Bu çalışmanın amacı sentetik sülfür birleşiği olan polipeptidlerin disülfür bağlarını yıkarak polipeptid yapının oluşumunu engelleyici özellik gösteren mesnanın topikal uygulaması ile endoskopik sinüs cerrahisinin başarısızlığı ve hastalığın nüksüne en sık neden olan adezyon oluşumu ve sinüs ostium stenozlarını önlemeyi amaçladık.Bu çalışmada 2.8-3.5 kg(ortalama 3.2 kg) ağırlığında 24 adet erkek Yeni Zelanda beyaz tavşanı kullanıldı. Hayvanlar üç gruba ayrıldı. 1. gruba operasyonu takiben ve postopertif 3. günde, 2. gruba operasyonu takiben 1ml %20'lik mesna pamuğa emdirilerek 5 dakika boyunca antrostomi bölgesine topikal uygulandı. 3. gruba operasyonu takiben salin solüsyon (%0.9NaCl) pamuğa emdirilerek 5 dakika boyunca antrostomi bölgesine topikal uygulandı.Maksiller antrostomi uygulamasının ardından ve 8 haftalık süreç sonrasında olmak üzere milimetrik ölçek antrostomi alanının yanına yerleştirilerek fotoğraflar çekildi. Çekilen bu fotoğraflar AutoCAD 2012 programına yüklenerek antrostomi alanları hesaplandı. Her tavşan için iki kez ölçülen (operasyon sonrası ve 8 hafta sonundaki) alanların farkı alınarak daralma yüzdeleri değerlendirildi.Histopatolojik inceleme için biyopsi örnekleri Hematoksilen-Eozin ve Masson-Trikrom boyaları ile boyandı. Histopatolojik skorlama sonuçlarına göre değerlendirildi.Çift doz mesna ve kontrol grubu arasında alan değişim değerleri açısından istatistiksel olarak anlamlı fark tespit edildi (p=0,046). Diğer grup kıyaslamalarında anlamlı fark tespit edilmedi (p=0,248 ve p=0,328). Ancak tek doz uygulamasında çift doza göre, tek doz uygulamasında kontrol grubuna göre daha çok daralma olduğu saptandı.Histopatolojik skorlama sonuçlarına göre MNHİ kontrol grubunda çift doz mesna grubundan istatistiksel olarak farklıydı (p=0,042). Diğer grup kıyaslamalarında anlamlı fark tespit edilmedi (p=0.301, p=0,178). İstatistiksel olarak anlamlı olmamakla birlikte MNHİ'nun kontrol grubunda belirgin olarak artış gösterdiği gözlendi.Fibrozis oluşumu açısından gruplar arasında istatistiksel olarak anlamı fark saptanmadı (p=0,329 p=0,146 p=0,639). Fibrozisin kontrol ve tek doz mesna grubunda çift doz mesna grubuna göre daha fazla olduğu gözlendi.Silya kaybı açısından çift doz ve tek doz mesna gruplarının kontrol grubu ile arasında anlamlı fark saptandı (p=0,022 ve p=0,046). Kontrol grubunda görülen silya kaybı diğer iki gruba göre daha fazlaydı. Ödem ve epitelyal hiperplazi açısından gruplar arasında fark saptanmadı.Yukarıdaki sonuçlar ve yan etki profilleri göz önüne alındığında edoskopik sinüs cerrahinde mesna kullanımın mitomisin-c ve steroidlere göre daha güvenilir olabileceği düşünülmektedir. Çalışmamızdaki verilerin değerlendirilmesi revizyon sinüs cerrahisi gereksinimini mesnanın azaltabileceği yönünde sonuçlar ortaya koymakla birlikte, bu konuda ileri klinik çalışmaların yapılması gereklidir. In this study we aimed to prevent maxillary antrostomy stenosis and adhesion that necessitate revision intervention most frequently in endoscopic sinus surgery. We used mesna which is a synthetic sulfur compound and belongs to a class of thiol compounds disrupting disulfide bonds of the mucous polypeptide chains. In this study, 24 young adult New Zealand white rabbits weighing between 2.8-3.5 kg (mean 3.2 kg) were used. Rabbits were randomized into three groups, with 8 rabbits in each. One ml %20 mesna was soaked to a cotton and applied for 5 minutes after operation and postoperative 3rd day to double dose mesna group. One ml %20 mesna was soaked to cotton and applied for 5 minutes after operation to single dose mesna group. The same procedure was repeated on the other maxillary antrostomy area using saline solution (%0.9 NaCl) and that side was called as control side.Intraoperative and postoperative 8th week antrostomies were exposed by opening with the removing of maxillary sinus walls and their pictures were taken. All pictures were loaded on AutoCAD 2012 program and their areas were measured objectively in an automated manner. Area differences were calculated for each rabbit and narrowing percentages were compared.Mucosa specimens of antrostomies were taken and immediately fixed in 10% formalin solution and embedded in paraffin wax and stained with hematoxylin and eosin, Masson’s trichrome for histopathological examination. The results were compared according to the histopathological scale.Double dose mesna and control groups were compared according to narrowing percentages of ostium, these results were statistically significant (p=0,046). Other groups were compared and the results were not statistically significant (p=0,248 and p=0,328). However, narrowing percentages in control group were higher than single dose mesna group, also in single dose mesna group result were higher than double dose mesna group. According to the histopathological scale, in control group MNHI were much more than in double dose mesna group and the results were statistically significant (p=0,042). Other groups were compared and the results were not significantly different (p=0.301, p=0,178). Despite of these results, MNHI in control group were much more than the other groups.There were no statistically difference in all groups according to fibrosis score (p=0,329 p=0,146 p=0,639). However fibrosis was much more in control and single dose mesna group.Loss of cilia score was higher in the control group than the each mesna groups and the the results were statistically significant (p=0,022 ve p=0,046). Comparion of all groups accroding to edema and epitelial hipertrofi was no difference.Our results showed that using mesna in endoscopic sinus surgery is more safety than mitomycin-c and steroids because of its effect and side effect profile. Also our results recieved that using mesna can reduce resion endoscoic sinus surgery. However, human studies should be performed following more comprehensive and ultrastructural studies on animals.
