Diyabetik ayak lezyonları yüksek mortalite ve morbiditesi olan, alt ekstremiteyi ve hayatı tehdit eden bir durumdur. Non-travmatik amputasyonların %50-70.i diyabetik hastalarda gerçekleşmektedir. Alt ekstremite amputasyon riski diyabetiklerde, diyabetik olmayanlardan 15 kat daha fazladır. Bu çalışmanın amacı, üç fazlı kemik sintigrafisinin diyabetik ayak hastalarının tedavi giderlerini, mortalite ve morbiditeyi azaltmak, hastanede kalış süresini kısaltmak, bir üst seviye revizyonu, doku nekrozu, enfeksiyon gibi nedenlerle tekrarlayan girişimleri önlemedeki etkinliğini incelemektir. Hastaların hastaneye başvuru anında diyabetik yak enfeksiyonu Wagner evrelemesine göre 12' si evre 4 (% 40) ve 16' sı evre 5(% 60) olarak değerlendirildi. İlk amputasyon seviyesi alt ekstremitelerin 6'sına parmak (%20), 5'ine ray(%16,6), 1'ine transmetarsal(%3,3), 2'sine Lisfranc(%6,6), 7'sine Chopart(%23,3), 1'ine Syme(%3,3), 6'sına diz altı (%20), 1'ine diz dezartikülasyonu(%3,3) ve 1'ine ise diz üstü(%3,3) seviyelerinden idi. Yedi ekstremite bir üst seviyeden tekrar ampute edildi. Amputasyon kararı vermek hem hasta hem de cerrah açısından zordur. Uygun olmayan seviye seçimi hastanın tekrar cerrahi müdahale geçirmesine neden olabileceğinden ameliyat öncesi değerlendirmenin önemi büyüktür. Biz bu çalışmada, ameliyattan önce yapılan üç fazlı kemik sintigrafisi tetkikleri sonucuna göre amputasyon seviyesine karar verdik. Bu hastalarda hastanede kalış süresinin kısaldığını, tedavi giderlerinin azaldığını gördük. Özellikle hastanede kalış süreleri literatürdeki benzer vaka serilerine göre daha kısa idi. İncelediğimiz yayınlardaki reamputasyon oranlarının değişiklik göstermesi nedeniyle, amputasyon öncesi üç fazlı kemik sintigrafisinin reamputasyon oranlarını azaltmadaki etkinliğini değerlendirmek üzere diğer preoperatif tanısal yöntemlerin de kullanıldığı, karşılaştırmalı, çok sayıda vaka içeren, uzun süreli ve kontrollü çalışmalara ihtiyaç olduğunu düşünüyoruz. Diabetic foot lesion is a situation which has a high mortality and morbidity, threatens the lower extremity and the life. The 50-70% of the non-traumatic amputations are performed on diabetic patients. The amputation risk of lower extremity is 15 times more on the diabetics than the non-diabetics. The aim of this study is to determine the efficacy of three phase bone cintigraphy on reducing the treatment cost, mortality and morbidity, duration of ospital stay, he number of revision, structure necrosis and infection. he evaluation of diabetic foot nfection n presentation according to Wagner lassification showed that 12 patients (40%) were stage 4 nd 16 (60%) were stage 5. nitial amputation level of lower limbs are toe 6 (20 %), digital ray (16,6 %), transmetarsal 1 (3,3%), Lisfranc joint 2 (6,6%), Chopart joint 7 (23,3%), Syme joint 1 (3,3%), below knee 6 (20%), knee disarticulation 1 (3,3%) and above the knee 1 (3,3%). Upper level reamputation was perfomed in 7 limbs. It is difficult to give an amputation decision for both the patient and the surgeon. Inappropriate level selection may cause the patient to have repeated surgical interventions. For this reason preoperative assessment is important. We examined the results of three phase bone scintigraphy before the surgery, and decied the amputation level. We have observed on these patients that the duration of hospital stay became shorter and the medical costs were reduced. Particularly the duration of hospital stay were shorter according to the parallel case series in the literature. Because the reamputation rates differ in the literature, we think that comperative, long term and controlled studies conducting other preoperative diagnostic modalities are needed to show the efficacy of three phase bone scintigraphy on reducing the reamputation rates.
