Kronik böbrek hastalığında endotel disfonksiyonu, hiperürisemi ve metabolik asidoz varlığı çeşitli çalışmalarla gösterilmiştir. Bunların geriye çevrilmesi hastalığın progresyonunun önlemesi bakımından önem taşımaktadır. Daha çok merak edilen nokta allopürinolün ; progresyonu önleme ve endotel disfonksiyonundaki rolüdür. Bu çalışmada KBH ve hiperürisemisi olan hasta grubunda allopürinolün metabolik asidoz, hastalığın progresyonu ve endotel fonksiyonlarına etkisi araştırıldı.Çalışma grubuna (n=30) evre 2-4 KBH ve serum ürik asit düzeyi 5,5 mg/dl ve üzeri olan hastalar alındı ve 300 mg/gün peroral allopürinol tedavisi 3 ay boyunca verildi. Kontrol grubuna (n=30) ise çalışma grubu ile benzer özelliklere sahip hastalar alındı ve allopürinol verilmedi. Hastaların endotel fonksiyonları ön kolda akıma bağlı dilatasyon ölçümü ile pH ve HCO3 düzeyleri venöz kan gazı, renal fonksiyonları ise GFH ve proteinüri hesaplanarak değerlendirildi.Her iki grubun erkek-kadın dağılımı, VKİ, yaş ortalaması, ürik asit, GFH, proteinüri, pH ve HCO3 değerleri benzerdi. Her iki grup karşılaştırıldığında 3 aylık takip sonunda allopürinol alan grupta akıma bağlı dilatasyonun almayan gruba göre belirgin arttığı görüldü (6,27 ± 2,70 ye karşın 5,71 ± 1,90, p=0,000). Ürik asit düzeyinin düşürülmesinin aynı zamanda GFH, HCO3 düzeyi ile de ters ilişkili olduğu saptandı.Sonuçta KBH ve hiperürisemisi olanlarda ürik asit düzeylerinin allopürinol ile düşürülmesinin endotel disfonksiyonu, hastalığın progresyonu ve HCO3 düzeyine olumlu katkısı olduğu bulunmuştur. Son dönem böbrek yetmezliğine gidişi yavaşlatmada ürik asitin düşürülmesinin endotel disfonksiyonunu düzeltip, metabolik asidozu önleyerek faydalı olduğu, progresyonun önlenmesi için yeni tedavi hedefi olacağı kanaatindeyiz. It has been shown the presence of endothelial dysfunction, hyperuricemia and metabolic acidosis in chronic kidney disease in various studies. Their regression has a crucial role for the prevention of disease progression. More curious part is the role of allopurinol in prevention of progression and endothelial dysfunction. In this study, we investigated the effects of allopurinol on metabolic acidosis, disease progression and endothelial functions in a patient group with CKD and hyperuricemia.The study group (n=30) was included the patients with stage 2-4 CKD and serum uric acid level 5,5 mg/dL and over. They were administered 300 mg/day per oral allopurinol treatment for 3 months. Control group (n=30) consisted of patients with similar characteristics as the study group and they were not administered allopurinol. Patients’ endothelial functions were measured via flow dependent dilatation over forearm, pH and HCO3 levels via venous blood gas and renal functions via GFR and proteinuria calculation.Men and women distribution, BMI, mean age, uric acid, GFR, proteinuria, pH and HCO3 levels were similar in both groups. In the comparison of both groups, flow dependent dilation significantly increased in allopurinol taking group compared to not-taking group following 3 month- follow up (6,27 ± 2,70 vs. 5,71 ± 1,90, p=0,000). It was also detected that there was a reverse relation between declining of uric acid level and GFR and HCO3.In conclusion, it was found that decreased uric acid levels with allopurinol in those with CKD and hyperuricemia have effects over endothelial dysfunction, disease progression and HCO3 level. We believe that decreasing uric acid levels is helpful to reform endothelial dysfunction, prevent metabolic acidosis, slow the progression to end stage kidney failure and will become a novel treatment target to prevent the progression.
