Çalışmamızın amacı, atipik göğüs ağrısı, çarpıntı, çabuk yorulma, göğüste baskı hissi gibi şikayetleri olan, koroner by-pass veya stenti olan hastaların koroner arterlerinin patensi başta olmak üzere diğer patolojilerinin yeni jenerasyon 128 Çok Kesitli Bilgisayarlı Tomografi (ÇKBT) anjiografi ile değerlendirilmesi, koroner arter darlıklarının derecelendirilmesi ve kullanılan radyasyon dozunun optimizasyonudur. Toplam seksenüç hastanın on.u (%12) koroner stent, 8.i (%9,6) koroner bypass-greft patensisi değerlendirilmesi amacıyla koroner BT anjiyografi tetkiki için ünitemize refere edilmişti. Koroner bypass-greft değerlendirilmesi için refere edilmiş hastaların üç.ünde (%3,6) ayrıca toplam 7 adet koroner stent mevcuttu. Geri kalan altmış sekiz hasta (%82) ise düşük-orta riskli hasta grubunda değerlendirilebilecek hastaları kapsamaktaydı. Koroner arterlerin görülebilirliğinin değerlendirilmesinde toplam 1245 segmentin 1211segmenti (%97,26) görülebildi. Koroner BT anjiografi tetkikinde DLP ortalaması 529.91 mGy.cm (9.01 mSv) olarak hesaplandı. DLP en düşük 181 mGy.cm (3,017 mSv) olarak hesaplandı. Çalışmamızda 83 hastanın BT anjiografi tetkiklerinin incelemesinde 32 hasta (%38,55) normal olarak izlendi.19 hastada (%23) hafif dereceli darlıklar, 20 hastada (%24) hafif-orta dereceli darlıklar, 10 hastada (%12,04) ileri derecede darlıklar ve 2 hastada (%2,41) ise koroner arter segmentlerinde oklüde görünüm mevcuttu. Normal olarak değerlendirilen 32 hasta (%38,55) ve hafif dereceli darlıkları bulunan 19 hasta (%23) toplam 51 hastada (%61,55) gereksiz invaziv anjiografi uygulamasından kaçınılmış oldu. Sonuç olarak 128 kesit BT Anjiografi özellikle orta riskli asemptomatik hastalar ve atipik göğüs ağrısı olan hastalarda daha düşük kontrast madde dozu ve hızlı çekim protokolü ile koroner arter darlıklarınının ve varyasyonlarının değerlendirilmesinde etkin bir yöntem olarak kullanılabilir. The aim of our study is evaluation of particularly coronary arteries, coronary artery stenosis grading, coronary by-pass or stent patency, optimization of radiation dose and other pathologies in patients who have complaints such as atypical chest pain, palpitations, fatigue, chest pressure sensation by using the new generation 128 Multi-Slice Computed Tomography (MSCT) angiography. Ten patients (%12) for the coronary stents patency and eight patients (%9,6) for coronary artery bypass-graft patency to assess for coronary CT angiography was referred to our unit in total eighty three patients. Three (%3,6) of patients were referred for evaluation of coronary artery bypass-graft patency also had total 7 coronary stents. The remaining sixty-eight patients (%82) have such as symptoms the non-specific ECG changes, atypical chest pain, palpitations, fatigue, chest pressure sensation in terms of coronary artery disease with low-intermediate-risk group included the patients evaluated. The evaluation of coronary artery segments in the eighty three patients, 1211of 1245 segments (%97,26) could be evaluated. Mean effective radiation dose for coronary CT angiography examination was calculated as 529,91 mGy.cm mean DLP (9,01 mSv). The lowest DLP value was calculated as 181 mGy.cm (3,017 mSv). Calculating of the DLP for calcium scoring average was 81.63 mGy.cm (1,38 mSv). The lowest DLP value for calcium scoring was 34 mGy.cm (0,578 mSv). İn our study of CT angiography examinations of 83 patients, 32 patients examination (%38,55) determined as normal. Nineteen patients (%23) have mild-grade stenosis, 20 patients (%24) have mild to moderate grade stenosis, 10 patients (%12.04) have severe grade stenosis and two patients (%2,41) have the occluded coronary artery segmentsThirty two patients (%38,55) the normally assessed and 19 patients with mild-grade stenosis (%23) of 51 patients (%61,55) were avoided unnecessary administration of invasive angiography. As a result, 128 slice CT angiography can be used as an effective method for the evaluation stenosis and variations of coronary arterias especially in asymptomatic patients with intermediate risk and the patients who have atypical chest pain, with a lower dose of contrast agent and shorter scanning time.
