Pankreasın infiltratif duktal adenokarsinomu (PDAK), en ölümcül kanserlerden biridir. Çalışmamızda NGF ve GDNF'nin PDAK ve öncü lezyonlardaki ekspresyonu ve bu belirleyicilerin klinikopatolojik özellikler ve prognoz üzerine etkisini incelemeyi amaçladık. Araştırmamıza 2003- 2010 yılları arasında SDÜ Tıp Fakültesi Patoloji Anabilim Dalı.ında tanı almış 33 PDAK alındı. PDAK komşuluğunda 18 olguda PanIN varlığı tespit edildi. Çalışmaya dahil edilen pankreatektomi uygulanmış 17 olgudan 13 (%76.4).ünde lenf nodu metastazı,15 (%88,2).inde PNI, 9 (%52,9).unda damar invazyonu görülmüştür. Olguların 24 (%72.7).ünde ağrı vardı. Çalışmamızda ağrı skoru yüksek olan (2-3) olgularda sağkalımın ağrısı olmayan veya az olan (0-1) olgulara göre anlamlı şekilde daha kısa olduğu bulundu (p=0,043). Ağrı skoru ile sağkalım arasında anlamlı ilişki tespit etmemize rağmen literatürden farklı olarak PNI ile sağkalım arasında anlamlı ilişki bulunmadı (p>0,05). NGF ekspresyonunun yüksek olduğu olgularda ekspresyonun düşük olduğu olgulara göre sağkalım süresinin daha kısa olduğu tespit edildi. Tümöre komşu pankreas lobül ve duktuslarında kuvvetli NGF (+) pozitifliğinin kötü prognoz ile ilişkili olduğunu saptadık ve tümör derecesi arttıkça tümöre komşu PanIN alanlarında NGF ile daha yaygın boyanma saptadık. GDNF aşırı ekspresyonunun tümör evresi ile ilişkili olduğunu (p=0,016), ileri evre tümörlerde GDNF ekspresyonunun arttığını tespit ettik Tümörde GDNF ekspresyonundaki artışın daha düşük sağkalım gösterdiği fakat bunun istatistiki olarak anlamlı olmadığı bulundu (p>0,05). Çalışmamızda normal pankreasda GDNF.nin aşırı eksprese edildiği, PanIN ve tümöre doğru gidildikçe bu ekspresyonun azaldığı tespit edidi. Çalışmamızda PDAK.ta yaygın GDNF ve NGF ekspresyonları saptamamıza rağmen boyanma şiddet ve yaygınlıkları arasında anlamlı bir ilişki bulunamadı(p>0,05). Bu sonuç bize GDNF ve NGF.nin tümörde farklı etki ettiğini düşündürmektedir. Pancreatic infiltative ductal adenocarcinoma (PDAC), has very high mortality rate. In our study, we examine the expressions of NGF and GDNF in PDAC and precursor lesions. We aimed to investigate the effects of these markers on clinicopathologic features and prognosis. Our cohort consisted of 33 patients with PDAC who were diagnosed at the pathology department of Medical Faculty of Universty of SD between 2003-2010. PanIN was found next to the PDAC in 18 cases. Seventeen patients had pancreatectomy 13 (% 76,4) of them had lymph node metastasis, 15(%88,2) had perineural invasion (PNI), 9(%52,9) of them had vascular invasion. Twenty four (%72,7) of the patients had pain. In our study, the patients who had high scores of pain (2-3) had less survival rates than patients who didn.t have pain or had low scores of pain (0-1). Although we detected a significant relationship between pain score and survival, we didn.t find a significant relationship between PNI and survival contrary to the previous reports (p>0,05). The cases that had high rates of NGF expression had shorte survival. We found that high positive of expression of NGF in the pancreatic lobes and ducts next to the PDAC was related with poor prognosis. There was a positive correlation with NGF nuclear immunoreactivity in PanIN and the grade of PDAC. We found a relationship between GDNF overexpression and tumor stage (p=0,016). Advanced stages of tumors have high GDNF expression rates. High GDNF expressing tumors had worse survival but this was not statistically significant (p>0,05). In normal pancreatic tissue there was overexpression of GDNF and this overexpression was decreasing from PanIN to PDAC. In our study we detected diffuse expressions of GDNF and NGF in PDAC, but there was no significant relationship between intensity extand of staining (p>0,05). We can conclude that GDNF and NGF have different effects on tumor.
