Hücre hasarının bir mekanizması olarak gösterilen poli (ADP-riboz) polimeraz (PARP) yolağı, pek çok iskemi reperfüzyon hasarı patogenezinde yer almaktadır. DNA'da oluşan iplik kırıklıkları, DNA tamir mekanizmalarında görevli olan PARP enzimini aktive etmektedir. Hafif genotoksik zarar PARP aktivasyonunu indükleyerek DNA tamirini sağlamaktadır. Orta derecede hücre hasarında, tamir edilemez derecede DNA hasarı bulunan hücreler apopitoza yönlendirilmektedir. Şiddetli DNA hasarında ise, PARP aşırı aktive olmakta ve hücre içi enerji metabolizmasını çökerterek hücreyi nekroza sürüklemektedir. PARP.ın hafif ve yüksek DNA hasarı durumunda iki farklı yöndeki etkisi dikkat çekicidir. Bu nedenle çalışmamızda testis torsiyonunda hafif iskemi-reperfüzyon hasarında germ hücrelerindeki apopitozis durumu ve PARP etkinliğinin gösterilmesi amaçlanmıştır. Çalışmamızda 44 adet wistar albino rat kullanılmış ve ratlar kontrol, sham, 1, 2 ve 4 saat torsiyon gruplarına ayrılmıştır. Sol testislere 720ʻ torsiyon yapılıp detorsiyon sonrası 4 saat reperfüzyon uygulanmıştır. Deney sonunda sol ve sağ testislere orşiektomi yapılarak testislerdeki iskemi-reperfüzyon hasarı ve apopitoz derecesi histopatolojik olarak, PARP aktivitesi western blot analizi ile gösterilmeye çalışılmıştır. Çalışmamızda kontrol ve sham gruplarında Johnson Tübüler Biyopsi Skoru (JTBS) normal olmasına rağmen çok düşük düzeyde apopitoz saptanmıştır. 1 saat torsiyon grubunda kontrol ve sham grubuna göre JTBS.nin düştüğü ve apopitoz ortalamasının arttığı tespit edilmiştir. Ancak bu değerlerin yüksek olmadığı 2 ve 4 saat torsiyonlu grupların sağ testis değerlerine yakın olduğu görülmüştür. 2 ve 4 saat torsiyon gruplarında da kontrol, sham ve 1 saat torsiyon grularına göre JTBS.nin düştüğü ve apopitozun arttığı tespit edilmiştir. 1 ve 2 saat torsiyonlu sağ testislerde shama göre JTBS açısından fark olmamasına karşın apopitoz ortalamaları yüksek saptanmıştır. JTBS açısından 2 ve 4 saat torsiyonlu grupların karşı testisleri 1 saat torsiyonlu sol testisle kıyaslandığında istatistiksel anlamlı fark tespit edilmemiştir. Sonuç olarak çalışmamızda testiste saptanabilir en az hasar oluşturulabilmesi için 1 saatlik bir sürenin gerekli olduğu, testiküler iskemi süresi arttıkça orantılı olarak JTBS.nin gittikçe düştüğü ve apopitoz ortalamalarının gittikçe arttığı, 2 ve 4 saatlik torsiyonda karşı testisin 1 saat torsiyonda kalan testis kadar etkilendiği, hafif iskemiye bağlı testis hasarının tespitinde ve karşı testis hasarının tespitinde JTBS.ye göre apopitotik incelemenin daha sensitif olduğu tespit edilmiştir. The ADP-ribose polymerase (PARP) pathway, indicated as a damage mechanism for cells, is implicated in many ischemic reperfusion damage pathogenesis. DNA strand breaks activate the PARP enzyme as part of the DNA repair mechanism. Mild genotoxic damage induces the PARP activation for DNA repair. In moderate cell damage, the cells with excessive DNA damage beyond a repairable level undergo apoptosis. In severe DNA damage, PARP is over activated and steers the cell towards necrosis by crushing the intracellular energy metabolism. Two distinct effects of PARP in mild and severe DNA damage are noteworthy. As a result, in our study, apoptosis response and PARP activity was shown in mild ischemic reperfusion damage in germ cells due to testicular torsion. In our study, a total of 44 Wistar Albino rats were included, and divided into control, sham, as well as 1, 2 and 4 hour torsion groups. Left testes were subjected to 720ʻ of torsion followed by 4 hours of reperfusion. Finally, the ischemic reperfusion damage and the apoptosis level in left and right testes were evaluated histopathologically and the PARP activity was assessed using western blot analysis. In our study, the JTBS was normal in the controls and the sham group, accompanied by very low levels of apoptosis. In the 1 hour torsion group, the JTBS was lower compared to the controls and the sham group and the average apoptosis level was higher. On the other hand, these levels were not drastically high and comparable to the right testis levels of the 2 and 4 hour torsion groups. In the 2 and 4 hour torsion groups, the JTBS was also lower and the apoptosis levels higher in comparison to the controls as well as the sham and 1 hour torsion groups. While there was no difference in the JTBS between the 1 and 2 hour torsion groups and the sham group, the average apoptosis levels of the former were higher. No statistically significant differences were found between the opposite testes of the 2 and 4 hour torsion groups and the left testis of the 1 hour torsion group in terms of the JTBS. In conclusion, the minimum torsion duration for discernible testis damage was determined to be 1 hour, the JTBS was lower and the average apoptosis levels were higher in proportion to the duration of the testicular ischemia, the opposing testis was affected at a level corresponding to a 1 hour torsion when the actual testes underwent 2 and 4 hours of torsion, and the apoptotic examination was more sensitive to determine the testicular damage in mild ischemia and the opposing testicular damage compared to the JTBS.
