Görüntüleme yöntemleri rehberliğinde uygulanan perkütan transtorasik biyopsiler, toraks kitlelerinin tanısında ve tedavi yönteminin belirlenmesinde oldukça önemli role sahiptir ve daha invaziv cerrahi işlemlerin önemli bir alternatifidir. Görüntüleme yöntemi olarak BT daha yaygın kullanılmaktadır. En yaygın görülen komplikasyonları pnömotoraks ve parankimal hemorajidir. Bu çalışmada BT rehberliğinde yapılan transtorasik akciğer kitle biyopsilerinin tekniğinin, komplikasyonlarının ve komplikasyon gelişimini etkileyen risk faktörlerinin araştırılması amaçlanmıştır.İubat 2010-şubat 2013 tarihleri arasında patoloji sonuçlarına ulaşılabilen 85 hastada, BT rehberliğinde yapılan transtorasik biyopsilerin tekniği ve komplikasyonları retrospektif olarak değerlendirilmiştir. Dokuz hastada klinik gereklilik nedeni ile biyopsi işlemi tekrar edildiği için, 87'si tru-cut, 7'si İİAB olmak üzere, toplam 94 BT eşliğinde perkütan transtorasik biyopsi işlemi uygulanmıştır. Veriler ki-kare ve bağımsız örneklem T testi ile analiz edilmiştir.Çalışmamızda 94 biyopsi işleminin 79'unda diagnostik sonuç sağlanmış olup, patoloji sonuçları olguların 54'ünde malign, 3'ünde kuşkulu malign, 5'inde benign, 17'sinde nonspesifik benign olarak değerlendirilmiştir. Tru-cut biyopsilerin 59'unda ( %62,8 ) spesifik tanı elde edilmiş olup, İİAB yapılan olguların hiçbirinde spesifik tanıya ulaşılamamıştır.İşlem sırasında ve sonrasında 27 ( %28,7 ) pnömotoraks, 8 ( %8,5 ) parankimal hemoraji gelişmiştir. Pnömotoraks gelişimi ile lezyon boyutu (p: 0,040) ve geçilen parankim mesafesi (p: 0,001) arasında anlamlı korelasyon saptanmıştır. Parankimal hemoraji ile lezyon boyutu ve geçilen parankim mesafesi arasında istatistiksel olarak anlamlı farklılık saptanmıştır (p değeri sırasıyla 0,021 ve 0,008).Sonuç olarak; BT rehberliğinde uygulanan perkütan transtorasik biyopsilerin en yaygın komplikasyonları pnömotoraks ve parankimal hemorajidir. Lezyon boyutu ve biyopsi traktında geçilen parankim mesafesi komplikasyon gelişimini etkileyen en önemli faktörlerdir. BT rehberliğinde uygulanan PTB, toraks kitlelerinin histopatolojik tanısını elde etmede yüksek doğruluk oranına sahip, uygulanması kolay, risk faktörlerinin önceden bilinmesiyle gereksiz komplikasyonların önlenebileceği ve kabul edilebilir düşük komplikasyon oranlarıyla, daha invaziv olan cerrahi işlemlere duyulan ihtiyacı azaltan güvenilir bir yöntemdir. Transthoracic image-guided percutaneous biopsies play an important role in determinig the diagnosis and treatment modality of thoracic masses, which is an important alternative to more invasive surgical procedures. CT is widely used as a method of imaging. Pneumothorax and parenchymal hemorrhage are the most common complications. In this study we purposed to investigate Transthoracic CT-guided lung nodule biopsy technique, its complications and risk factors for the development of complications.Between February 2010-February 2013, in 85 patients whose pathology results could be obtained, CT-guided transthoracic biopsy technique and complications were evaluated retrospectively. In nine patients, the biopsy procedure was repeated because of the clinical requirement and 87 tru-cut, 7 FNA, CT-guided percutaneous transthoracic biopsy was performed in a total of 94 patients. Data were analyzed by Chi-square and independent sample t-test.In our study, the diagnostic results achieved in 79 of 94 biopsy procedure and pathology results were evaluated as malignant in 54 patients, suspected malignant in 3 patients, benign in 5 patients and benign nonspecific in 17 patients. Specific diagnoses were obtained from 59 patients (62.8%) in tru-cut biopsy but no specific diagnosis could be reached in FNA.27 patients (28.7%) developed pneumothorax and 8 patients (8.5%) developed parenchymal hemorrhage during and after the procedure. A significant correlation was obtained between the development of pneumothorax and lesion size (p = 0.040), and the distance that traversed parenchyma (p = 0.001). There was statistically significant difference between parenchymal hemorrhage and lesion size and distance from passed parenchyma (p values were 0.021 and 0.008 respectively). An increased incidence of parenchymal hemorrhage and pneumothorax was observed at small size and deep-seated lesions.Consequently; CT-guided percutaneous transthoracic needle biopsy has high accuracy rate at the acquisition to histopathological diagnosis of thoracic masses. Lesion size and the distance that traversed parenchyma on biopsy tract are the most important factors that influence the development of complications. It is practical and unnecessary complications can be avoided by knowing in advance the risk factors. It has acceptably low complication rates.
