma Grubu tarafından önerilen sekilde degerlendirilerek reaksiyon veren maddeler kaydedildi. Hasta grubunda en az bir allerjene karsı gelisen pozitiflik oranı %40 ve kontrol grubunda %28 olarak tespit edilmekle birlikte bu fark istatistiksel olarak anlamlı degildi. Kozmetik seride yer alan allerjenlerden sorbitan monooleat duyarlılıgı hasta grubunda anlamlı oranda yüksek bulundu (p=0,022). Sorbitan monooleat dısında diger allerjenlere karsı gelisen duyarlılık açısından hasta ve kontrol grubu arasında istatistiksel olarak fark saptanmamakla birlikte hasta grubunda daha fazla sayıda allerjen ile pozitif reaksiyon tespit edildi. Sonuç olarak hasta grubunda yama testi ile duyarlılık saptanan allerjenlerin fazla olması kozmetik ürünlerin akne vulgarisli hastalarda allerjik kontakt dermatit gelisiminden sorumlu olabilecegini düsündürmektedir. Bununla birlikte sonuçlarımızın desteklenmesi için daha fazla sayıda hasta grupları ile yapılacak çalısmalara gereksinim oldugu görüsündeyiz. Acne vulgaris is a chronic inflammatory disease of pilosebaceous unit, characterized by comedos, papules, pustules, nodules, cystes and scars in seborrheic regions. It is a multifactorial disease mostly developing at adolescence. While some cosmetic products can trigger acne, topical and cosmetic agents used in the treatment of acne may cause allergic contact dermatitis. This situation breaks patients. adaptation to treatment besides its negative effect on the quality of life. Therefore, the choice of cosmetic products is very important for the patients. Limited number of studies have been found in literature investigating allergic contact dermatitis due to only topical drugs in patients with acne vulgaris. In this research, it is aimed to find out the frequency of allergic contact dermatitis caused by cosmetics in patients with acne vulgaris. Fifty patients with acne vulgaris but without clinical signs of contact dermatitis and 50 control cases without contact dermatitis on admittance were included in the study. Global Acne Grading System was used in order to detect the severity of acne. Skin patch tests of the European standard series (27 allergens) and cosmetic series (45 allergens) with IQ Chamber test materials were applied on the unlesional back of the patient and control groups. Materials with testing substances were taken out of skin 48 hours later and read after 30 minutes and at 72nd hour, as suggested by International Contact Dermatitis Research Group, and substances leading to reactions were recorded. While positivity for at least one allergen is 40 % in patient group and 28 % in the control group, this difference was not found statistically significant. Sensitivity to sorbitan monooleate, in the cosmetic series, was found to be significantly higher in the patient group (p=0,022). While no statistical difference except for sorbitan monooleate was detected in the patient and control groups, it was found that the patient group reacted positively to more allergens than the controls. In conclusion, because of abundance of allergens detected with patch test in the patient group, it may be suggested that cosmetic products might be responsible for the development of allergic contact dermatitis in patients with acne vulgaris.
