İnme, gelismis ülkelerde tüm ölüm nedenleri arasında 3. sıklıktadır. Ve olguların %75'inde internal karotis arteri kaynaklıdır. Serebral tromboembolik olayların yaklasık %90'ının nedeni aterosklerozdur. Hastalığın yaygınlığı ve yol açtığı ciddi sekeller nedeniyle tanısı ve tedavisi büyük önem tasımaktadır. Karotis arterlerinin değerlendirilmesinde altın standart günümüzde sayısal çıkarma anjiografidir. Ancak bu yöntemin bilinen tromboemboli riski ve maliyeti nedeniyle alternatif görüntüleme yöntemlerine olan ilgi giderek artmaktadır. zotropik vokseller, yüksek spatial ve temporal rezolüsyona sahip olması, hızlı kontrast madde enjeksiyonu yapılabilmesi ve geliştirilen post-processing araçları ile ÇKBTA'nın karotis arter stenoz oranlarının değerlendirilmesi ve plak karakterizasyonundaki sensitivitesi ve spesifitesi artmıştır. Kontrast maddeye karsı gelişebilecek olası kemotoksik reaksiyonları azaltmak için bu çalısmamızda, karotise yönelik ÇKBTA çekimlerinde kullanılan standart doz kontrast madde yerine daha düsük dozda kontrast madde kullanılabilirliğini arastırmayı hedefledik. Karotis BTA istemiyle başvuran 41 olgunun iki gruba ayrıldı. Grup A olgulara 1 ml/kg, Grup B olgulara 0,5 ml/kg dozunda kontrast madde verilerek karotis BTA.ler elde olundu. 41 hastada, 615 arteryel segment değerlendirildi. Olguların hepsinde tüm arteryel segmentlerde görüntü kalitesi diagnostik açıdan yeterli bulundu. Grup A olgularda ortalama arteryel atenüasyon değeri 322,33+/-78,55 HU, grup B olgularda ise 352,56+/-106,99 HU idi. statiksel olarak, p değeri 0,05 alındığında, iki grup arasında anlamlı farklılık saptanmadı (Mann-Whitney U= 182,000 p=0,465). Grup A olgularda 8 segmentte ılımlı stenoz, 3 segmentte ciddi stenoz tesbit edilirken oklüzyon saptanmadı. Grup B olgularda ise 3 segmentte oklüzyon, 3 segmentte ciddi stenoz, 25 segmentte ise ılımlı stenoz saptandı. Karotis arterlerin 128 kesitli BTA ile değerlendirilmesinde kullanılan standart kontrast madde miktarı azaltılarak görsel ve sayısal anlamda uygun görüntüler elde edilebilir. Stroke is the third major cause of death in the developed world. İn the 75% of the cases it.s originated from carotid artery. Cerebral thromboembolic disease is commonly caused by atherosclerosis. Therefore, it is common and cause disability, the diagnosis and the treatment is so important. Today, digital subtraction angiography (DSA) is a gold standart in evaluation of the carotid artery. As known, because of the thromboembolism risk and the cost of this method, there is an increasing interest in the alternative imaging techniques. By the, izotropic voxels, high spatial and temporal resolution, fast contrast media enjection rates, and developed postprocessing tools, specificity and sensitivity of the multislice CT in the evaluation of the carotid artery and plaque characterization is being increased. In order to decrease the possible chemotoksic reactions, in this study, we aimed to research availability of the low dose contrast media instead of standart dose, in the evaluation of the carotid artery with multidedector CT. 41 patients were divided into two groups which were referred to have CT angiography of the carotid artery. Patients in group A and group B had contrast media with a dose of 1 ml/kg and 0,5 ml/kg respectively. In 41 patients, 615 segments of carotid artery were evaluated. In all cases, the image quality was adequate for diagnosis in all arterial segments. The mean arterial attenuation values in group A and B were 322,33+/-78,55 HU and 352,56+/-106,99 HU respectively. While p= 0,05, there was no significan differences statistically in the two groups(Mann-Whitney U= 182,000 p=0,465). In group A, 8 mild stenoses, 3 severe stenoses and no occlusion was dedected. İn group B, 3 occlusion, 3 severe stenoses and 25 mild stenoses were dedected. With reducing the standard amount of contrast media for CT angiography of the carotid artery, visual and numerical appropriate images can be obtained.
