KF, tüm sistemlerdeki egzokrin bezleri etkileyen, 2.500 canlı doğumda bir görülen, otozomal resesif geçiş gösteren beyaz ırktaki en yaygın ölümcül kalıtsal hastalıklardan biridir.KFTR geninin klonlanmasıyla, çok sayıda çalışma KF’de genotip fenotip arasındaki ilişkiye odaklanmıştır. Bu durum, atipik KF klinik bulguları gösteren, ter testi değerleri normal ve/veya arada olan yeni grup hastaların ortaya çıkmasına yol açmıştır.Çalışmamızdaki amaç klinik olarak KF düşündüren, ter testi sonuçları arada olan hastalarda fenotip-genotip ilişkisini ortaya koyabilmekti.Bu çalışmamızda, Süleyman Demirel Üniversitesi Tıp Fakültesi Pediatri Kliniği ve/veya Polikliniklerine Haziran 2010 ve Aralık 2013 tarihleri arasında başvuran, klinik olarak KF şüphesi uyandıran, ter testi sonuçları ile arada kalınan 46 hasta ve bu hastaların 47 kardeşi değerlendirildi.Çalışmaya alınan hastalardan bir tanesine, KFTR mutasyonu saptanmamasına rağmen atipik KF tanısı konuldu. Bir kardeşte de KF ile ilişkili klinik bulgu saptanmaması, ter testi sonuçları normal olmasına rağmen G542X gen mutasyonu saptandı.Bizim çalışmamız, atipik KF bulguları gösterdiği halde herhangi bir KFTR mutasyonu gösterilemeyen pek çok hastanın olabileceği veya bir mutasyonun saptanmasının o bireyde mutlaka hastalık olduğu anlamına gelmediği görüşünü desteklemektedir. CF is the most common fatal hereditary disease in the Caucasians, which is transmitted by autosomal recessive trait, effects exocrine glands of all systems and encountered in one of every 2.500 live births.Upon the cloning of the CFTR gene, a high number of studies emerged focusing on the relation between the genotype and the phenotype in CF. This led to the development of new patient groups that represent atypical findings of CF with normal and/or borderline levels of the sweat test.In the present study, we aimed to demonstrate the relation between phenotype and genotype in patients who are clinically considered to have CF and who have borderline levels of the sweat test.Forty six patients with clinically suspected CF and indefinitive sweat test results, who were referred to the Pediatric out-patient Clinics of the Suleyman Demirel University Medical Faculty between June 2010 and December 2013, and their 47 siblings were evaluated in this study.The patient diagnosed with Atypical CF had no CFTR gene mutation. G542X gene mutation was detected in one sibling, despite the absence of CF associated clinical findings and normal sweat test results.The results of the present study support that there may be patients with Atypical CF findings but no CFTR mutation, and presence of a mutation does not always imply that the patient has the disease.
