Akustik travma en sık görülen on mesleki hastalıktan biridir. Hem iş gücü hem de finansal önemli kayıplara neden olur. Akustik travma kısa süreli yüksek şiddette sese maruz kalma sonucu ortaya çıkan geçici veya kalıcı işitme kaybı ile sonuçlanabilen bir durum olup gelişmiş ülkelerde işitme kayıplarının önemli nedenlerinden biridir. Etanercept TNF α reseptör blokörü olup, akustik travmada etanercept ile TNF α blokajı apoptozisi engellemektedir. İmmün mediatör salınımlı iç kulak hastalıklarında da etanercept ile tedavi edilen hastaların semptomlarında %50 düzelme görülmüştür. Çalışmamızda TNF alfa reseptör antagonisti olan Etanercept ile akustik travmanın faktörlerinden biri olan apoptozisi engellemeye çalıştık. Çalışmaya dahil edilen 38 rat dört eşit gruba ayrıldı. Öncelikle kontrol otoakustik emisyonları yapılarak ratların kohlear fonksiyonlarının normal olduğu teyit edildi. Birinci grupkontrol grubu, ikinci grup Etanercept1.25 grubu, üçüncü grup Etanercept 2.5 grubu, dördüncü grup ise Etanercept 5 grubu olarak belirlendi. Birinci gruba 1 ml serum fizyolojik ip., İkinci gruba 1.25mg/kg etanercept ip, üçüncü gruba 2.5 mg/kg etanercept ip, dördüncü gruba 5mg/kg etanercept ip. olarak akustik travmadan önce uygulandı. Otoakustik emisyon ölçümleri yapılan tüm ratlar ses geçirmez kabin içerisinde AC 40 model odyometri cihazı (Interacoustics Co. Assens, Danimarka) kullanılarak 40 dakika süre ile 4 kHz frekansında 120 dB saf ton sese maruz bırakıldı. Deney hayvanlarının 24. saatte, 3. günde ve 7. günde distorsiyon ürünü otoakustik emisyon (DPOAE) ölçümleri yapıldı. Deney hayvanlarının kuyruk veninden akustik travmadan önce ve travmadan hemen sonra olmak üzere 1 er cckan alındı. Hayvanların serumlarında sitokin düzeyleri ölçümü yapıldı. Çalışmamızda etanercept 2.5 ve 5 grubu kontrol grubuna göre 1., 3. ve 7. günlerde yapılan emisyon kontrollerinde bütün frekanslarda istatistiksel olarak anlamlı koruyuculukta çıkmıştır. Etanercept 1.25 grubu ise kontrol grubuyla karşılaştırıldığında; 1., 3. ve 7. günlerde yapılan emisyon kontrollerinde bütün frekanslarda istatistiksel olarak anlamlı bir iyileşme sağlayamamıştır. Ölçülen serum sitokin seviyelerinde ise Etanerceptin serum TNFalfa, IFNgama, IL1 seviyelerinde anlamlı bir değişikliğe neden olmadığı saptanmıştır. Etanerceptin serum IL6 düzeylerinin akustik travmaya bağlı olarak yükselmesini engellediği saptanmıştır. Etanerceptin iç kulakta kullanımı ile ilgili çok az çalışma olması ve çalışmamızda akustik travmadaki koruyucu etkisinin gösterilmesi bu konudaki çalışmaların artmasına katkıda bulunacağını umudumuzu güçlendirmektedir. Acoustic trauma is one of the most common on occupational diseases. Causes significant losses in both the labor and financial. Acoustic trauma as a result of short-term exposure to high intensity noise may result in temporary or permanent hearing loss is a condition in developed countries, is one of the major causes of hearing loss. Etanercept is a TNF alfa blocker and, is inhibited the apoptosis. İmmün inner ear disease symptoms in 50% of treated patients with etanercept showed a considerable improvement. In our study, TNF-alpha receptor antagonist, etanercept tried to prevent apoptosis is one of the factors for the acoustic trauma. Included in the study were divided into four equal groups of 38 rats. Initially, cochlear functions of the rats were proved to be normal by measuring otoacustic emmissions. The first, second, third and fourth groups were determined as control, etanercept1.25, etanercept2.5 and etanercept5 groups respectively. The first, second, third and fourth groups were intraperitoneal administered 1 mL isotonik saline, 1.25mg/kg etanercept, 2.5 mg/kg etanercept and 5 mg/kg etanercept.All the rats with normal otoacoustic measurement results were exposed to pure tone sound with the 4 kHz frequency and 120 dB intensity by an audiometry device of AC 40 model (Interacoustics Co. Assens, Denmark) within a sound proof cabin. Distortion product otoacoustic emissions (DPOAE) of experimental animals were measured at 24th hour, 3rd and 7th days.. In our study, etanercept 2.5 mg/kg, 5 mg/kg was found to be protected from acoustic trauma in 3, 4, 6, and 8 kHz frequencies at 1st, 3rd and 7th days measurement.However etanercept 1.25 mg/kg wasn‘t found to be protected in 3, 4, 6, and 8 kHz frequencies at 1st, 3rd and 7th daysmeasurement.In our measurement of serum cytokine levels etanercept does not cause significant change in serum TNFalfa, IL1 and IFNgama. In acoustic trauma etanercept was found to inhibit the rise of IL6. Very few studies on the use of etanercept in the inner ear and the study showing the protective effect of acoustic trauma studies on this issue strengthens our hope to contribute to the increase.
