Kserostomi erişkin hastalarda ekstraözefageal reflü (EÖR) ve EÖR komplikasyonlarına yatkınlık oluşturabilir. EÖR'ün kulaktaki komplikasyonları östaki tüp disfonksiyonu (ETD) ve effüzyonlu otitis mediadır. Bununla birlikte literatür kserostomi ile orta kulak hastalıkları arasındaki bağlantıyı gösteren çalışmalardan yoksundur. Bu çalışma ile sialoadenektomili ratlarda tekrarlayan nazofarengeal HCl/Pepsin infüzyonun östaki tüpüne (ET) etkilerini ve HCl/Pepsin.e karşı yapay tükürüğün koruyucu etkilerini değerlendirdik. Toplam 43 albino rat HCl/Pepsin ve/ veya yapay tükürük verilip verilmemesine veya sialoadenektomi uygulanıp uygulanmamasına göre 5 gruba ayrıldı. İlaç infüzyonu amacıyla ratların tümünün nazofarenksine polietilen tüpler yerleştirildi. Gruplardan üçüne sialoadenektomi uygulandı. Gruplardan üçüne HCl/Pepsin uygulamasına başlandı. HCl/Pepsin uygulamasının 7nci gününde tüm hayvanlar sakrifiye edilerek ET'lerine ait kesitler alındı. Goblet hücre yoğunluğu, submukozal inflamatuar hücre infiltrasyonu, submukozal bezlerdeki hasar, hipertrofi ve bağ doku artışı olmak üzere 5 parametre gruplar arasında karşılaştırıldı. Deney gruplarındaki goblet hücre yoğunluğu kontrol grubuna göre değişmedi, diğer parametreler arttı. Sialoadenektomi bezlerdeki hipertrofiyi ve HCl/Pepsin uygulaması da inflamasyonun şiddetini belirgin olarak arttırdı. Sonuç olarak, kserostomi ve nazofarengeal gastrik reflünün ET üzerine, EÖR'ün erken dönemlerinde bile submukozal bez hipertrofisini, inflamasyonun şiddetini ve bağ dokusunu arttırarak ETD oluşturmasını sağlayan sinerjik etkileri görüldü. Tükürük salgılama fonksiyonu kalmayan EÖR.lü hastalara komplikasyonları engellemek amacıyla yapay tükürük preparatları önerilebilir, bununla birlikte en uygun tedavinin belirlenmesi ileri çalışmaların amacı olmalıdır. Xerostomia may predispose to extraesophageal reflux (EER) and EER complications in adult. The otologic complications due to EER are eustachian tube disfunction (ETD) and otitis media with effusion. However literature is void of articles adressing the relationship between xerostomia and middle ear disease. In the present study, we investigated the effect of repetitive nasopharyngeal HCl/Pepsin infusion on eustachian tube (ET) and protective effects of artificial saliva against HCl/Pepsin in sialoadenectomised rats. Total of 43 albino rat were divided into five groups according to being exposed to HCl/Pepsin and/or artificial saliva, or being sialoadenectomised or not. Polyethylen tubes were inserted into the nasopharynx in all rats for medicine infusion. Rats in three groups were sialoadenectomised. In three groups HCl/Pepsin exposure was performed. On the seventh day of HCl/Pepsin exposure all animals were sacrified and cross section of ETs were obtained. Five parameters were compared among the groups: goblet cell density (GCD), submucosal inflammatory cell infiltration, hypertrophy and depredation level of the submucosal glands and submucosal connective tissue thickening. GCD in the experimental groups didn.t differ from the control group, the other parameters increased. Sialoadenectomy increased gland hypertrophy and HCl/Pepsin exposure increased the inflammation severity markedly. Eventually xerostomia and nasopharyngeal HCl/Pepsin exposure had a synergistic effect upon ET, by triggering submucosal gland hypertrophy, inflammation severity and thickening connective tissue that leads to ETD even in early stages of HCl/Pepsin exposure. Patient with EER which fail to secret saliva may be advised saliva substitudes to avoid complications, however to determine the convenient treatment further studies are required.
