Metastatik kolorektal kanserli hastalarda tedavi etkinliğinin PET-BT ile değerlendirilmesidir. Bu amaçla çalışmaya 22 metastatik kolorektal kanserli hasta dahil edildi. 6 kür kemoterapi ve eşliğinde Bevacizumab tedavisi alan hastalar tedavi öncesi ve sonrasında tüm vücut PET- BT ile tarama yapılarak değerlendirildi.EORTC kriterlerine göre yapılan değerlendirmede hastaların %31.8’ inin tedaviye yanıt verdiği (tam cevap + kısmi cevap) %68.2 sinin ise tedaviye yanıt vermediği (stabil hastalık + progresif hastalık) tespit edildi. Metastatik odakların tedavi öncesi ve sonrası SUVmax değerlerine bakıldı. Tedavi öncesi karaciğer, ekstrahepatik, abdominopelvik, akciğer ve kemik metastazlarının ortalama SUVmax değerlerine göre tedavi sonrası ortalama SUVmax değerlerinde artış olduğu izlendi. Tedaviye cevap veren ve tedaviye cevap vermeyen hastalar kendi aralarında değerlendirildiğinde tedavi öncesi ve sonrası SUVmax değerlerinde tedaviye cevap verenlerde belirgin düşüş izlenirken, tedavi cevabı olmayanlarda artış gözlendi.Cinsiyet, histopatolojik alt tip, histolojik grade, primer tümör lokalizasyonu, ilk tanı anında karaciğerde ve bölgesel lenf nodlarında metastaz varlığı ile tedaviye yanıt arasındaki ilişki istatistiksel olarak anlamlı değildi.Çalışma süresince sağ kalan ve ex olan hastaların PET-BT’ de tespit edilen tedavi yanıtı ile ilişkisi değerlendirildiğinde tedaviye yanıt veren hastaların tamamının çalışma süresince sağ olduğu tespit edildi ve bu istatistiksel olarak anlamlı bulundu.Sonuç olarak kemoterapi eşliğinde Bevacizumab tedavisi alan metastatik kolorektal kanserli hastaların tedaviye yanıtını değerlendirmede, anatomik yanıt henüz oluşmadan metabolik yanıtın değerlendirilebilmesi ve ekstrahepatik uzak metastazların tespit edilebilmesi nedeniyle PET-BT’nin kullanılması prognozu belirlemede doğru bir yaklaşım olacaktır. The purpose of this study is to evaluate the effectiveness of treatment with PET-CT in patients with metastatic colorectal cancer who treated with Bevacizumab and chemotherapy. For this purpose, 22 patients with metastatic colorectal cancer were included in this study. The patients, treated with Bevacizumab along with 6 cycles of chemotherapy, were evaluated by whole-body PET-CT scan before and after treatment. In the evaluation that is performed in accordance with EORTC criteria, it is detected that the 31.8% of patients responded to treatment (complete response + partial response) and 68.2% of them did not respond to treatment (stable disease + progressive disease). SUVmax values of metastatic foci were evaluated before and after treatment. The mean SUVmax values were higher after treatment in comparison to the pre-treatment hepatic, extra hepatic, abdomen, lung and bone metastases mean SUVmax values. When patients who responded to the treatment and those who did not respond were compared separately among themselves for pretreatment and after-treatment SUVmax values, there were an increase in those who did not respond to the treatment while there were a decrease in those who responded to treatment.The relationship between gender, histological subtype, histological grade, primary tumor location, presence of metastases in regional lymph nodes and liver at the time of diagnosis and the response to the treatment was not statistically significant.When the treatment response observed in PET-BT was evaluated in patients who survived or died during the study period, it is found that all the patients who responded to the treatment were survived and it was at a statistically significant level.As a result, use of PET-BT would be an appropriate approach in determining prognosis in patients with metastatic colorectal cancer who were treated with Bevacizumab and adjuvant chemotherapy along with treatment response since it allows the evaluation of metabolic response before the development of anatomic response and also allows the detection of distant extra hepatic metastasis.
