Giriş: Açık kalp cerrahisinde tam median sternotomi sonrasındaki sternal dehissens, sternal yara enfeksiyonları, osteomyelit ve mediastinit gibi yara iyileşme bozukluklarına neden olabilir. Bu çalışma sternum band kapama tekniğinin etkili ve güvenli bir sternum tespit yöntemi olup olmadığını değerlendirmek ve geleneksel sternum kapama yöntemleri ile karşılaştırmak için yapılmıştır.Yöntem: İki sternum kapama tekniğinin karşılaştırıldığı prospektif çalışmada 110 hasta grup A (n= 55) ve grup B (n= 55) olmak üzere ayrıldı. Çalışma; Ocak 2011 ve Haziran 2012 tarihleri arasında her iki tekniğin bu hastalardaki sternal dehissens, sternal ağrı, sternal yara enfeksiyonları ve mediastinit için sonuçları karşılaştırıldı.Bulgular: Sternal band kapama yönteminin kullanımı sternal ağrı indeksinde anlamlı bir düzelme sağlamıştır (p < 0.05). Geleneksel teknik grubunda sadece bir vakada yüzeyel yara enfeksiyonu ortaya çıkarken, sterna band grubunda ortaya çıkmamıştır (p > 0.05). Sternal dehissens sıklığı geleneksel teknik grubunda daha yüksek olduğu halde bu komplikasyon için istatistiksel olarak anlamlı bir fark bulunamamıştır ( p > 0.05 ).Sonuç: Bu çalışma, median sternotomi kapatılması için sterna band kullanımının basit, güvenilir ve tekrar edilebilir bir teknik olduğunu göstermektedir. Background: Sternal dehiscence after full median sternotomy in open cardiac surgery may result in wound-healing disorders, including sternal wound infections, osteomyelitis and mediastinitis. This study was carried out to evaluate whether the sterna band closure technique is an effective and safe fixation of sternum in cardiac procedures and to compare with the conventional sternal- closure method.Methods: A prospective study of two sternal closure techniques in population of 110 patients divided into: group A, conventional technique closure (n= 55) and group B, sterna band closure (n= 55) ,was carried out between January 2011 and June 2012 in order to describe and compare the results of both techniques in these patients for sternal dehissence, sternal pain, sternal wound infections and mediastinitis.Results: The use of sterna band closure produced significant improvement in sternal pain index (p < 0.05). Superficial wound infection appeared in only one case of conventional technique group and no case of sterna band group. (p> 0.05). Although the frequency of sternal dehissence was higher in conventional technique group, a statistically significant difference could not be estabilished in terms of this complication (p> 0.05).Conclusion: This study demonstrates that the use of sterna bands for median sternotomy closure is a simple reliable and reproducible technique.
