Ülkemizin kırsal kesimlerinde görülen zehirlenme olgularında en sık rastlanılan etken organofosfatlardır. Bu zehirlenmelerde mortalitenin en önemli sebebi solunum kasları tutulumu ile gelişen solunum yetmezliğidir. Ciddi zehirlenme vakalarının yoğun bakımda takibi gerekmektedir. Bu hastalarda gelişebilecek pankreas toksisitesi önemlidir. Kafeik asit fenetil ester (CAPE) bal arısının ürettiği propoliste bulunan flavonoid benzeri yapıda ve antiinflamatuvar, antioksidan, immün düzenleyici ve apopitozis düzenleyici etkileri çalışmalarla gösterilen bir bileşiktir. Bu bizi, doğal olarak yiyeceklerle de alınabilen CAPE.nin, sık kullanılan organofosfatlardan diazinonun yol açtığı pankreas hasarını iyileştirmede olumlu etkisinin olup olmadığını araştırmaya yönlendirdi. 35 adet erkek Wistar Albino rat 4 gruba ayrıldı. Kontrol grubu, 200 .g/kg (subletal) diazinon verilen grup, 1 gün önce 10 .mol/kg CAPE uygulaması sonrasında diazinon verilecek grup ve diazinon uygulamasından yarım saat sonra aynı dozda CAPE uygulanan grup olmak üzere. Hayvanlara herhangi bir ek ilaç veya besin kısıtlaması uygulanmadı. Kesim işlemi anestezi altında 48. saatte gerçekleştirildi. Biyokimyasal olarak serumda ALP, kolinesteraz, GGT, amilaz ve lipaz bakmak için kan alındı. Ayrıca histopatolojik inceleme için pankreas dokuları çıkarılarak özel fiksatör solüsyonda bekletildi. Çalışmamızda yalnız diazinon verilen Grup II ile CAPE uygulanan gruplar kolinesteraz düzeyleri açısından karşılaştırıldığında, Grup III ve IV.te istatistiksel olarak anlamlı (p=0.002) artma gözlendi. CAPE.nin bir gün önce verildiği grupta amilaz aktivitesi de artmıştır. Bu ilginç bir bulgudur. Kolinesteraz enzimi dışında diğer enzimlerin de aktivitesini artırması bizce olumlu bir bulgu olarak değerlendirilebilir. Çünkü organofosfat zehirlenmelerinde temel mekanizma AChE enzim aktivitesinin çok fazla azalmasıdır. CAPE.nin bu azalmayı % 50.den fazla oranda önlemiş olması önemli bir sonuçtur. Bu uygulama organofosfat zehirlenmelerinde hastaların hayatta kalması için iyi bir profilaktik ilaç olabileceği fikrini bize vermektedir. Anahtar Kelimeler : CAPE, diazinon, organofosfat, pankreatit.
They are the most frequently encountered organophosphates in the intoxication events which are seen in rural areas of our country. The most important reason of the mortality in these intoxications is the restrictive pulmonary syndrome which develops by muscle involvement. The serious intoxication events should be followed up in intensive care. The pancreatic toxicity is important which may be developed on these patients. The caffeic acid phenethyl ester is a compound which the honeybees produce, which is on the bee gums and is similar the flavonoidler structure, which is antiinflammatory, antioxidant and immune regulator. Herewith we aimed to investigate whether it has a positive effect the damage on pancreas which the CAPE that can be taken as a natural food, and diazinon from the organophosphates which are often used, cause. 35 male Wistar albino rat were divided into 4 groups. The control group which was given diazinon of 200 .g/kg (subletal) to, the group which will be given diazinon after the application of 10.mol/kg CAPE one day ago and the group which was given the same dose of CAPE after the application half an hour later. Animals were not applied any additional drug or feed restriction. The cutting process was performed under the anesthesia at 48th hour. Blood has been received to see ALP, cholinesterase, GGT, amylase and lipase in the serum as biochemical. In addition the pancreas tissue cells were waited in a special fixer solution by being extracted for histopathological analyzing. A meaningful (p=0.002) increase was observed on the Group III and IV as statistical at our work, when the Group II which was given only diazinon and the groups which were applied CAPE to in terms of cholinesterase levels. The amylase activity also increased on the group which was given CAPE one day ago. This is an interesting finding. The increasing of the other enzymes except the cholinesterase enzyme can be evaluated by us as a positive finding. Because the main mechanism is that the enzyme activity reductives in the organophosphate intoxications very much. It.s a valuable result preventing of CAPE this reductive more than over the rate of 50%.
