Amaç: Çalışmanın amacı, nadir görülmekle birlikte prognozu ve sağkalımı oldukça kötü bir hastalık olan İPF'nin önlenmesinde veya tedavisinde N. sativa'nın etkin olup olmadığını araştırmaktı. Materyal ve metod: Çalışmada, her grupta 8'er sıçan olmak üzere toplam 5 grup mevcuttu. Kontrol grubuna yalnızca intratrakeal salin verildi. N. sativa grubuna günde tek doz oral gavajla N. sativa yağı verildi. Bleomisin ve tedavi gruplarına 0. günde tek doz intratrakeal bleomisin (2,2 mg/kg) uygulandı. Tedavi gruplarından birine 0-16., diğerine 8-16. günler arasında oral gavaj ile N. sativa yağı verildi. Çalışma sonunda tüm sıçanların akciğer dokusu histopatolojik olarak incelendi; hem akciğer dokusunda hem de BAL sıvısında GSH, GSH-Px, SOD enzim aktiviteleriyle birlikte Il-6, TNF-., TGF-. ve MDA düzeylerine bakıldı. Bulgular: Histolojik değerlendirmede inflamasyon, bleomisin grubunda %45,6; uzun tedavi kolunda %19,3 ve kısa tedavi kolunda %14,1 oranında görüldü. Bleomisin grubu ve tedavi kolları arasındaki fark istatistiksel olarak anlamlıydı (sırasıyla p=0,019; p=0,003). Bleomisin grubunda fibrozis skoru 1,6 olup bu skor uzun süreli tedavi kolunda 0,78 ve kısa süreli tedavi kolunda 0,35 idi ve aradaki fark istatistiksel olarak anlamlıydı (sırasıyla p=0,004; p=0,0001). İntratrakeal bleomisin verilmesi, kontrol grubuyla karşılaştırıldığında akciğer dokusu ve BAL sıvısında GSH-Px ve GSH aktivitelerinde istatistiksel olarak anlamlı azalma ve MDA düzeylerinde anlamlı artış ile sonuçlandı. Her iki tedavi grubunda MDA düzeyi anlamlı olarak azalmıştı (p<0,01). Tedavi gruplarında hem doku hem de BAL örneklerinde enzim aktiviteleri bleomisin grubuna göre yüksekti ve BAL sıvısında GSH-Px enzim aktivitesi için bu fark, istatistiksel olarak anlamlıydı (p<0,01). Tedavi kolları karşılaştırıldığında BAL sıvısında GSH-Px enzim aktivitesindeki artış kısa süreli tedavi kolunda daha belirgindi (p<0,01). BAL sıvısında GSH enzim aktivitesi kısa süreli tedavi kolunda bleomisin grubuna göre daha yüksekti (p=0,0001). TGF-. düzeyi bleomisin grubunda kontrol grubuna göre artmıştı ve bu artış istatistiksel olarak anlamlıydı (p<0.01); tedavi gruplarında ise bleomisin grubuna göre azalmıştı. Hem doku hem de BAL örneklerinde tedavi grupları ve bleomisin grubunda SOD enzim aktivitesi ve IL-6 düzeyi arasında anlamlı bir fark saptanmadı. TNF-. düzeyi beklenenin aksine bleomisin grubunda azalmıştı; tedavi grupları ve kontrol grubunda TNF-. düzeyleri benzerdi. Sonuç: Antioksidan ve antiinflamatuvar özelliği birçok çalışmada gösterilmiş olan N. sativa, hem inflamasyon ve fibroziste azalma hem de antioksidan enzim aktivitelerinde artış sağlaması nedeniyle İPF tedavisinde veya hastalığın önlenmesinde etkin olabilir. Bu etkinin daha kesin bir şekilde gösterilebilmesi için, N. sativa.nın farklı dozlar ve sürelerde uygulanacağı başka çalışmalara ihtiyaç vardır. Aim: The aim of the study was to investigate the effect of N. sativa on the prevention or treatment of IPF which although a rare disease has a poor prognosisand survival. Materials and method: The study included 5 groups each containing 8 rats. Only intratracheal saline was administered to the control group. N. sativa group was given a single dose of N. sativa oil by oral gavage. A single dose of intratracheal bleomycin (2.2 mg/kg) was administered to the bleomycin group (BG) as well as the treatment groups on day 0. N. sativa oil was given by oral gavage between days 0-16 in one treatment group and between days 8-16 in the other. At the end of the study,the lung tissues of all rats were investigated histopathologically; GSH, GSH-Px, SOD enzyme activities as well as IL-6, TNF-., TGF-. and MDA levels were measured in lung tissues and BAL fluids. Results: On histologic examination, the ratio of inflammation was observed as 45.6% in the BG while it was 19.3% and 14.1% in the long and short treatment arms. The difference between treatment arms and BG being statistically significant (p=0.019; p=0.003, respectively). The fibrosis score of the BG was 1.6 while it was 0.78 in the long treatment arm and 0.35 in the short treatment arm; and the difference was statistically significant (p=0.004; p=0.0001, respectively) The intratracheal administration of bleomycin resulted in a statistically significant decrease in the GSH-Px and GSH activities and increase in MDA levels of the lung tissue and BAL fluid in comparison with the control group. The MDA level was decreased in both treatment groups (p<0.01). The enzyme activities in both tissue and BAL samples of both treatment arms were high with respect to the BG, the difference being statisticaly significant for GSH-Px enzyme activity of the BAL fluid (p<0.01). When the treatment arms were compared, the increase in the GSH-Px activity in BAL fluid was more prominent in the short treatment arm (p<0.01). The GSH enzyme activity in BAL fluid was higher in the short treatment arm with respect to the BG (p=0.0001). The level of TGF-. was increased in BG with respect to the control group and the increase was statistically significant (p<0.01) and it was found to be decreased in treatment groups. The difference between SOD activities and IL-6 levels of the bleomycin and treatment groups were not statistically significant in both tissue and BAL samples. TNF-. was unexpectedly decreased in BG, however the level of TNF-. was similar between control and treatment groups. Conclusion: N. sativa, of which the anti-oxidant and anti-inflammatory characteristics have been shown in numerous studies, may be effective in the treatment or prevention of IPF since it resulted both a decrease in inflammation and fibrosis and an increase in the antioxidant enzyme activities.
