Tıkanma sarılığı; endotoksemi, inflamuar sitokinler, reaktif oksijen radikalleri, lipit peroksidasyonu gibi patofizyolojik faktörler aracılığıyla multiorgan hasarına neden olan bir sepsis modelidir. Bu çalışma, ortak safra kanalı ligasyonu yoluyla tıkanma sarılığı geliştirilen bir hayvan modelinde L-karnitin’ nin TS da oluşan akciğer hasarını önlemedeki etkinliğini araştırmak amacıyla yapılmıştır.Otuz sekiz adet Wister Albino rat, kontrol, tıkanma sarılığı, tıkanma sarılığı + L-karnitin grubu olarak üçe ayrıldılar. I. gruba sadece laparatomi uygulandı. II. gruba ana safra kanalı ligasyonu yapıldı. III. gruba ise ana safra kanalı ligasyonu ve günde 1 kez, 50 mg/kg dozunda L-karnitin 10 gün boyunca iv. şekilde kuyruk veninden verildi. Çalışmanın birinci ve onuncu günlerinde biyokimyasal inceleme için 1 cc kan alındı. Onuncu gün sonunda ratlar sakrifiye edildi. Biyokimyasal ve histopatolojik inceleme için akciğer dokusu alındı.Sistemik inflamatuar cevabı değerlendirmek için kanda TNF-α, IL-6, IL-8 ve CRP düzeylerine baktık.Grup 1 ve 2 de, uzak organ hasarının göstergesi olan IL-8 açısından anlamlı artış saptanırken grup 3 de anlamlı artış saptanmadı. TNF-a, IL-6 ve CRP açısından gruplar arasında anlamlı fark saptanmadı.Akciğer dokusunda, lipid peroksidasyonunun son ürünü olan malondialdehit (MDA), PMN stimülasyonunun indeksi olan Myeloperoksidaz (MPO), anahtar antioksidan glutatyonu okside eden Glutatyon peroksidaz (GSH-Px) ve yine serbest oksijen radikallerini yok eden Katalaz (CAT) ve Süperoksit dismutaz (SOD) enzim seviyeleri araştırıldı.MDA, CAT değerlerinde, gruplar arasında anlamlı fark saptanmaz iken, antioksidan etkinliğin en önemli göstergesi olan GSH-Px açısından L-karnitin grubunda diğer tıkanma sarılığı grubuna göre anlamlı düşük değer saptandı. Ayrıca L-karnitin’ in antioksidan etkisi ile GSH-Px açısından laparotominin etkilerini bile geri çevirdiği görüldü.MPO'nun ise tıkanma sarılığında arttığı, fakat L-karnitin grubu ile diğer tıkanma sarılığı grubu arasında anlamlı fark olmadığı görüldü.SOD değerlerine baktığımızda L-karnitin verilen tıkanma sarılıklı grubun değerlerinin kontrol grubuyla aynı, tedavi verilmeyen grupta ise daha yüksek olduğu saptandı. Fakat her üç grup arasındaki SOD değerleri farkı istatistiksel olarak gösterilemedi.Alınan akciğer dokularının histopatolojik incelemesinde kullanılan skorlama sistemi sonucunda L-karnitin verilen tıkanma sarılığı grubunda, diğer tıkanma sarılığı grubuna göre anlamlı derecede akciğer hasarı düşük düzeyde bulundu. L-karnitin tıkanma sarılığı grubunu, akciğer hasarı yönünden kontrol grubu düzeyine çekmiştir.Sonuç olarak, L-karnitin TS da AC hasarını histopatolojik ve biyokimyasal parametreler ışığında önler. Obstructive jaundice is a sepsis model that cause a multi-organ damage through the pathophysiologic factors such as endotoxemia, inflamuar cytokines, reactive oxygen radicals, and lipid peroxidation. This study is prepared to evaluate the efficiency of L-carnytine to prevent lung injury in an animal model with obstructive jaundice which provided by ligation common bile duct.Thirty-eight Wister albino rats, were divided into three as the control, obstructive jaundice, and obstructive jaundice + L-carnitine group. Group I. was only performed laparotomy. Group II. was applied the main bile duct ligation. Group III. was performed the main bile duct ligation from tail vein shaped the iv., and 50 mg / kg dose of L-carnitine for 10 days 1 times a day. 1 cc of blood were taken for biochemical analysis at the first and tenth days of the study. Rats were sacrificed at the end of the tenth day. Lung tissue were taken for biochemical and histopathological examination.We looked at TNF-a, IL-6, IL-8 and CRP levels to assess the systemic inflammatory response in the blood.In groups 1 and 2, IL-8, as an indicator of the remote organ injury was detected a significant increase, in group 3 wasn’t significant increased. In terms of TNF-α, IL-6 and CRP were not significantly different between the groups.The enzyme levels of malondialdehyde (MDA) which is the end product of lipid peroxidation, myeloperoxidase (MPO) the index of the PMN stimulation, glutathione peroxidase (GSH-Px) which oxidized the key antioxidant glutathione and catalase (CAT) and superoxide dismutase (SOD) that destroy free oxygen radicals were investigated in lung tissue.While no significant difference values between the groups MDA, CAT was detected, the most important indicator of antioxidant activity of GSH-Px, L-carnitine group value was significantly lower than in other obstructive jaundice. Also, L-carnitine in terms of antioxidant effect of GSH-Px was turned back even if the effects of laparotomy. MPO increases in the obstructive jaundice, but with L-carnitine group showed no significant difference between the other obstructive jaundice group.Looking at the values of SOD, given L-carnitine obstructive jaundice with the control groups are the same, the values were higher in the group without treatment. But difference in SOD values between the three groups wasn’t statistically showed.In addition, to histopathologic examination of lung tissue is taken that found a significantly lower level of lung injury between the group of obtructive jaundice compared L-carnitine to other group obstructive jaundice. L-carnitine has taken the obstructive jaundice group to the level of the control group in terms of lung injury.As a result, L-carnitine prevents obstructive jaundice at the lung injury in the light of histological and biochemical parameters.
