Amaç: Kronik obstrüktif akciğer hastalığı (KOAH) ve obstrüktif uyku apne sendromu (OUAS) arasındaki ilişkiyi ve noktürnal non invaziv ventilasyon (veya BPAP) tedavisinin etkinliği araştırılmak istendi.Gereç ve Yöntemler: Ocak 2007 ile Aralık 2010 tarihleri arasında KOAH tanısı ile hastanemizde yatan 2485 hasta ve Uyku Bozuklukları Merkezimize başvuran 407 hastanın dosyaları Overlap sendromlu hastaları bulabilmek için retrospektif olarak araştırıldı. Çalışma popülasyonu (99 hasta) üç gruba bölündü ;Grup 1: OUAS’lı hasta (n=40) (AHI≥5 olay/saat), Grup 2: KOAH ile birlikte OUAS’lı hastalar (Overlap sendromu) (n=40) ve Grup 3: AHI<5 olay/saat olan kontrol grubu (n=19) Bulgular: Çalışmaya dahil edilen hastaların yaşları 19-89 yıl arasında değişmekteydi. Overlap sendromlu hastalar daha yaşlı idi (p<0.01). Overlap sendrom tanısı konulan olgular ortalama 8.6 ±1.26 yıldır KOAH tanısı ile tedavi gören, büyük çoğunluğu Evre II KOAH (%47.5) idi. Overlap sendromlu olguların hepsi sigara kullanmakta ve daha kilolu idiler. Pulmoner hipertansiyon Overlap sendromlu hastaların 29’unda, OUAS hastaların 5’inde saptandı. Overlap sendromlu olgularda uyku etkinliği (p<0.05), REM (%) (p<0.05), Dİ (p<0.01) ve Evre 3(%) azalmış, Evre 2 (%) artmıştı. Olguların %60’ında AHİ ≥30 olay/saat bulundu (p<0.01). BPAP tedavisi sonrasında pulmoner arter basıncı (p<0.05) ve alevlenme sayılarında azalma saptandı. VKİ‘nin 30 kg /m2 altında olması (p<0.05) ve Dİ≥30 olay/saat (p>0.05) yaşam süresini kısalmaktaydı.Sonuç: KOAH’lı hastalarda OUAS birlikteliği noktürnal desatürasyonları derinleştirmekte ve gündüz hipoksemisi ve hiperkapniyi etkilemektedir. Hipoksemi sonucu oluşan pulmoner hipertansiyon prognozu olumsuz etkilemektedir.Çalışmamızda BPAP tedavisinin pulmoner hipertansiyonu düzeltmesi ve alevlenme sayılarını azaltması prognozu olumlu etkilediğini düşünmekteyiz. Çalışmamız Overlap sendromlu hastalardaki uyku mimarisindeki değişiklikler literatürle benzerlik göstermesine karşın, BPAP tedavisinin etkinliğini göstermesi bakımından ilk çalışmalardan biridir. KOAH’lı hastalarda prognozu olumlu etkilediği düşünülen yüksek VKİ’nin ise Overlap sendromlu olgularda ayrıntılı araştırılması gerekmektedir. Anahtar Kelimeler : KOAH, polisomnografi, prognoz.
Aim: We want to investigate the relationship between chronic obstructive pulmonary disease (COPD) and obstructive sleep apnea syndrome and the effectiveness of nocturnal noninvasive ventilation (or BPAP).Material and Methods: A total of 48 patients (40 men/8 women) among 2485 patients hospitalized with COPD diagnosis and 407 patients applied to our Sleep Disorders Center between January 2007 and December 2010 were investigated retrospectively to identify Overlap Syndrome patients. We divided the study population (99 men) into three groups. Group 1:OUAS patients (n=40) (AHI≥5events/hour), Group 2:COPD patients with OUAS (Overlap Syndome) (n=40) and Group 3: control group(n=19) with AHI<5events/hour).Results: The age of the patients included to the study ranged between 19-89 years. Overlap syndrome patients were older than others (p<0.01). The patients diagnosed as Overlap syndrome have been treating for 8.6 ±1.26 years with COPD and most of them were in Stage 2 (47.5%). All of them were overweight and smoking cigarette. Pulmonary hypertension was diagnosed in 29 patients with Overlap syndrome and in 5 patients with OUAS. The decreased sleep efficiency (p<0.05), REM (%), DI (p<0.01), Stage 3 (%) and increased Stage 2 (%) were found in Overlap syndrome patients. 60% of them had AHI≥30 events/hour (p<0.01).Pulmonary arterial pressure (p<0.05) and the number of exacerbations were decreased after BPAP treatments. The survival period was decreased with BMI being below 30 kg /m2 (p<0.05) and DI being above 30 events /hour (p>0.05).Conclusion: Coexistance of OUAS in patients with COPD deepens the nocturnal desaturations and affects the daytime hypoxemia and hypercapnia. The pulmonary hypertension caused by hypoxemia affects the prognosis negatively. We think that the BPAP therapy which reduced the number of exacerbations and pulmonary hypertension has positive effect on prognosis. Although the changes in sleep architecture of patients with Overlap syndrome were similiar with literatures, the present study is the first one to our knowledge to show the positive effect of BPAP. If it is thought that high BMI has a positive effects on the prognosis of COPD patients, the patients with Overlap syndrome should be investigated in detail.
