Giriş ve Amaç: Endoskopik retrograd kolanjiopankreatografi safra yolları ve pankreasın benign ve malign hastalıklarının tanı ve tedavisinde önemli yer tutan ancak günümüzde daha çok tedavi amaçlı kullanılan bir yöntemdir. Başta pankreatit olmak üzere birçok komplikasyonla ve nadiren mortalite ile ilişkili olabilir. Oluşabilecek komplikasyonların ve seyrinin önceden öngörülebilmesi riski ve mortaliteyi azaltmak açısından önemlidir. CRP'nin ERKP sonrası komplikasyonların öngörülmesi ve ciddiyetinin belirlenmesinde de güvenilir ve doğru bilgiler verdiği çeşitli çalışmalarda gösterilmiştir. Çalışmamızda ERKP uygulanan hastalarımızda, CRP.nin başta pankreatit olmak üzere komplikasyonları öngörme ve takibindeki yerinin araştırılması amaçlanmıştır. Materyal ve Metot: Çalışma için ERKP yapılan 476 hasta retrospektif olarak incelendi. İşlem öncesi ve işlemden 12-24 saat sonra serum amilaz, lipaz ve CRP seviyeleri ölçülmüş olan 136 hasta (76 kadın, 60 erkek; ortalama yaş 62,6 ł 16,89 yıl) çalışmaya dahil edildi ve bu hastalar işlem sonrası komplikasyon gelişimi ve CRP seviyelerindeki değişiklik açısından değerlendirildi. Ayrıca komplikasyon gelişen ve işlemden 36-48 saat sonraki serum amilaz, lipaz ve CRP seviyelerinin de ölçüldüğü tespit edilen 22 hastada, CRP'nin komplikasyonların takibi ve ciddiyetini öngörmedeki yeri araştırıldı. Komplikasyonlar hafif, orta ve ciddi olarak sınıflandırıldı. Bulgular: Çalışmaya alınan 136 hastanın 23.ünde (% 17) pankreatit, 2.sinde (% 1,25) kolanjit ve 1.inde kanama (% 0,75) olmak üzere toplam 26 hastada (% 19) ERKP sonrası komplikasyon saptandı. Pankreatit 9 hastada hafif, 14 hastada orta derecedeydi. İşlemden 12-24 saat sonraki CRP, amilaz ve lipaz ölçümleri komplikasyon olan ve olmayan hastalarda anlamlı olarak farklı bulundu. 12-24. saatteki ortalama CRP seviyeleri (mg/l) komplikasyon olmayanlarda 23,5 ł 24,18, pankreatiti olanlarda 59,2 ł 44,87 idi (p<0,05). Hafif ve orta derecede pankreatiti olan hastalar arasında da işlemden 12-24 ve 36-48 saat sonraki CRP ve amilaz seviyeleri açısından anlamlı farklılık vardı. Lipaz için bu farklılık gözlenmedi. Hafif ve orta derecede pankreatiti olanlarda, işlemden 12-24 saat sonraki ortalama CRP seviyesi (mg/l), sırasıyla, 30,8 ł 14,24 ve 77,5 ł 48,66 (p<0,05), 36-48 saat sonraki değerler ise 37,7 ł 20,73 ve 84,6 ł 48,31 idi (p<0,05). Amilaz seviyeleri (U/L) için bu değerler, sırasıyla, işlemden 12-24 saat sonra 1000,7 ł 382,46 ve 1592,5 ł 634,39 (p<0,05), 36-48 saat sonra ise 406,2 ł 250,77 ve 660,7 ł 245,75 (p<0,05) olarak bulundu. Sonuç: ERKP sonrası serum CRP seviyelerinin takibi, işlem sonrası pankreatit gelişiminin ve ciddiyetinin öngörülmesinde kullanışlı bir yöntemdir. The prediction of complications and their severity is important to decrease risc and mortality. In previous studies, it has been shown that, CRP provides reliable informations about post-ERCP complications and their severity. In our study, the role of CRP in the prediction and determination of severity of ERCP complications was investigated. Material and Method: 476 patients, whom performed ERCP, were retrospectively evaluated. 136 patients (76 women, 60 men; mean age 62,6 ł 16,89 years) with measurement of serum amylase, lypase and CRP levels before and 12-24 hours after the procedure were included the study and complications and alterations of CRP levels were investigated in these patients. In addition, the role of CRP in the follow-up and prediction of severity of complications was investigated in 22 complicated patients with measurement of serum amylase, lypase and CRP levels 36- 48 hours after the procedure. Complications were classified as mild, moderate, or severe. Results: Post-ERCP complications were detected in 26 patients (19%). Pancreatitis, cholangitis and hemorrhage occured, respectively, in 23 (17%), 2 (1,25%), and 1 (0,75%) patients. Mild and moderate pancreatitis occured, respectively, in 9 and 14 patients. There were significant differences in levels of CRP at 12 to 24 hours for complicated versus uncomplicated patients. The mean CRP levels (mg/l) at 12 to 24 hours were 23,5 ł 24,18 in uncomplicated patients, and 59,2 ł 44,87 in patiens with pancreatitis (p<0,05). There were significant differences in levels of CRP and amylase but no lipase at 12 to 24 and 36 to 48 hours for patients with mild versus moderate pancreatitis. The mean CRP levels (mg/l) at 12 to 24 hours were 30,8 ł 14,24 in mild pancreatitis and 77,5 ł 48,66 in moderate pancreatitis (p<0,05). The levels, respectively, at 36 to 48 hours were 37,7 ł 20,73 and 84,6 ł 48,31 (p<0,05). The mean amylase levels (U/L) at 12 to 24 hours were 1000,7 ł 382,46 in mild pancreatitis and 1592,5 ł 634,39 in moderate pancreatitis (p<0,05). The levels, respectively, at 36 to 48 hours were 406,2 ł 250,77 and 660,7 ł 245,75 (p<0,05). Conclusion: The follow up of serum CRP levels after ERCP is a useful method for predicting and determining of severity of post-ERCP pancreatitis.
