Radyolojik kontrast ajanlar, doğru teşhis için sıkça kullanılan ve vazgeçilmez olan maddelerdir. Bu maddelerin böbrekte bazı yan etkileri tanımlanmıştır. Radyolojik kontrast ajanların neden oldukları renal hasarlanmaların etyolojisinde serbest radikallerin rolü ile ilgili pek çok çalışma yapılmaktadır. Manyetik Rezonans Görüntüleme (MRG) kontrast ajanları diğer radyokontrast ajanlara göre nispeten çok daha az yan etkiye sahiptirler. Ancak bazı çalışmalarda, MRG kontrast ajanlarının çok yüksek kümülatif dozlarda uygulanmasından sonra, değişik derecelerde böbrek hasarlanmaları bildirilmiştir. Fakat bu hasarlanmanın etyopatogenezi henüz tam olarak aydınlatılamamıştır. Biz çalışmamızda, MRG kontrast ajanı olan Gadobutrolün, intraperitomeal (IP) ve intravenöz (IV) kümülatif doz uygulamalarından sonra böbrek dokusundaki yan etkilerini, böbrekte ve kanda oksidatif strese neden olup olmadığını araştırdık. Oksidatif stres göstergesi olarak nitrik oksit (NO), malondialdehit (MDA) ve glutatyon (GSH) değerlerini araştırdık. Çalışmamızda ratlar 3 kontrol ve 3 ilaç grubu olmak üzere 6 gruba ayrıldı. 5 mmol/kg dozda Gadobutrol, gruplardan birine 6 dozda IP, birine 6 dozda IV, diğerine ise 20 dozda IP olarak verildi. 3 kontrol grubuna ise aynı yöntemlerle aynı dozlarda salin uygulandı. Tüm Gadobutrol verilen gruplarda, histopatolojik olarak böbrekte tubuler vakuolizasyon, intratubuler eritrosit birikimi ve tubuler cast oluşumu gibi bazı hasarlanmalar gözlendi. Gadobutrol alan gruplardaki ratların böbrek dokusunda, oksidatif stres göstergelerinde, kontrol grubuna oranla istatistiksel olarak anlamlı düzeye ulaşmayan artış saptandı. Gadobutrol verilen grupların deney sonu plazma MDA değerleri, kontrol grupları ile kıyaslandığında anlamlı düzeyde yüksek saptandı. 6 kez IV Gadobutrol verilen grupta plazmada, deney başlangıcı ile sonu arasında MDA ve NO değerlerinde anlamlı artış, GSH değerlerinde ise anlamlı Hazalma izlendi. 20 doz IP Gadobutrol verilen grupta plazmada, başlangıç değerlerine göre deney sonu MDA değerlerinde anlamlı artış saptandı. 6 doz IP uygulama yapılan grupta ise oksidatif stres göstergelerinde anlamlı düzeylere ulaşmayan artışlar saptandı. 20 doz IP Gadobutrol uygulanan gruptaki plazma MDA değeri, 6 doz IP uygulanan gruba göre anlamlı olarak daha yüksek, GSH değeri ise daha düşük saptandı. Bu sonuçlar, yüksek doz Gadobutrol uygulaması sonrası oksidatif stres göstergelerinde doza bağlı bir artışı desteklemektedir. Sonuç olarak Gadobutrolün çok yüksek kümülatif dozlardaki uygulaması plazmada oksidatif stresi artırmaktadır. Fakat böbrek dokusunda oksidatif stres göstergelerindeki artış istatistiksel olarak anlamlı düzeylere ulaşmamaktadır. Radiologic contrast medias are used frequently for the exact diagnosis and therefore they are indispensable. However, some nephrotoxic side effects of these medias have been reported. Lots of investigations have been done about the role of free radicals as an etiology of renal damage by using contrast media. MRI contrast medias have less side effects than the other radiocontrast medias. But different degrees of renal damage have been reported after administration of MRI contrast medias with high cummulative dosage in some investigations. But pathogenesis of this damage has not been explained yet. In our study, we investigated that renal side effects of Gadobutrol as a MRI contrast agent which was administered via IP and IV routes in various cummulative dosage as well as the presence of oxidative stress in renal tissue and blood. As an indicator of oxidative stress, we measured the levels of NO, MDA and GSH. In our study we divided rats into 6 groups, 3 of 6 as a drug group and the other 3 as a control group. Gadobutrol in 5 mmol/kg dosage was given to one group 6 times via IP, was given the other group 6 times via IV and was given to third group 20 times via IP. We gave salin to other remainig 3 groups with the same method and with same dosages. In Gadobutrol groups, we demonstrated some damages like vacuolation of kidney tubules, intratubular erytrocyte accumulation and tubular cast formation. The oxidative stress was found to be increased in the renal parenchyma of Gadobutrol groups when conpared to the control groups. However, the idfference did not reach statitically significant. At the end of our study, values MDA in the Gadobutrol given groups were high meaningfully when we compared with control group. In the plasma of group which was given 6 times Gadobutrol via IV, end levels of MDA and NO increased significantly when we compared with initial levels. On the other hand, levels of GDA decreased significantly. In the group of 20 doses Gadobutrol given via IP, when we compared with initial levels, there was increased values at the end of experiment. In the group of 6 doses given via IP, we showed that there was increase in the indicators of oxidative stress, but they were not meaningfully. The levels of MDA in the group of 20 doses Gadobutrol given via IP was higher significantly than in the group of 6 doses via IP. But level of GSH was lower. These results support dose dependent increasement of oxidative stress finings after high dose Gadobutrol administration. As a result, administration of Gadobutrol with a high cummulative dosages increases oxidative stress. But, oxidative stress findings of renal parenchyme can not reach statistically significant levels. This condition shows us that some other factors can play a role in the damage of kidney.
