Bu çalışmanın amacı subakut ve kronik non-spesifik bel ağrılı (NSBA) hastalarda ekstansiyon kontrollü lomber korse ve elastik korsenin bel ağrısı, bel fleksibilitesi ve fonksiyonel dizabilite üzerine olan etkinliğini araştırmak, ekstansiyon kontrollü lomber korse ve elastik korseyi etkinlik ve hasta uyumu açısından karşılaştırmaktır.Subakut ve kronik NSBA'sı olan 119 hasta randomize edilerek 3 gruba ayrıldı. Birinci gruba (n=40) ekstansiyon kontrollü lomber korse, 2. gruba (n=40) elastik lomber korse, günde ardarda 6 saat, 3 hafta boyunca kullanılacak şekilde reçete edildi. Grup 3'e (n=39) lomber korse reçete edilmedi. Bütün hastalar düzenli olarak 1000 mg/gün naproksen kullandı ve her gruba bel ağrılarından korunma yolları ile ilgili eğitim verildi. Ağrı şiddeti Vizüel Analog Skala (VAS) ile bel fleksibilitesi Schober testi ile ve bel dizabilitesi Roland-Morris Dizabilite Sorgulaması (RMDS) ile değerlendirildi. Reçete edilen sürede korseyi kullanan hastalar uyumlu olarak tanımlandı.Üçüncü haftada tüm gruplarda VAS skoru, Schober testi ve RMDS'de açısından iyileşme görüldü. Grup 1 ve grup 2'deki VAS skoru farkı grup 3'ten daha fazla idi (sırasıyla, p=0,0001, p=0,001). Grup 1 ve grup 2 arasındaki fark ise istatistiksek olarak anlamlı değildi (p=0,847). Gruplar arasında Schober ölçümü değişim farkları açısından istatistiksel olarak anlamlı fark yoktu (p=0,148). RMDS'de ise grup 1 ve grup 2 arasındaki fark grup 3'e göre istatistiksel olarak anlamlı idi.( sırasıyla, p=0,0001, p=0,018). Grup 1 ve grup 2 arasındaki fark istatistiksel olarak anlamlı değildi (p=0,063). Grup 1'de 22 hasta, grup 2'de 16 hasta korse kullanım süresine uyum göstermişti. Gruplar arasında uyum açısından istatistiksel olarak anlamlı fark yoktu (p=0,157). Korseye bağlı sorun varlığı ile korseye uyum arasında negatif korelasyon vardı (r=-0,404, p=0,003).Subakut ve kronik NSBA'da korse tedavisinin SOAİİ'larla beraber kullanımı sadece SOAİİ'lara göre kısa dönemde ağrı şiddeti, dizabilite, lomber mobilite üzerine olan etkileri bakımından daha üstündür. Korse tiplerinde ağrı şiddetini ve dizabiliteyi azaltma açısından birbirlerine üstünlükleri yoktur. Korse tedavisinde hasta uyumu düşük bulunmuştur.
The aim of this study is to assess the effectiveness of extension-controlled lumbar support and elastic lumbar support on low back pain, lumbar flexibility and functional disability in subacute and chronic non-specific low back pain (NSLBP) patients and compare extension-controlled lumbar support and elastic lumbar support for efficiency and patient compliance.One hundred and nineteen chronic and subacute non-spesific low back pain patients allocated into 3 groups randomly. Group 1 (n=40) was prescribed to wear extension-controlled lumbar support; group 2 (n=40) was prescribed to wear elastic lombar support; both for the consecutive hours of the day, for 3 weeks. Lumbar support was not prescribed for group 3 (n=39). All patients used 1000 mg/day naproxen regularly and in both groups, patients were educated about prevention methods from back pain. Pain severity was evaluated with Visual Analog Scale (VAS), back flexibility was evaluated with Schober test and back disability was evaluated with Roland-Morris Disability Questionnaire (RMDQ). Compliance was defined as wearing the support during the time it was prescribed.All groups improved in terms of VAS score, Schober test and RMDQ score at 3 weeks (p<0.001). Reduction in VAS score was more evident in group 1 and 2 than group 3 at 3 weeks (p=0,0001 and p=0,001, respectively). There was no difference for the improvement in VAS score between group 1 and group 2 (p=0,847). There was no difference between the groups for decrease in Schober test. Reduction in RMDS score was more evident in group 1 and 2 than group 3 at 3 weeks (p=0,0001, p=0,018, respectively). There was no difference for the improvement in RMDS score between group 1 and group 2 (p=0,063). Twenty-two patients from group 1 and 16 patients from group 2 wore the lumbar support as it was prescribed. There was not statistically significant difference for the frequency of compliance. Complaints were found to be negatively correlated with the compliance with the lomber support (r=-0,404, p=0,003).Extension-controlled lumbar support and elastic lumbar support was more efficient than not wearing a lumbar support for decreasing low back pain and back disability in subacute and chronic NSLBP. No efficiency difference was detected between extension-controlled lumbar support and elastic support. Patient compliance was repoted low in the corset treatment.
