Ayağın yoğun tedavi gerektiren doğumsal ortopedik deformiteler arasında en sık karşılaşılan hastalığı olan pes ekinovarus, doğru tedavi edilmediği taktirde ciddi bir morbidite ile sonuçlanmaktadır. Çalışmamızın amacı, bu olgulardaki Ponseti yöntemi ile tedavi sonuçlarımızı değerlendirmektir. Pes ekinovaruslu 30 hastanın 51 ayağını çalışmamıza dahil ettik. Hastalarımızı prospektif olarak ortalama 23,8 ay (dağılım 7-64 ay) takip ettik. Değerlendirmede Dimeglio ve Pirani sistemlerini kullandık. Haftalık seri alçılamaları sonunda gerekli gördüğümüz 28 ayağa ameliyathane şartlarında lokal anestezi altında perkütan aşilotomi yaptık ve son alçılarını uyguladık. Alçı tedavisi tamamlanan hastalarımızda 3-4 sene sürecek olan ortez ile idame tedavisine başladık. Pes ekinovarusun Ponseti yöntemi ile tedavisinde % 96 başarılı sonuç elde ettik. Geçmişte invaziv cerrahi ile tedavi edilen pes ekinovarusta ilk tedavi seçeneğinin Ponseti yöntemi olduğunu düşünüyoruz. Pes equinovarus, which is the most common congenital disease of the foot that needs intensive therapy, turns out to a serious morbidity unless it is treated correctly. The aim of our study is to evaluate the result of our treatment by Ponseti method in these cases. We included 51 clubfeet of 30 patients to our study. We followed prospectively our patients 23,8 months (range 7-64 months) on an average. We used Dimeglio and Pirani systems in evaluation. After serial casting weekly, we performed percutaneus achillotomy in operating room condition by local anesthesia to 28 feet that we consider them necessary and applied their last casts. We started maintenance treatment by orthesis in our patients whose casting treatment were completed that will go on 3-4 years. We achieved %96 of successful rate in the treatment of pes equinuvarus by Ponseti method. We consider the Ponseti method as the first option in the treatment of pes equinovarus which was treated by invasive surgery in the past.
