Metisilin dirençli Staphylococcus aureus (MRSA), hastane kaynaklı infeksiyonların en önemli etkenlerinden biridir. Metisilin dirençli Staphylococcus aureus.larda metisilin direncinin en kısa sürede doğru olarak saptanmasının bu infeksiyonların kontrol altına alınmasında ve tedavisinde doğru antibiyotiğin seçilmesinde büyük önem taşımaktadır. Staphylococcus aureus kökenlerindeki metisilin direnci, mecA gen kompleksi tarafından kodlanan değişmiş bir penisilin bağlayan protein olan Penisilin Bağlayıcı Protein2a (PBP2a) ile ilişkilidir. MRSA tanısında konvansiyonel veya moleküler testler kullanılabilir. Günümüzde metisilin direncinin saptanmasında mecA geninin moleküler yöntemlerle saptanması altın standart olarak kabul edilmektedir. Erken tanıya yönelik özel testler ve moleküler yöntemler gibi modern ve hızlı tanı yöntemleri önem kazanmaktadır. Çalışmamızda MRSA'nın fenotipik olarak erken tanımlanmasında, şimdiye kadar kullanılan konvansiyonel yöntemler ile lateks aglütinasyon testi, EVİGENE testi ve PZR testlerinin karşılaştırılması amaçlandı. MRSA olarak tiplendirilen 30 suş çalışmaya alındı. MRSA suşlarının disk difüzyon yöntemiyle antibiyotik duyarlılıkları tespit edildi. Suşların 1 tanesinde kinupristin-dalfopristin direnci, 15 suşta tetrasiklin direnci, 3 tanesinde amikasin direnci ile 10 tanesinde rifampin direnci saptandı. Suşlarda linezolid, teikoplanin ve Vankomisin'e karşı direnç tespit edilmedi. Metisilin dirençli bulunan 30 suşta nitrosefin diskiyle 23 suşta betalaktamaz varlığı tespit edildi. 30 MRSA izolatının 10 tanesinin PZR yöntemiyle mecA geni taşıdıkları tespit edildi. Kırmızı renk değişimi gözlenen 14 suşta EVİGENE yöntemiyle mecA pozitif MRSA izolatları tespit edildi. PBP2a latex aglütinasyon testi ile 10 suşta aglütinasyon gözlendi. Sonuçlara McNemar Ki-Kare testi uygulandı. Çalışmada, mecA geninin tespitinde altın standart kabul edilen PZR yöntemine göre EVİGENE testinin, mecA pozitif ve negatif suşların tespitinde %90 duyarlı, %75 seçici olduğu saptandı. EVİGENE testinin pozitif prediktif değeri %64, negatif prediktif değeri %94 olarak bulundu. Bu çalışmada mecA negatif 20 şuştan 2 tanesinde latex aglütinasyon yöntemiyle metisilin direnci (yalancı direnç) saptandı, PZR yöntemine göre PBP2a lateks aglütinasyon testinin %80 duyarlı, %90 seçici olduğu tespit edildi. PBP2a lateks aglütinasyon testinin pozitif prediktif değeri %80, negatif prediktif değeri %90 olarak saptandı. Sonuçta; EVİGENE yöntemi ve latex aglütinasyon testinin PZR yöntemi ile arasında istatistiksel olarak anlamlı fark bulunmadığı, yeni çalışılan ve hızlı sonuç veren bu yöntemlerin mecA geninin tespitinde kullabileceği düşünüldü. The methicilline resistance on the roots of Staphylococcus aureus is related with penicilline binding protein2a (PBP2a) which is coded by mecA gene complex. For the diagnosis of MRSA, conventional or molecular tests may be used. Presently, detecting of mecA gene with molecular methods for determining the resistance of meticilline, is considered as gold standard. Modern and rapid diagnosis methods for early detection as special tests and molecular procedures has been coming into prominence. In our study, the comparison of, latex agglutination tests, EVIGENE test and PCR tests for the phenotypic early detection of MRSA was aimed. 30 strains of MRSA were included in this study. The antibiotic susceptibilities of MRSA strains were detected by disc diffusion method. Of which, 1 had quinupristin-dalfopristin, 15 stains had tetracycline, 3 has amikacine and 10 had rifampicine resistance. No resistance was detected on the strains, for linezolid, teikoplanin and vancomycin. On the 23 strains of 30 methicilline resistant strains, beta-lactamase existence was found. By performing PCR procedure, it was found that 10 of 30 MRSA isolates carry mecA gene. mecA positive MRSA isolates were detected on 14 strains on which red colour transition was seen. Agglutination was seen on 10 strains by performing PBP2a latex agglutination test. McNemar chi-square test was used for outcomes. In the present study, in comparison with PCR method which is considered as gold standard for detecting mecA gene, it was found that EVIGENE test has 90% sensitivity and 75% specifity on mecA positive and negative strains. The positive predictive value of EVIGENE test was 64% and negative predictive value was 94%. In this sudy, on 2 of 20 mecA negative strains, methicilline resistance (pseudoresistance) was obtained by latex agglutination method; in comparison with PCR procedure, PBP2a agglutination test has 80% sensitive and 90% specifity. The positive predictive value of PBP2a was 80% and negative predictive value was 90%. In conclusion, there were no significantly difference between Latex agglutination test- PCR method, EVIGENE test-PCRmethod, thus, these rapid methods may be used for detecting mecA gene.
