AAA hastalığına pyrin proteinini kodlayan MEFV genindeki mutasyonların neden olduğu ileri sürülmektedir. Genetik mekanizma iyi bilinmesine rağmen bu defektin nasıl olup da inflamatuvar ataklara neden olduğu tam olarak bilinmemektedir. Çalışmalarda AAA hastalarında farklı periferik hücrelerde apopitozis belirteçlerine ait farklı bulgular yer almaktadır. Bu da AAA gibi inflamatuvar hastalıkların patogenezinde diğer hipotezlerin yanı sıra apopitozis regülasyonundaki defektlerin de rolü olabileceğini düşündürmektedir. Mutasyon sonrasında yapısı değişen pyrin, inflamasyonun ve apopitozisin regülasyonunda yetersizliğe neden olabilir.Bu hastalıkta günlük kolşisin tedavisinin devam ettirilmesiyle tam remisyon ya da atakların süre ve sıklığında azalma hastaların çoğunda sağlanabilir. Bilinen etkilerine rağmen kolşisinin AAA ataklarını baskılamadaki mekanizması tam olarak bilinmemektedir. AAA’de patogenezde sorumlu olabilecek mekanizmalardan birisi olan nötrofillerdeki apopitozis üzerine kolşisinin etkileri net olarak bilinmemektedir.Biz bu çalışmamızda akut atakla başvuran AAA hastalarında nötrofillerdeki apopitozis belirteçlerini ve böylece apopitozisi değerlendirmeyi ve bu belirteçler üzerinden kolşisinin apopitozise etkisini araştırmayı planladık.Bu çalışma akut atak ile başvuran 7 hasta ile 7 sağlıklı kişide yapıldı.Apopitozis göstergeleri olarak kaspaz-3 ve kaspaz-9 çalışıldı. Daha sonra kolşisinin apopitozise olan etkisi değerlendirildi.Nötrofillerdeki apopitozis düzeyleri ve apopitozis belirteçleri AAA atak grubunda kontrol grubuna göre anlamlı oranda yüksek saptandı. Kolşisin apopitozis yolağını engellediği, apopitozis düzeyinlerinde ve apopitozis belirteçlerinde anlamlı bir azalma sağladığı görüldü.Nötrofillerdeki apopitozis düzeyleri AAA hastalarında akut atak döneminde artış göstermektedir. Kolşisin apopitozis yolağını baskılamakta ve apopitozis düzeylerini azaltmaktadır. Kolşisinin bir etki mekanizması da bu yolağın inhibisyonu gibi görünmektedir. Fakat koşisinin apopitozis yolağını hangi basamakta inhibe ettiği tam net değildir. Bu konuda daha ileri araştırmalara ihtiyaç vardır. It is claimed that FMF is caused by mutations in MEFV gene which encodes pyrin protein. Despite of well-known mechanism of this defect, it is not clearly known how FMF leads to inflammatory attacks. According to findings in several studies, FMF patients have various signs of apoptosis in different peripheric cells.This shows that, defects in apoptosis regulation may also play a role in the pathogenesis of inflammatory diseases like FMF. Mutations in pyrin may lead to failure in regulation of inflammation and apoptosis.Many patients can be treated by the long use of daily colchicine for completely remission or reduction of period and frequency of attacks. Despite of the effects of colchicine, it is not certainly known how colchicine prevents the FMF attacks. The colchicine effects on neutrophil apoptosis, which may be one of mechanisms responsible for pathogenesis of FMF, are not clearly known.In this study, we planned to investigate, the markers of apoptosis in neutrophils, apoptosis in general and the effect of colchicine on apoptosis by these markers, in AAA patients presenting with an acute attack.This study was performed on 7 patients presenting with an acute attack and 7 healthy persons. Caspase 3 and caspase 9 are studied as the markers of apoptosis.After that the effect of colchicin on apoptosis are evaluated The apoptosis rate and the markers of apoptosis in neutrophils are found to be higher than the control group in AAA attack group. It is estimated that colchicin causes inhibition of the apoptosis pathway, a decrease in the apoptosis rate and the apoptosis markers.The apoptosis levels are elevated in AAA patients during an acute attack.Colchicin causes supression of the apoptosis pathway and lowers the apoptosis levels. Another effect mechanism of colchicin looks like the inhibition of this pathway but which stage of apoptosis pathway is inhibited by colchicin is not clear.There is a need for further research about this subject.
