Oksidatif stres ve lipid peroksidasyonunun, Henoch Schönlein Purpurası (HSP) patogenezindeki rolü birçok çalışmada gösterilmiştir. Bu çalışmada oksidatif stresin HSP patogenezindeki rolünü belirlemek ayrıca antioksidan enzimlerden olan paraoksonaz (PON1) ve mangan süperoksit dismutaz (MnSOD).a ait gen polimorfizmlerinin HSP ile ilişkisini araştırmak amaçlanmıştır. Çalışmaya 70 HSP.li hasta (27 erkek, 43 kız) ve 60 sağlıklı çocuk alınmıştır. 70 HSP'li hastanın 16.sında, hastalığın aktif ve iyileşme döneminde oksidatif stres ve biyokimyasal belirteçlere bakılmış, kontrol grubundan 14 sağlıklı çocuğun verileri ile karşılaştırılmıştır. Çalışmanın son bölümünde her iki grubun PON1 Q/R192, L/M55 ve MnSOD A16V gen polimorfizmlerine değerlendirilmiştir. İdrarda n-asetil-.-D-glukozaminidaz (NAG) düzeyi aktif dönemde olan hastalarda, kontrol grubuna göre yüksek bulunmuştur. Hastalığın aktif döneminde bulunan hastalarda bakılan tübüler fosfor reabsorpsiyonu (TPR) oranı iyileşme döneminde olan hastalara göre düşük bulunmuştur. Bu bulgular, HSP.nin aktif döneminde proksimal tübüler hasarın varlığını ortaya koymaktadır. Oksidatif stres belirteçlerine bakıldığında; hastalığın aktif döneminde malondialdehit (MDA)düzeyleri artmış, PON aktivitesi ve total antioksidan kapasite azalmıştır. Bu bulgular, oksidatif stres ve lipid peroksidasyonunun HSP patogenezinde rolü olduğunu desteklemektedir. PON1 Q/R192 genotiplerinin dağılımı değerlendirildiğinde, HSP grubunda QQ genotipi sıklığı kontrol grubuna göre daha yüksek olsa da bu fark istatistiksel olarak anlamlı bulunmamıştır. Allel dağılımında hasta grubunda Q allel sıklığı, kontrol grubuna göre yüksek bulunmuştur (p<0,05). Bu bulgu, Q allelinin HSP gelişiminde rol oynayabileceğini düşündürmektedir. PON1 L/M55 genotiplerinin dağılımına bakıldığında, HSP grubunda MM genotip sıklığının kontrol grubuna göre daha yüksek olduğu bulunmuştur (p<0,05). Allel dağılımında hasta grubunda M allel sıklığı, kontrol grubuna göre daha yüksek saptanmıştır. Bu bulgu M allelinin, HSP gelişiminde bir risk faktörü olduğu söylenebilir. MnSOD A16V genotiplerinin dağılımına bakıldığında ise HSP grubunda AA genotip sıklığı kontrol grubuna göre yüksek olsa da bu fark istatistiksel olarak anlamlı değildir (p>0,05). Sonuç olarak, bizim bulgularımız oksidatif stres ve lipid peroksidasyonunun HSP patogenezinde rolü olduğunu, PON1 ile ilişkili PON1 Q/R192 ve PON1 L/M55 polimorfizmlerinin HSP gelişme riski ile ilişkili olduğu ancak MnSOD A16V polimorfizmi ile HSP gelişme riski arasında kesin bir ilişki olmadığını düşündürmektedir. Yapılacak daha geniş ve kapsamlı çalışmalarla bulgularımızın karşılaştırılmasının uygun olacağını düşünmekteyiz. Anahtar Kelimeler : Gen Polimorfizm, HSP, MnSOD, PON.
The role of oxidative stress and lipid peroxidation in Henoch Schönlein Purpura (HSP) pathogenesis is shown by many researches. In this study, we aimed to state the role of oxidative stress in the pathogenesis of HSP, also to investigate the relation between gene polymorphisms of the antioxidant enzymes PON1 and MnSOD with HSP. Seventy patients with HSP (27 male, 43 female) and 64 healthy children as control group were enrolled in this study. Sixteen of 70 patients with HSP, oxidative stress and biochemical markers in active and remission periods of disease were evaluated and compared with the results of 14 healthy children from control group. At last part of the study, PON1 Q/R192, L/M55 and MnSOD A16V gene polymorphisms of both groups were evaluated. Urine n-asetil-.-D-glukozaminidase (NAG) levels were higher in patients with active period of the disease than control group. Tubular phosphorus reabsorption (TPR) ratio which is evaluated in patients within active period of the disease, was lower than patients within remission period of the disease. This findings affirmed the presence of proximal tubular injury during the active period of HSP. When considering oxidative stress markers; malondialdehyde (MDA) levels in active period of the disease increased, PON activity and total antioxsidation capacity (TAC) levels decreased. These findings support the presence of the role of oxidative stress and lipid peroxidation in HSP pathogenesis. When we assessed distribution of PON1 Q/R192 genotypes, although QQ genotype frequency was higher in HSP group than control group, this difference was not statistically significant. In allele distribution, Q allele frequency was higher in patient group than control group (p<0,05). This finding suggests possible role of Q allele in developing of HSP. In view of PON1 L/M55 genotype distribution, MM genotype frequency was higher in HSP group than control group (p<0,05). In allele distribution of patient group, M allele frequency was higher than control group. Through this finding, it might be said that, M allel may be a risk factor for developing of HSP. In view of MnSOD A16V genotype distribution, AA genotype frequency higher in HSP group than control group but this difference was not statistically significant(p>0,05). In conclusion, our findings indicate the current role of oxidative stress and lipid peroxidation in HSP pathogenesis, PON1 Q/R192 and PON1 L/M55 polimorphisms related with PON1 might have relation with the risk of HSP development whereas the relation between MnSOD A16V polimorphisms with the risk of developing HSP is not definite.
