Hastane kaynaklı enfeksiyonların önemli bir nedeni olan Multi Drug Resistant (MDR) Acinetobacter baumannii, tüm insanlar için tehdit oluşturan bir fırsatçı patojendir. Özellikle immün sistemi zayıflayan ve invaziv işlemlere tabi tutulan hastaları etkileyen bu patojenin hızlı tespit edilmesi, enfeksiyonla mücadelede önemlidir. Salgın kaynağının araştırılmasında, PFGE yöntemi bugün için altın standart olarak kabul edilmektedir. DiversiLab sistemi ise yeni geliştirilen bir moleküler tekniktir. Çalışmamızda, Süleyman Demirel üniversitesi Tıp Fakültesi Hastanesi Anestezi Yoğun Bakım, Nöroloji Yoğun Bakım, Koroner Yoğun Bakım ve cerrahi servis hastalarında izole edilen, 48 adet MDR A.baumannii izolatının PFGE ve rep- PCR DiversiLab yöntemleri ile moleküler benzerliklerinin araştırılması amaçlanmıştır. Çalışma sonunda, 2009 yılına ait 19 izolat, PFGE yöntemi ile 6 farklı patern oluşturmuştur (P A 7, P-B 2, P-C 2, P-D 6, P-E ve P-F 1.er izolat). Aynı suşların DiversiLab sistemi ile yapılan moleküler çalışmasında 7 farklı patern oluşmuştur (P14 6, P4 4, P12 4, P15 2 izolat ve P2, P3 ve P11 1.er izolat). 2010 yılına ait 29 izolat, PFGE yöntemi ile 4 farklı patern oluşturmuştur (P-A 18, P-B 8, P-C 2 ve P-D 1 izolat). 2010 yılı suşlarının rep-PCR DiversiLab sistemi ile yapılan analizinde, 13 farklı patern oluşmuştur. Bu paternlerin 21 tanesi 5 kümeye ait bulunmuş ve kalan 8 izolatın her biri farklı patern oluşturmuştur (P1 8, P2 6, P3 2, P4 3, P5 2 izolat ve P6-P13 arası 1.er izolat). PFGE yöntemine göre daha kısa sürede sonuç veren DiversiLab sistemi, patern kümelenmelerine göre daha fazla sayıda patern oluşumuna neden olmuştur. MDR A.baumannii, which is an important cause of hospital acquired infections, is an opportunistic pathogen being a threat for all human beings. Early detection of this pathogen, which effects especially immunocompromised patients and patients who have invasive procedures, is important in struggling with infection. For today, PFGE is a gold standard in searching for the source of outbreaks. DiversiLab system is a newly developed molecular technique. In our study, we aimed to investigate molecular relatedness of 48 MDR A.baumannii, by PFGE and rep-PCR DiversiLab systems in patients from Anesthesia Intensive Care Unit (ICU), Neurology ICU, Coronary ICU and surgical wards. At the end of study, 19 isolates belonging to the year 2009, we identified 6 different patterns by PFGE method (P-A 7, P-B 2, P-C 2, P-D 6 isolates, P-E and P-F 1 isolate each). In an analysis of same isolates with DiversiLab system, we obtained 7 different molecular patterns (P14 6, P4 4, P12 4, P15 2 isolates and P2, P3 ve P11 1 isolate each). With 29 isolates belonging to the year 2010, we obtained 4 different paterns by PFGE method (P-A 18, P-B 8, P-C 2 isolates and P-D 1 isolate). In an analysis of isolates belonging to 2010, rep-PCR DiversiLab system differentiated 13 different molecular patterns. Of these patterns 21 isolates were belonged to 5 clusters and the rest 8 isolates had a different pattern each (P1 8, P2 6, P3 2, P4 3, P5 2 isolates and P6-P13 1 isolate for each). DiversiLab system which we obtained a result in a shorter time than PFGE method, has resulted in more pattern formation according to pattern clustering.
