Mide ve duodenum perforasyonlarında batın içerisine mide sıvısı, gıda artıkları ve safra gibi irritan maddeler dökülmektedir. Buda peritonite ve postoperatif peritoneal adezyonlara neden olmaktadır. Cerrahi işlem sırasında öncelikle kaynak kontrolü yapıldıktan sonra batın içerisinin peritonite neden olan yabancı maddelerden arındırılması gerekmektedir. Bunun için peritoneal kavitenin serum fizyolojik, laktat ringer gibi sıvılarla lavajı yada nemli kompresler aracılığı ile kuru temizlik yöntemleri sıklıkla kulanılmaktadır. Bu çalışmada kırk adet Wistar Albino erkek sıçan (ortalama ağırlık 250 ł20 gram; ortalama yaş 7-8 ay) dört eşit gruba ayrıldı. 1. grup kontrol grubu olarak belirlendi ve laparotomi yapılarak mide perforasyonu oluşturulmadan batın kapatıldı. 2. grupta mide perforasyonu oluşturularak bir saat beklendikten sonra mide primer onarımı yapılarak batın içi temizlenmeden kapatıldı. 3. grupta mide perforasyonu oluşturularak bir saat beklendikten sonra mide primer onarımı yapıldı ve batın içi serum fizyolojikle yıkanarak kapatıldı. 4. grupta mide perforasyonu oluşturularak bir saat beklendikten sonra mide primer onarımı yapıldı ve batın içi nemli kompreslerle temizlenerek kapatıldı. Postoperatif yedinci günde tüm ratlar sakrifiye edilerek oluşan adezyonlar evrelendirildi. Tüm ratlardan peritoneal örnekler alınarak rat spesifik t-PA-1 ve PAI-1 çalışıldı. Perforasyon grubunda sekiz denekte grade 3(%80), iki denekte grade 2 (%20); serum fizyolojik grubunda yedi denekte grade 3 (%70), üç denekte grade 2 (%30); kompres grubunda dört denekte grade 3 (%40), beş denekte grade 2 (%50), bir denekte grade 1 (%10) adezyon tespit edildi. Postoperatif yedinci günde peritoneal örneklerden çalışılan t PA düzeylerinin kompres grubunda arttığı görüldü. PAI-1 düzeyleri ise serum fizyolojik grubunda en yüksekti. Çalışmamız göstermiştir ki mide perforasyonlarından sonra batın içinin nemli kompresle temizlenmesi, serum fizyolojikle yapılan irrigasyona göre daha az adezyona neden olmuştur. Klinik uygulamada, sekonder peritonit olgularında kaynak kontrolünden sonra batın içinin nemli kompreslerle temizlenmesinin adezyonları önlemede daha faydalı olacağını düşünmekteyiz. Intraperitoneal cleaning methods effects on postoperative peritoneal adhesions in stomach perforations In gastric and duodenum perf orations, irritating substances like gastric juice, food remnants, and bile are infused into the abdomen. This causes peritonitis and postoperative peritoneal adhesions. During surgical operations, after source control is done the inner part of the abdomen should be cleaned from foreign substances that cause peritonitis. To do this, lavage of the peritoneal cavity using liquids such as physiological saline, lactated Ringer solution or dry cleaning methods involving moist compresses are frequently used. In this study, forty Wistar Albino male rats (average weight 250ł 20 gr; average age 7-8 months) were divided into four equal groups. First group was assigned as the control group and abdomen was closed after performing laparotomy without performing gastric perforation. In the second group, gastric perforation was performed in the stomach and after waiting for an hour, the primary repair of the stomach was performed and the abdomen was closed without cleaning the inner part of the abdomen. In the third group, gastric perforation was performed in the stomach and after waiting for an hour, the primary repair of the stomach was performed and the abdomen was closed after washing the inner part of the abdomen with physiological saline. In the fourth group, gastric perforation was performed in the stomach and after waiting for an hour, the primary repair of the stomach was performed and the abdomen was closed after cleaning the inner part of the abdomen with moist compresses. On the seventh postoperative day, all rats were sacrificed and the resulting adhesions were phased. Peritoneal samples were taken from all the rats; rat-specific t-PA-1 and PAI-1 were studied. In the perforation group, grade 3 (80%) adhesion was detected in 8 subjects and grade 2 (20%) adhesion was detected in 2 subjects; in the physiological saline group, grade 3 (70%) adhesion was detected in 7 subjects and grade 2 (30%) adhesion was detected in 3 subjects; and in the compress group, grade 3 (40%) adhesion was detected in 4 subjects, grade 2 (50%) adhesion was detected in 5 subjects and grade 1 (10%) adhesion was detected in 1 subject. On the seventh postoperative day, it was observed that the t PA levels of the studied peritoneal samples increased in the compression group. PAI-1 levels were highest in the physiological saline group.This research shows that after stomach perforations, cleaning the iner part of the abdomen with moist compress causes less adhesion as compared with that caused by the irrigation with physiological saline. We think that in clinical practice, in secondary peritonitis cases, cleaning the inner part of the abdomen with moist compresses after the source control is more useful in preventing adhesions.
